Editorial Commentary: Remplissage Is Not Needed When Performing the Latarjet Procedure in the Setting of Off-Track Hill-Sachs Lesions: One of the Classics Continues to Get Better With Age (and Some Help From New Data).

The glenoid track paradigm has played a pivotal role in the development of a contemporary approach to shoulder stabilization surgery. A number of studies have validated the importance of bipolar bone loss in the pathogenesis of recurrent anterior shoulder instability and defined the limitations of soft tissue-based, arthroscopic surgical approaches. The glenoid track paradigm has also helped to clarify the indications for coracoid transfer procedures, although some question persists as to the necessity of addressing an "off-track" Hill-Sachs lesion with remplissage concurrently with the Latarj

Editorial Commentary: Arthroscopic Latarjet: An Analysis of Outcomes and Complications Through its Learning Curve.

The arthroscopic Latarjet has been proposed in the past decade to reduce the rate of bone block malpositioning, decrease soft-tissue damage associated with open approach, and possibly decrease intraoperative complications. Several recent studies have reported similar or even superior results with arthroscopic Latarjet compared with those achieved with open technique. However, arthroscopic Latarjet is known to be more technically demanding and some feel that it should be performed by expert shoulder surgeons with advanced arthroscopic skills. Surgical innovations should be adopted when they hav

Editorial Commentary: Computed Tomography-based Analysis of the Arthroscopic Latarjet Procedure Suggests Graft Positioning Is More Variable than Expected.

The Latarjet operation is one of the most successful techniques for the treatment of recurrent anterior shoulder instability, which has recently been translated into the arthroscopic field. Several studies suggest that its arthroscopic variant is safe and reproducible and report promising short-term and mid-term results. Some authors claim that, in addition to less soft tissue damage, positioning of the coracoid graft is more accurate during arthroscopic control. There is currently no evidence of superiority of the arthroscopic or the open technique. As with the open procedure, the arthroscopi

Early Outcomes and Perioperative Complications of the Arthroscopic Latarjet Procedure: Systematic Review and Meta-analysis.

BACKGROUND: The arthroscopic Latarjet-Bristow procedure is emerging as a reliable alternative to the open procedure. The reduced soft tissue damage with potential advantages of early pain control and functional recovery is attractive. However, the operation is technically more demanding, and there are concerns regarding the potential for increased recurrence and complication rates. PURPOSE: To evaluate the available literature focusing on the reported functional outcomes and complications of the arthroscopic Latarjet procedure and compare them with the open procedure. STUDY DESIGN: Systemati

Glenohumeral Instability Related to Special Conditions: SLAP Tears, Pan-labral Tears, and Multidirectional Instability.

Glenohumeral instability is one of the more common conditions seen by sports medicine physicians, especially in young, active athletes. The associated anatomy of the glenohumeral joint (the shallow nature of the glenoid and the increased motion it allows) make the shoulder more prone to instability events as compared with other joints. Although traumatic dislocations or instability events associated with acute labral tears (ie, Bankart lesions) are well described in the literature, there exists other special shoulder conditions that are also associated with shoulder instability: superior labru

Evaluation of Oxford instability shoulder score, Western Ontario shoulder instability index and Euroqol in patients with SLAP (superior labral anterior posterior) lesions or recurrent anterior dislocations of the shoulder.

BACKGROUND: Having an estimate of the measurement error of self-report questionnaires is important both for assessing follow-up results after treatment and when planning intervention studies. Specific questionnaires have been evaluated for patients with shoulder instability, but not in particular for patients with SLAP (superior labral anterior posterior) lesions or recurrent dislocations. The aim of this study was to evaluate the agreement, reliability, and validity of two commonly questionnaires developed for patients with shoulder instability and a generic questionnaire in patients with SLA

[Pathomorphology of shoulder instability].

In cases of a traumatic anterior first-time dislocation of the shoulder, pathomorphological changes may initially occur at three different sites: at the capsule itself, at its origin or at its insertion. The typical injury is an avulsion of the labrum and the capsule from the glenoid and is called a Bankart lesion. There is a tendency to underestimate the amount of plastic deformation of the capsule and alternative injuries, such as avulsion of the capsule from the humeral head (HAGL lesion). Bony deformities at the humeral head or at the glenoid are of utmost importance for the prognosis of s

[Open surgical treatment of recurrent anterior instability of the shoulder].

