Depression and the reporting of diabetes symptoms.

The relationship of diabetes symptoms to current mood and general metabolic control was studied. Symptoms commonly associated with poorly controlled diabetes (e.g., thirst, polyuria, weight loss) were measured in 114 patients with diabetes mellitus (type 1 = 57, type 2 = 57). Scores for these individual symptoms were correlated with glycosylated hemoglobin (HbA1) and depression as measured by the Beck Depression Inventory (BDI). HbA1 was poorly correlated (r less than 0.2) with nine of the eleven symptoms and made a significant independent contribution only to the reporting of polyuria (p = 0.

Living with an amputation: the helper.

One hundred and nine key helpers were interviewed in connection with a study of the psychosocial implications of amputation. They were found to carry formidable burdens. Social isolation was a major problem which increased over time and was associated with diminished capacity to express needs. The respondents discussed emotional problems readily. Social workers had little sustained contact with helpers: welfare agencies had intervened mainly to provide practical help. The Artificial Limb Centre did not routinely see helpers or communicate with General Practitioners. A more realistic approach t

Epidemiologic investigation of a silage-associated epizootic of ovine listeric encephalitis, using a new Listeria-selective enumeration medium and phage typing.

The role of silage feeding in the origin of an epizootic of encephalitic listeriosis in a sheep flock was investigated by use of a new direct Listeria-selective isolation and enumeration medium, in combination with serotyping and phage typing. The silage contained high numbers (about 10(6) cells/g) of a L monocytogenes strain indistinguishable with respect to serovar and phagovar from that isolated from the brains of sick sheep. These results provided unambiguous bacteriologic evidence of the epidemiologic link between silage consumption and listeriosis in ruminants.

Narrating dementia: self and community in an online forum.

In this article, I examine how individuals diagnosed with early-onset Alzheimer's disease use illness narratives to construct community. The ability to narrate is a fundamental component of the self. Using 354 posts by 32 members of an Internet forum, I argue that people with Alzheimer's, whose ability to narrate, and thus create a self, was compromised, nonetheless managed to tell stories of redemption out of which a salvaged self emerged. Narratives are essential for the construction of self, but as I show in this article, they are also essential for the construction of community. Forum memb

Personality disorders are not associated with nonrecovery in patients with traffic-related minor musculoskeletal injuries.

BACKGROUND: Personality disorders (PDs) have been suggested to be one of the determinants that might influence recovery after injuries but has rarely been measured. This study describes the occurrence of PDs among patients with minor traffic-related musculoskeletal injuries and relates these disorders to nonrecovery 12 months after the injury. METHODS: This is a single-center, prospective, cohort study. We included patients with minor traffic-related musculoskeletal injuries at a general hospital in Stockholm, Sweden, with a catchment area of 0.6 million people. Structured Clinical Interview

Psychotherapy with a narcissistic playboy facing the end of his life: a self-psychology and object relations perspective.

Narcissistic Personality Disorder is considered to be one of the most tenacious and stable types of personality organization. It usually presents a challenge to clinicians and is often resistant to treatment. The continuous search for the affirmation by others of the grandiose self and the devaluation of others in an attempt to stabilize their self-esteem is typically seen in individuals with narcissistic personality organization. On the other hand, corrective life events such as personal achievements, long-term nurturing relationships, and the management of loss and disillusionment can contri

The Anger Expression Scale for Children: initial validation among healthy children and children with cancer.

OBJECTIVE: To evaluate the proposed structure of the Anger Expression Scale for Children (AESC) in samples of healthy children and those with cancer, and to examine correlations between AESC subscales and other indicators of anger and hostility. METHOD: A total of 803 children from two independent studies of healthy and ill children (mean age = 12.7, SD = 3.1) completed the AESC and other measures of anger expression and hostility, and a sub-sample of 298 of their parents completed measures of anger expression and hostility. RESULTS: Results provided initial support for the proposed four-fac

Anxiety in the eating disorders: understanding the overlap.

This paper reviews research investigating the comorbidity between eating disorders and anxiety disorders. Whilst there is some inconsistency in the literature, it appears that women with eating disorders have higher rates of anxiety disorders than normal controls. Potential causal relationships between eating disorders and anxiety disorders are outlined, though their relative chronology appears to be somewhat inconsistent. Safety behaviours and cognitive avoidance strategies (i.e., cognitive narrowing and blocking) are suggested as potential mechanisms linking the disorders. A model outlining

Disclosure of an HIV diagnosis to children: history, current research, and future directions.

Disclosing the diagnosis of human immunodeficiency virus (HIV) or AIDS to a child is a controversial and emotionally charged issue among both the health care communities and parents and caregivers of these children. This paper provides a systematic review of research on disclosure of pediatric HIV infection. It begins with a brief discussion of disclosure drawing from research on pediatric cancer. Next, we review the available research including patterns of disclosure, factors associated with disclosure and nondisclosure, and the effect of disclosure on psychological health and adherence. Health-related quality of life in adolescents with bipolar I disorder.

OBJECTIVE: To examine abnormalities in health-related quality of life (HRQOL) and to determine whether pharmacological intervention with divalproex or quetiapine alters HRQOL abnormalities in bipolar adolescents. METHOD: Parents of 23 adolescents diagnosed with a manic or mixed episode associated with bipolar I disorder were asked to rate their child's health using the Child Health Questionnaire-Parental Form 50 (CHQ-PF 50) at baseline, prior to receiving medication and then at 28 days later. RESULTS: Manic adolescents exhibited significantly worse HRQOL than national norms at baseline on al

Psychosomatic characterization of adjustment disorders in the medical setting: some suggestions for DSM-V.

