A cost-effective 3D-printed cement spacer reconstruction in a rare case of calcaneal Ewing sarcoma: a case report and surgical technique.

BACKGROUND: Ewing sarcoma (ES) accounts for 15% of primary bone tumors and is the second most common type seen in children and young adults between ages 5 and 20, with a male-to-female ratio of 1.5 to 1. Around one-third of patients have metastatic disease at diagnosis, while involvement of the foot is rare, accounting for only 5% of cases. For patients without metastases, treatment typically includes neoadjuvant chemotherapy, limb salvage surgery when possible, and adjuvant chemotherapy. Recent advances in 3D printing enable customized, patient-specific reconstruction in complex anatomical si

Primary pelvic extraskeletal Ewing sarcoma: a case report and comprehensive literature review of molecularly confirmed cases arising within the female genital tract.

INTRODUCTION: Primary extraskeletal pelvic Ewing sarcoma within gynecologic organs is extremely rare and current literature is limited to case reports or series. Definitive diagnosis requires molecular confirmation of the EWSR1 fusion gene which drives cancer proliferation. Current treatment is guided by the National Comprehensive Cancer Network (NCCN) guidelines for Ewing sarcoma of the bone. CASE DESCRIPTION: A previously healthy 21-year-old woman presented to her primary care provider with two weeks of malaise and pelvic symptoms. Imaging revealed a 10 cm heterogenous cystic and solid mass

Long-term outcome and relapse patterns in Ewing sarcoma patients with extensive lung/pleural metastases after a complete response to systemic therapy.

BACKGROUND: Ewing sarcoma (ES) is sensitive to systemic therapy, including chemotherapy and anti-angiogenesis Tyrosine Kinase Inhibitors(aaTKIs). However, the prognosis of patients with metastatic disease remains poor. Recurrence or distant metastasis after a complete response (CR) or near-CR due to systemic therapy is not rare. METHODS: We reviewed data from 187 ES patients between 2014-2019 treated at a single institute in China. Patients with extensive lung/pleural metastases (L/Pmeta) who had a CR or near-CR after first- or second-line chemotherapy with or without aaTKIs were retrospectiv

Comparison of clinical features and outcomes in patients with extraskeletal versus skeletal localized Ewing sarcoma: A report from the Children's Oncology Group.

BACKGROUND: The prognostic significance of having extraskeletal (EES) versus skeletal Ewing sarcoma (ES) in the setting of modern chemotherapy protocols is unknown. The purpose of this study was to compare the clinical characteristics, biologic features, and outcomes for patients with EES and skeletal ES. METHODS: Patients had localized ES and were treated on two consecutive protocols using five-drug chemotherapy (INT-0154 and AEWS0031). Patients were analyzed based on having an extraskeletal (n = 213) or skeletal (n = 826) site of tumor origin. Event-free survival (EFS) was estimated using t

Efficacy and feasibility of treating adults with paediatric chemotherapy protocols in Ewing sarcoma, rhabdomyosarcoma and osteosarcoma - A systematic review by the Australia and New Zealand Sarcoma Association clinical practice guidelines working party.

BACKGROUND: Ewing sarcoma (EWS), rhabdomyosarcoma (RMS), and osteosarcoma (OSS) are rare in adults; efficacy and feasibility of treating adults with standard paediatric protocols (PP) are uncertain as chemotherapy protocols have been extrapolated from a paediatric population. METHODS: A systematic review was conducted for EWS, RMS, and OSS using the Population, Intervention, Comparison, and Outcome (PICO) framework. PICO 1 compared adults treated with PP vs personalised "adult protocols" (non-PP). PICO 2 compared adults vs children treated with PP. PP was defined as: VDC/IE, VIDE for EWS; IVA

European guideline for imaging of primary paediatric and adult osteosarcoma and Ewing sarcoma: systematic review and joint statement by the FOSTER consortium, Euro Ewing Consortium, European Society of Paediatric Radiology, and the European Association of Nuclear Medicine.

