BACKGROUND: The longevity of total shoulder replacement is primarily limited by the performance of the ultrahigh-molecular-weight polyethylene (UHMWPE) glenoid component in vivo. Variations in glenoid design (conformity, thickness), biomechanics (joint kinematics), and UHMWPE material selection (sterilization, cross-linking) distinguish total shoulder replacements from hip and knee arthroplasty devices. These variables can lead to severe mechanical failures, including gross fracture.
METHODS: Sixteen retrieved glenoids with severe fracture were analyzed. The explant cohort included 3 material

BACKGROUND: The diagnosis of infection after shoulder arthroplasty can be challenging. The current study evaluated the utility of a prerevision biopsy sample in predicting positive cultures or a final diagnosis of infection in the setting of an "at-risk" failed shoulder arthroplasty.
METHODS: The study reviewed 77 patients with no history of infection undergoing revision shoulder arthroplasty by a single surgeon between June 2010 and July 2015. All patients with a C-reactive protein and erythrocyte sedimentation rate within normal reference ranges and no fluid on aspirate, or an abnormal valu

BACKGROUND: Reverse shoulder arthroplasty (rTSA) is increasingly used as a revision option, but long-term survivorship and patient-reported outcomes (PROs) after revision rTSA remain poorly defined. We hypothesized that revision rTSA for failed rTSA would have inferior survivorship and PROs when compared to revision rTSA for failed anatomic total shoulder arthroplasty (aTSA) or failed hemiarthroplasty (HA). Moreover, we hypothesized that shoulders undergoing secondary revision would have a higher risk of failure when compared to those undergoing first-time revision surgery.
METHODS: This stud

BACKGROUND: This study evaluated patients with and without a prosthetic dislocation after reverse total shoulder arthroplasty (RTSA) to identify risk factors for instability.
METHODS: Dislocation and nondislocation cohorts were established for analysis in 119 patients who had undergone RTSA at our institution between 2011 and 2014. Preoperative history and parameters pertaining to RTSA design were evaluated for correlation with instability. A logistic regression model was used to analyze independent predictors.
RESULTS: Eleven patients (9.2%) demonstrated instability in the early postoperati

Humeral-sided arthroplasty design evolution continues to be supported by the published literature in the transition away from traditional stemmed devices. Early studies have shown not only absence of failure with these newer shorter and stemless designs but also equivalence in terms of early outcomes compared to traditional stemmed devices with the benefits of shorter operative time, less blood loss, easier revision, and the potential to reduce stress shielding and periprosthetic fractures. We will review the literature available on the different designs of both short stem and stemless humeral

BACKGROUND: Orthopaedic implants and osteosynthesis materials are increasingly being used. Complications include mainly physical-mechanical problems and infections. Uncommonly, an allergic reaction towards an alloy metal or a bone cement component has been implicated. Potential bone cement allergens include acrylates, benzoyl peroxide, N,N-dimethyl-p-toluidine, and gentamicin. Typical symptoms are pain, swelling, inflammatory skin reactions, implant loosening, and fistula formation.
OBJECTIVES: To report on 5 patients with complications from a knee or a shoulder joint implant in whom a releva

Surgical treatment is difficult in elderly patients who have suffered a comminuted fracture of the head of the humerus, often associated with dislocation. We performed follow-up examinations in our hospital in 11 patients after implantation of 12 endoprostheses since July 1990. Very good results were confirmed in respect of regained freedom of movement and overall high personal satisfaction on the part of the patients. Endoprosthesis appears to us as the treatment of choice in complex fractures of the head of the humerus in patients of advanced age.

Developed in 1985, the Grammont-style reverse total shoulder arthroplasty offered a biomechanical advantage for the deltoid muscle as well as predictably reduced pain and improved shoulder function in rotator cuff-deficient shoulders. Despite favorable outcomes, reverse total shoulder arthroplasty is associated with a unique set of complications, one of which is scapular notching. Scapular notching is believed to be a result of mechanical impingement of the humeral component on the lateral scapular pillar. Although it appears that scapular notching progresses with time, its effect on implant s

BACKGROUND: Prosthetic shoulder arthroplasty provides excellent pain relief and functional restoration for patients with glenohumeral arthritis, but concerns of survivorship have limited its use in younger patients.
DISCUSSION: Despite general reports of high long-term survivorship, implant failure and functional deterioration after total shoulder arthroplasty are major concerns in the management of younger patients. In addition to having a longer life expectancy, younger patients also tend to be more active and can be expected to place greater demands on their shoulder arthroplasty.
CONCLUS