BACKGROUND: Adjustment disorders have been found to be the most frequent psychiatric diagnosis in the medically ill. Problems have been raised, however, as to their clinical value. The aim of the study was to characterize the psychosomatic features of adjustment disorders. METHODS: One hundred patients with medical illness and a diagnosis of adjustment disorder according to DSM-IV criteria were interviewed according to the Diagnostic Criteria for Psychosomatic Research (DCPR) system, consisting of 12 clusters. RESULTS: A considerable overlap was shown between adjustment disorders and DCPR cl

Social stigmatization in patients with cranial and cervical dystonia.

Patients with cranial and cervical dystonia (CCD) suffer from visible involuntary facial, head, and neck movements. Therefore, the social appearance of patients with CCD may be seriously affected and self-perceived stigma can be a major source of disability. The present study investigated enacted social stigmatization of patients with CCD. In a pilot study, a semantic differential scale for assessment of stigma was constructed and validated. The final scale contained eight items representing personality traits to be rated on a seven-point scale (-3 negative extreme to 3 positive extreme). Shor

The Health Anxiety Inventory: development and validation of scales for the measurement of health anxiety and hypochondriasis.

BACKGROUND: A self-rated measure of health anxiety should be sensitive across the full range of intensity (from mild concern to frank hypochondriasis) and should differentiate people suffering from health anxiety from those who have actual physical illness but who are not excessively concerned about their health. It should also encompass the full range of clinical symptoms characteristic of clinical hypochondriasis. The development and validation of such a scale is described. METHOD: Three studies were conducted. First, the questionnaire was validated by comparing the responses of patients su

Mothers' representations of relationships with their children: relations with mother characteristics and feeding sensitivity.

OBJECTIVE: To examine parenting representations and feeding interactions of mothers and their children with cerebral palsy (CP) and the extent to which mothers' representations predict their feeding behavior beyond other mother and child characteristics. METHODS: Fifty-eight mothers of children with mild to severe CP ages 16 to 52 months were interviewed with an adapted form of the Parent Development Interview (PDI). Correlation and regression analyses examined relations between representations (compliance with parental requests, achievement, secure base, enmeshment, worry about the child's f

The psychosocial experience of women treated for breast cancer by high-dose chemotherapy supported by autologous stem cell transplant: a qualitative analysis of support groups.

Autologous bone marrow transplantation (AuBMT) is probably among the most aggressive of physical treatments endured by cancer patients. High-risk breast cancer patients who choose this therapy face prolonged, agonizing and life-threatening interventions that are no less arduous than confronting the malignant disease itself. The study, which aimed to broadening our understanding of the psychosocial impact and the implications of AuBMT, presents a protocol analysis of group support intervention in 45 recipients (eight to ten women in five groups). The sessions were held at the Transplant Departm

The effect of labeling on perceived ability to recover from acute illnesses and injuries.

BACKGROUND: The process of giving a patient a diagnosis may cause harm. The adverse effects of labeling, best documented for the diagnosis of hypertension, include increased absenteeism from work and lower earnings, increased depressive symptoms, and reduced quality of life. We tried to determine whether the diagnosis of hypertension affects perceptions about the time required to recover from common acute medical problems. METHODS: In an academic family practice clinic, equal numbers of patients with and without hypertension were asked to estimate how long it would take them to recover from a

A neuropsychological study of the postpolio syndrome: support for depression without neuropsychological impairment.

OBJECTIVE: This study aimed to examine cognitive functioning in postpolio syndrome (PPS) after controlling for the effects of depression and illness behavior. BACKGROUND: Few studies have investigated the possible cognitive sequelae of PPS, despite widespread documented subjective complaints of "mental fatigue." METHOD: A total of 23 PPS sufferers, 20 polio survivors without PPS, and 22 matched controls were compared using the Beck Depression Inventory-II; the Illness Behaviour Questionnaire; a chronic fatigue syndrome symptom checklist; and several measures of memory, attention, and concent

Psychological variables as predictors of rubella antibody titers and fatigue--a prospective, double blind study.

When exposed to infectious pathogens, human beings manifest variability in the incidence and severity of infection. This variability may partly depend on psychological variables, which have long been thought to contribute to the predisposition, onset, and course of various physical illnesses, including infectious diseases. The objective of the study was to investigate the predictive value of several personality and other psychological variables on antibody titers and fatigue following a specific viral infection. Subjects were divided into a seronegative group (not immune prior to vaccination)

Juvenile myoclonic epilepsy: clinical characteristics, treatment and prognosis in a Norwegian population of patients.

Forty-three patients with juvenile myoclonic epilepsy (JME) is presented. The female to male ratio was 2.9:1. The patients answered a standardized questionnaire pertaining to social situation, medical history, onset of epilepsy, types and frequency of seizures, treatment, experienced control over seizures and consequences of having epilepsy. Myoclonic jerks, which are the hallmark of the condition, are often forgotten by the patients or not considered as epileptic seizures. This could be one reason why JME still seems underdiagnosed. JME may comprise absence, myoclonic and generalized tonic-cl