Imaging is central to diagnosing, staging, evaluating treatment response, and guiding post-therapy surveillance in osteosarcoma and Ewing sarcoma, yet no international consensus defines optimal modalities or schedules. This European guideline, developed by 36 multidisciplinary experts, seeks to standardise imaging for research and enhance patient care. A systematic review identified 2026 studies, of which 13 met criteria, revealing marked heterogeneity in diagnoses, chemotherapy regimens, and imaging protocols. Evidence remains inconclusive regarding the optimal modality for detecting bone met

Radiation therapy dose escalation achieves high rates of local control with tolerable toxicity profile in pediatric and young adult patients with Ewing sarcoma.

BACKGROUND: Local control for patients with Ewing sarcoma (EWS) who present with large tumors are suboptimal when treated with standard radiation therapy (RT) doses of 54-55.8 Gy. The purpose of this study is to determine local control and toxicity of dose-escalated RT for tumors ≥8 cm (greatest diameter at diagnosis) in pediatric and young adult patients with EWS. METHODS: Eligible patients ≤30 years old with newly diagnosed EWS ≥8 cm treated with definitive conformal or intensity modulated photon, or proton radiation therapy techniques were included. All patients in the study received dose-

Adamantinoma-like Variant of Ewing Sarcoma in the Metatarsal Bone After Chemotherapy: Report of a Case Successfully Treated with Pedicled Osteocutaneous Fibular Transfer.

Adamantinoma-like Ewing sarcoma is a rare variant of Ewing sarcoma with histologic and immunohistochemical evidence of squamous differentiation. This variant most commonly occurs in the head and neck region with a few cases reported in the long bones of the limbs. It may be associated with poorer clinical outcome and could pose a diagnostic challenge, particularly if it occurs in older patients or as a metastatic lesion. We present a case of Ewing sarcoma in the metatarsal of an 11-year-old boy that manifested adamantinoma-like morphology after neoadjuvant chemotherapy. Chemotherapy has been r

Bone marrow examination in patients with Ewing sarcoma/peripheral primitive neuroectodermal tumor without metastasis based on 18F-fluorodeoxyglucose positron emission tomography/computed tomography.

Ewing sarcoma/peripheral primitive neuroectodermal tumor (ES/PNET) is an aggressive bone tumor. Bone marrow aspiration and biopsy (BMAB) has been recognized as the gold standard for assessing bone marrow status. While the latest guideline suggests the need to omit bone marrow aspiration in patients with no findings on 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) based on one retrospective report, there is no study using 18F-FDG PET/computed tomography (CT). We retrospectively reviewed 26 consecutive, previously untreated, ES/PNET patients. We compare the results of bone ma

The Importance of Surgery as Part of Multimodal Therapy in Rapid Progressive Primary Extraosseous Ewing Sarcoma of the Cervical Intra- and Epidural Space.

Primary extraosseous Ewing sarcomas (EESs) are an extremely rare pathological entity. Less than 32 cases have been reported in the literature. Here we report an uncommon case with very rapid progression in the cervical region with extra- and intradural involvement. We present a thorough review of the literature and discuss possible treatment modalities. The Medline database was searched using the search terms: Ewing sarcoma, extraosseus tumour, treatment, management, cervical spine. A previously healthy 29-year-old man complained of right-sided radiculopathy (C7). Magnetic resonance imaging sh

A rare case of Ewing's sarcoma primary of the cervical spine in a 1-year-old child: A case report and literature review.

INTRODUCTION AND SIGNIFICANCE: Ewing sarcoma is a rare and malignant bone tumor that can primarily affect the cervical spine. Symptoms may not always be present, but muscle weakness is a critical indicator of disease progression and necessitates urgent surgical intervention to relieve pressure on the spinal cord. CASE PRESENTATION: We present a case of a child who experienced sudden muscle weakness in the left upper limb. An MRI of the spine revealed a tumor in the cervical spine compressing the spinal cord. Urgent surgical intervention was performed, successfully removing most of the tumor.

Respiratory-gated bilateral pulmonary radiotherapy for Ewing's sarcoma and nephroblastoma in children and young adults: Dosimetric and clinical feasibility studies.

