Computed tomography of a diploic epidermoid cyst of the skull.

The computed tomographic (CT) and other roentgenological characteristics of a skull diploic epidermoid cyst in a 14-year-old boy with Ito syndrome, a neurocutaneous syndrome, are presented. When this boy was 3 years 10 months of age, a plain roentgenogram of the skull showed an area of decreased density in the left parietal bone. The area has grown for the subsequent 10 years plus and a characteristic sclerotic margin has become evident. CT examination showed a cystic lesion in the diploe, the outer third of high density and the inner third low density. CT allowed prediction of the operative a…

Combined-modality treatment of osteogenic sarcoma of the skull.

The authors present the results of combined-modality treatment in eight patients with osteosarcoma of the skull. Six patients had de novo tumors, and two others had secondary sarcomas resulting from malignant transformation in Paget's disease. Wide surgical excision and combination chemotherapy were used in seven patients, and surgery and radiation therapy were employed in one case. Following chemotherapy, six patients underwent additional surgery. This aggressive approach resulted in four long-term survivors among the patients with de novo tumors. These data suggest that surgery in combinatio…

[Primary osteosarcoma of the base of the skull in Garcin syndrome].

This contribution presents a case study of a 17-year-old female patient with unilateral failure of several brain nerves within a period of five months. Further findings included fleeting homolateral signs of the small brain and, during the terminal phase, discrete hemiparesis. Diagnosis was based on the tomograms of the skull base and computerized tomography, which revealed a tumour at the base of the skull. During the obduction this was found to be a primary osteosarcoma. According to the literature it is rare for this kind of tumour to lead to Garcin 's syndrome.

[Roentgen diagnosis of the medial part of the base of the skull].

The median section of the base of the skull offers many diagnostic problems in radiology. Findings of the plain radiographs and tomographs, of computed tomography and angiography are demonstrated in cases of space-occupying growths of the sella region, lesions of the fossa temporalis and of the region of the pyramid of the medulla oblongata, from a differential diagnostic viewpoint. Sequelae of traumas are discussed separately.

Ocular changes in invasive osteogenic sarcoma of the skull.

Serial narrow-band green-light fundus photographs documented the evolution of descending optic atrophy in a case of rapid bilateral blindness produced by a highly invasive osteogenic sarcoma of the base of the skull. Disappearance of the nerve fiber layer (NFL) was observed to take place between the sixth and 12th week after onset of blindness; thus, it is similar to the evolution of descending atrophy found to occur in clinical cases of blindness caused by traumatic involvement of the optic nerve. Parallel to the disappearance of the NFL there was an increase in optic nerve head excavation an…

Surgical problems of the base of the skull. An interdisciplinary approach.

Lesions of the clivus, upper cervical vertebrae, and the cervicomedullary junction often present difficulties to the neurologic surgeon. Currently available surgical approaches often provide less than optimal exposure, for both removal of the primary lesion and repair of the dura. A transoral approach that combines a median labiomandibular glossotomy, soft palate "split," and hard palate resection has been used in illustrative cases. This technique not only provides maximum transverse and sagittal exposure, but considerably reduces the operating distance between the neurosurgeon and the cervic

[Results of computerized tomography of the skull in hemodialysis patients].

Psychiatric and neurological symptoms as well as the clinical diagnosis of 22 patients on chronic hemodialysis were set in contrast with the findings of the cranial computerized tomography. The cranial computerized tomography turns out to be a valuable diagnostic aid to recognise or exclude morphologic lesions in the central nervous system, particularly because the complex overlap of the various pathogenetic mechanisms in hemodialysed patients points out frequently the little value of the clinical symptoms.

Skull x-ray in the evaluation of endocrine diseases.

The skull x-ray remains invaluable as a first step in the diagnosis of many endocrine disorders. Pituitary tumors commonly cause enlargement or distortion of the sella turcica, which can be seen on skull x-ray. Pinealomas and craniopharyngiomas may also be detected. Hyperparathyroidism can cause granular decalcification in the skull. Hypoparathyroidism produces calcification of the basal ganglia and cerebellum. Lesions due due to metastatic malignancies or eosinophilic granulomas may be noted in cases of diabetes insipidus. Sellar enlargement may be due to the "empty sella" syndrome. Twenty-fi…

[Dislocations of the face and skull. Report of 20 cases].

The authors present a serie of 20 cases of craniofacial injuries with or without CSF leakage. They propose a simple classification in 2 groups following the impact: braniofacial injury or orbitofacial injury. For their, craniofacial or orbitofacial injuries are to be operated in one stage by an neurosurgical and maxillofacial team. The best time for operation is approximately around the 10th day following trauma.

Seizures and demineralization of the skull. A diagnostic presentation of rickets.

We present four infants with seizures, and one without seizures, in whom the diagnosis of rickets was first suggested because of a demineralized skull. A review of the skull films in all of our patients with rickets showed that 20 of 25 (80%) showed demineralization. Demineralization of the skull is a common and important feature of rickets. Furthermore, as demonstrated by our cases, it may be the first clue to the presence of rickets in certain infants.

Hydatid cyst of the skull: report of four cases.

Four new cases of cranial echinococcosis are presented and the literature is reviewed. Young males are most commonly affected. Early cases present as an expansile osteolytic lesion in the diploe. The more advanced forms manifest as multilocular or unilocular destructive lesions with adjacent soft-tissue masses. Two patients with unilocular hydatid cysts recovered completely after surgery. One youth with the multilocular form died four months after operation. The other patient is well two years after surgery. The location, anatomical forms of the lesion, and the importance of early diagnosis ar

A morphologic description of a dry skull with mandibulofacial dysostosis.

The morphology of an East Indian, dry skull exhibiting the characteristics of mandibulofacial dysostosis (MFD) is described. The skull was small, but the shape of the calvarium was essentially normal. Extensive anomalies were registered in the sphenoid bone, the temporal bone, the zygomatic bone, the maxilla, and the mandible. A lateral cephalometric radiograph of the skull was compared with a mean value diagram of adult Danish males in order to illustrate the morphologic aberrations of the MFD skull. The etiology and pathogenesis of the syndrome are disucssed.