Recurrent anterior gleonohumeral instability is the most frequent joint instability of the body. Because of the complex stability mechanisms and diverse instability patterns of the glenohumeral joint, most cases present with more than one anatomic cause. Thus, the treatment of recurrent anterior instability of the shoulder should be designed to treat these pathologies. Although arthroscopic repair has outweighed the use of open surgical methods especially for the first dislocations, recurrent dislocations still require open repair techniques to overcome capsular laxity accompanying a Bankart l

[Open surgical treatment of posterior instability of the shoulder].

Posterior instability of the shoulder remains a diagnostic and therapeutic challenge, with a growing incidence especially in athletes. Evaluation of the type of the instability, whether unidirectional or multidirectional, is very important with regard to treatment. Although many patients benefit from strengthening and balancing rotator cuff muscles and scapular stabilizers, surgical procedures may be required in those unresponsive to conservative treatment. Posterior capsular shift is the method of choice to eliminate redundancy of the posterior and inferior capsules; moreover, in the presence

Arthroscopic treatment of anterior shoulder instability.

In the late 1980s and early 1990s, several advances in arthroscopic shoulder stabilization procedures were made. However, these early techniques were associated with higher failure rates compared with the traditional open procedures and therefore were not widely disseminated throughout the orthopaedic community. Because of improved understanding in anatomy and pathology in conjunction with dramatic technologic advances, the past decade has demonstrated significant improvement in the arthroscopic management of the unstable shoulder Evolution of arthroscopic techniques over the past 5 years to m

How to identify and calculate glenoid bone deficit.

In recent years, we have seen increased interest in bone lesions of the glenoid rim as acute fractures (Bony-Bankart) and as chronic bone defect in instability. This derives from three main clinical and statistical findings: a significant incidence of bony Bankart lesion after a first dislocation, a high percentage of glenoid bone defects in chronic instability, and, finally, a close relationship between bone defect and incidence of recurrence after arthroscopic stabilization. The authors agree on determining glenoid bone defect that exceeds 15-20% as the main contraindication to arthroscopic

Cineradiographic studies with shoulder instabilities.

Thirty patients with shoulder disorders were examined with fluoroscopy under general anesthesia. Prior to this cineradiography, 20 shoulders had diagnoses of instability and 10 had diagnoses of other stable-shoulder disorders. Three patients (10%) had changes in their shoulder diagnoses after imaging: 1) One patient with an impingement syndrome also had an unstable shoulder; 2) One patient suspected of having anterior subluxations prior to imaging, demonstrated posterior subluxations instead with imaging; and 3) One patient with recurrent posterior dislocations demonstrated a multidirectional

Design and mechanical properties analysis of drill pipe's joint thread with unequal taper under complex loads.

The design of drill pipe joint thread with unequal taper is proposed to investigate the fracture failure of the API NC38 used in the drill p … mpared to SUT. The maximum Mises stress value of DUT-I is reduced by 34.87% compared to NC38 double-shoulder joint thread (DUT). This design approach can guide the optimization of other API threads and enhance joint strength for non-API and specialized taper threads.

Nontyphoidal salmonella septic arthritis in A patient with systemic lupus erythematosus: A case report.

We report a case of septic arthritis in a 43-year-old female patient. Despite initial treatment with ceftriaxone for Nontyphoidal Salmonella based on blood and joint fluid culture results, the shoulder joint pain worsened. Suspected systemic lupus erythematosus associated synovitis did not respond to immunosuppressive therapy including methylprednisolone, hydroxychloroquine and methotrexate. Subsequent radiograph revealed a shoulder joint abscess, leading to arthroscopic joint debridement. Ceftriaxone was administered post-operatively until analgesic efficacy was attained. This case highlights…

Hill-Sachs defects and repair using osteoarticular allograft transplantation: biomechanical analysis using a joint compression model.

BACKGROUND: Humeral head defects have been associated with failed anterior shoulder instability repairs. Quantitative data are required to determine (1) the critical defect size for consideration of surgical repair and (2) the ability of proposed repair techniques to restore normal joint function. HYPOTHESES: Increasing defect size will decrease stability and anterior translation before dislocation. Stability will decrease in shoulder positions where the defect is oriented in line with the anterior glenoid. Osteoarticular repair will restore joint stability to intact shoulder level. STUDY DE