To obtain more information on the pattern of damage of prosthetic glenoid components, we analyzed 7 retrieved glenoid components. The consecutive series included 2 standard polyethylene components and 5 highly crystalline polyethylene glenoids (Hylamer; DePuy Dupont Orthopaedics, Warsaw, IN) retrieved 3 to 12 years after implantation. At revision, 4 of 5 Hylamer components were fractured. Common wear patterns were i) deformation and crumbling of the rim, particularly at the inferior hemicircumference, probably caused by direct contact of the humerus with the prosthetic component; ii) roughenin

Between 1982 and 1985, 23 Kessel total shoulder arthroplasties were performed on 22 patients with rheumatoid arthritis. A clinical and radiographic review of the 5-year experience was evaluated in 1988 and published in 1992. Ten years later a followup study was done. Of the 22 patients, 11 have died and two were seriously ill and could not participate. Of the remaining nine patients, one had revision surgery after 2 years, leaving eight patients with a mean age of 61 years for this followup study. Shoulder function was evaluated subjectively using the Simple Shoulder Test, and pain was evaluat

BACKGROUND: Cementless reverse shoulder arthroplasty has become increasingly popular because of the improved implant design, porous ingrowth surface, and surgical techniques. When avoiding the risks of cement use, a press-fit arthroplasty stem that has been implanted may not feel immediately stable, especially if the medullary canal size is in between standard stem diameters. To help surgeons improve fixation and avoid overstuffing the medullary canal, we present the matchstick autograft augmentation technique. The use of humeral autograft, analogous to impaction grafting in hip arthroplasty,

In two-stage revision of infected implants, the first stage involves removing the implant and implanting a joint spacer, and the second stage involves implanting a new prosthesis at least 6 weeks later. Spacers have two main functions: local administration of high-dose antibiotics, and preservation of the joint space by reducing soft tissue retraction and improving patient comfort until reimplantation. The present review aims to detail the necessary characteristics of antibiotics added to cement to achieve good joint diffusion, to describe the steps of two-stage revision, and to present the ty

Automation of wrist rotations in upper limb prostheses allows simplification of the human-machine interface, reducing the user's mental load and avoiding compensatory movements. This study explored the possibility of predicting wrist rotations in pick-and-place tasks based on kinematic information from the other arm joints. To do this, the position and orientation of the hand, forearm, arm, and back were recorded from five subjects during transport of a cylindrical and a spherical object between four different locations on a vertical shelf. The rotation angles in the arm joints were obtained f

Joint dysplasias always represent a great challenge for prosthetic surgeons. The common altered anatomical landmarks and the subversion of the anatomy of soft tissues surrounding the dysplastic joint are problems that can cause difficulties if approached with standard methods. Together with the resolution of functional issues related to dysplasia, the understanding of the underlying cause is fundamental. DNA analysis is generally performed via blood sampling; however, this might lead to misdiagnosis in case mosaicism is not detected in blood components. The etiology of genetic diseases can be

Despite great innovations in upper-extremity prosthetic hardware in recent decades, controlling a multiple joint upper limb prosthesis such as an elbow/wrist/hand system is still an open clinical challenge, in large part due to an insufficient number of control inputs available to users. While simultaneous control is in its early stages, the common control approach is sequential control, in which joints and grasps are driven one at a time. In this paper, we introduce and evaluate a concept we call trajectory control, that builds upon this approach, in which motions of the wrist, elbow, and sho

Hemiarthroplasty implantation for proximal humerus fracture is challenging. In case of young demanding patients with functional rotator cuff, humeral head replacement is an option if open reduction internal fixation is not a viable solution. The criteria for success in terms of clinical outcomes and longevity are anatomic healing of the tuberosities and stem positioning (restoring correct height, version, and axis). The purpose of this article is to describe a technique including a new stem guide and a tuberosity anchor "lasso" system for the implantation of hemiarthroplasty in complex proxima

AIM: To develop a Web-based self-care program for patients after total laryngectomy according to a participatory design approach.
METHODS: We conducted a needs assessment with laryngectomees (n = 9) and their partners (n = 3) by means of a focus group interview. In 4 focus group sessions, a requirement plan was formulated by a team of health care professionals (n = 10) and translated into a prototype. An e-health application was built including illustrated information on functional changes after total laryngectomy as well as video demonstrations of skills and exercises. Usability of the proto

We retrospectively reviewed 11 consecutive patients with an infected reverse shoulder prosthesis. Patients were assessed clinically and radiologically, and standard laboratory tests were carried out. Peroperative samples showed Propionbacterium acnes in seven, coagulase-negative Staphylococcus in five, methicillin-resistant staphylococcus aureus in one and Escherichia coli in one. Two multibacterial and nine monobacterial infections were seen. Post-operatively, patients were treated with intravenous cefazolin for at least three days and in all antibiotic therapy was given for at least three mo…