PURPOSE: Bilateral pulmonary radiotherapy in children and young adults aims to reduce the recurrence of lung metastases. The radiation field includes liver tissue, which is sensitive to even low radiation doses. We investigated the feasibility of respiratory gating radiotherapy using voluntary deep inspiration breath hold and its toxicity in these patients. PATIENTS AND METHOD: A retrospective clinical review was conducted for all patients who had undergone bilateral pulmonary radiotherapy, with or without deep inspiration breath hold, treated in our institution between October 1999 and May 2

A Unique Case of Primary Ewing's Sarcoma of the Cervical Spine in a 53-Year-Old Male: A Case Report and Review of the Literature.

Extraskeletal Ewing's sarcoma (EES) is a rare presentation, representing only 15% of all primary Ewing's sarcoma cases. Even more uncommon is EES presenting as a primary focus in the spinal canal. These rapidly growing tumors often present with focal neurological symptoms of myelopathy or radiculopathy. There are no classic characteristic imaging findings and thus the physician must keep a high index of clinical suspicion. Diagnosis can only be definitively made by histopathological studies. In this report, we discuss a primary cervical spine EES in a 53-year-old man who presented with a two-m

The XAGE family of cancer/testis-associated genes: alignment and expression profile in normal tissues, melanoma lesions and Ewing's sarcoma.

The existence of XAGE genes was first reported after database homology searches for PAGE-like sequences identified 3 XAGE EST clusters. One of these clusters, XAGE-1, has in later studies been identified as a cancer/testis-associated gene. Here, we report the expression profiles of all 3 reported XAGE genes, as well as several splice variants of XAGE-1, in normal human tissues, Ewing's sarcoma and melanocytic tumors. We also provide the genetic structure of the corresponding genes. Moreover, by searching the databases for XAGE homologues, we identified 3 additional GAGE-like genes. RT-PCR stud

A case with extraosseous Ewing's sarcoma: a late effect related to bone marrow transplantation for thalassemia or a component of a familial cancer syndrome?

Allogeneic bone marrow transplantation has proved to be a radical form of cure in patients with beta-thalassemia major who have a human leukocyte antigen identical donor. Although malignant neoplasms are serious late complications of bone marrow transplantation, very few reports describing the development of malignant tumors after allografting for thalassemia appeared in the literature. A case is presented here of extraosseous Ewing's sarcoma that developed 8 years after allogeneic bone marrow transplantation performed for beta-thalassemia major. The phenotypic features of the patient's family

Current Practices and Variations in the Use of FDG-PET and Bone Marrow Biopsy in Ewing Sarcoma Diagnosis: A Survey of Provider Recommendations.

INTRODUCTION: Ewing sarcoma (ES) is the second most common malignant bone and soft tissue tumor in children, adolescents, and young adults. Current consensus guidelines, consistent with Children's Oncology Group practice, support the universal use of full-body fluorodeoxyglucose positron emission tomography (FDG-PET) and recommend reserving bone marrow aspiration and biopsy for cases in which FDG-PET raises concern for marrow involvement, leaving final decisions to the treating physician. Although bone marrow metastasis is defined by morphologic evidence on biopsy rather than imaging alone, th

Identification of a novel translocation producing an in-frame fusion of TAF15 and ETV4 in a case of extraosseous Ewing sarcoma revealed in the prenatal period.

Ewing sarcoma (ES) is a highly malignant round cell sarcoma, characterized by gene fusion involving FET (FUS, EWSR1, TAF15) and ETS family genes, respectively. The involvement of the EWSR1 gene has been reported in approximately 90% of cases of ES, with the EWSR1::FLI1 fusion being the most frequent. We report the case of a newborn with a localized soft tissue paravertebral neoplasm diagnosed prenatally. Histopathology and immunophenotype were consistent with a CD99 + , NKX2.2 + undifferentiated round cell sarcoma (URSC); whole-exome RNA-sequencing demonstrated an undescribed in-frame TAF15::E
