INTRODUCTION: Avascular necrosis (AVN) of the proximal pole of the scaphoid is a well-recognized complication of scaphoid fractures, particularly when diagnosis or treatment is delayed. Due to the scaphoid's retrograde blood supply, the proximal pole is highly susceptible to ischemia, often necessitating surgical intervention to prevent long-term complications such as scaphoid nonunion advanced collapse and wrist osteoarthritis.

OBJECTIVE: Pseudoarthrosis and necrosis of the proximal pole of the scaphoid require complex treatment. If primary treatments fail, there are only techniques that sacrifice part of the mobility. We present the design and in vitro results of an anatomical partial prosthesis of the scaphoid bone.

We investigated a new means of diagnosing occult scaphoid fractures. Eighty-six patients underwent vibratory testing at presentation, while the clinical examination and standard four-view x-ray examination findings were unknown to the persons who performed the vibratory testing of both the injured and uninjured wrists. In De Quervains dislocation fracture pseudarthroses of the scaphoid bone following closed reduction are very common, 59% being reported in a former publication. Exact adaptation of the fragments after reduction of the perilunar dislocation is essential for healing of the fracture. In cases where this is not possible by conservative means, open reduction and screwing of the scaphoid fracture is necessary for better result with a rate of pseudarthroses of only 16%.

Scaphoid fractures predominantly affect young men, in whom the UK smoking prevalence approaches 40%. We examined the association between smoking and failure of non-vascularized bone grafting and screw fixation for scaphoid non-union and delayed union in a retrospective cohort study. Radioscapholunate arthrodesis is a desirable method to treat isolated radiolunate arthritis or ulnar translocation of the carpal bones. The aim of this review was to present currently available evidence on the management of acute scaphoid fractures. Acute scaphoid fractures are usually diagnosed by a combination of history, physical examination, and radiography. However, in many patients scaphoid fractures are still missed. Thus, the general trend is to over-treat patients with a suspicion of scaphoid fracture. Many aspects of scaphoid fracture management are still controversial and different institutions vary in their approach.

Forty-four fractures of the scaphoid bone were treated with a short-term removable orthoplast cast and compared with 48 fractures treated with a conventional long-arm plaster cast. At the follow-up, there was no difference between the two treatment groups as regards nonunion or other sequelae. We conclude that the inconvenience of the treatment of scaphoid fracture and the need of physiotherapy can be reduced by using an orthoplast cast.

The presence of a concomitant hook of hamate fracture is of particular relevance given that it is often occult on routine wrist/scaphoid radiographs and that hook of hamate fractures are prone to symptomatic non-union, resulting in chronic ulnar wrist pain.
