Outcomes After Pericranial Flap Reconstruction of the Anterior Skull Base After Transcribriform Resection of Sinonasal Tumors: A Retrospective Study.

BACKGROUND: The nasoseptal flap is commonly used in the reconstruction of the anterior skull base following sinonasal tumor resection, but has limitations in its use. METHODS: A retrospective case series at a quaternary referral center involved 13 consecutive anterior skull base resections reconstructed with a pericranial flap. Data collection included patient demographics, type of resection (endoscopic/open/combined), length of stay, follow-up and complications. The primary outcome was the cerebrospinal fluid (CSF) leakage rate and the secondary outcome was the overall complication rate. RE

Long-Term Results of Stereotactic Radiosurgery for Skull Base Meningiomas.

BACKGROUND: Gamma knife radiosurgery (GKRS) is well established in the management of inaccessible, recurrent, or residual benign skull base meningiomas. Most series report clinical outcome parameters and complications in the short intermediate period after radiosurgery. Reports of long-term tumor control and neurological status are still lacking. OBJECTIVE: To report the presentation, treatment, and long-term outcome of skull base meningiomas after GKRS. METHODS: From a prospectively collected institutional review board-approved database, we selected patients with a World Health Organization

Osteochondroma of the temporomandibular joint: report of 2 cases emphasizing the importance of personalizing the surgical treatment.

BACKGROUND: Osteochondromas (OCs) are rare in the craneofacial area (0.6%). We present 2 cases of OC of the mandibular condyle, emphasizing the surgical decision of each case. CASE 1: In a 48-year-old woman with facial asymmetry, left cross-bite, and mandible deviation to the left, a computerized tomographic (CT) scan confirmed the presence of a bony expansion of the right condyle. The clinical diagnosis was osteochondroma. The patient underwent condylectomy with costochondral reconstruction. CASE 2: In a 76-year-old woman with a 1-month history of right preauricular pain, CT showed a deformed

Use of the free vastus lateralis flap in skull base reconstruction.

Free flaps in skull base reconstruction are indicated for providing an effective separation of the intracranial cavity from the oronasal space, for eliminating a dead space, and for the treatment of established wound complications such as dural exposures and cerebrospinal fluid leaks. Seven patients with cranial base defects underwent reconstructions using a free vastus lateralis muscle flap. In two cases, a vastus lateralis flap was raised to incorporate the anterolateral thigh skin as a myocutaneous flap. In four cases, a free flap was indicated for reconstruction following tumor ablation, a

[Trans-facial and cranio-facial approach and resection of the skull base tumors].

The paranasal sinus cancers comprise about 10% of all head and neck neoplasms. There are 80% of SCC among them. Paranasal sinus cancer is difficult to treat with surgery due to the anatomic integration of sinuses into the cranial base. Suborbital approach allows to simultaneous resection of the tumor from sinuses and anterior cranial fossa. The excision of the nasofrontal bone lamella gives a chance to orbital decompression and optic nerve decompression as well and opening the anterior cranial fossa through posterior wall of the frontal sinus. The goal of the study was to present cases showing

Management of the optic canal invasion and visual outcome in spheno-orbital meningiomas.

OBJECTIVE: Spheno-orbital meningiomas often present with visual deficit due to invasion of the optic canal by the tumor. This study discusses the reasons of visual impairment, the choice of the surgical approach according to the type of optic canal involvement, and the factors correlated to the visual outcome in patients harboring a spheno-orbital meningioma. MATERIALS AND METHODS: A surgical series of 60 spheno-orbital meningiomas is reviewed. The preoperative visual symptoms, the involvement of the optic canal in both neuroradiological studies and surgical descriptions, the different surgic

Neurodegeneration in Langerhans Cell Histiocytosis: beyond a Sequela.

Langerhans cell histiocytosis (LCH) is a rare inflammatory myeloid neoplasm with heterogeneous organ and system involvement. Neurodegenerative LCH (ND-LDH) is an uncommon late manifestation with progressive clinical and radiological features, substantial morbidity and an unpredictable course. Historically it was considered a paraneoplastic sequela for which treatment was largely ineffective. Multisystemic disease, central diabetes and orbital and/or skull base bone lesions are risk factors associated with the development of ND-LCH. Early diagnosis, guided by clinical, radiological, electrophys

[Synchronic multicentric osteosarcoma. A case in a 10 year old boy and review of the literature].

The osteosarcoma is a malignant neoplasm originating in bone-producing cells whose usual presentation is predominantly monostotic in long bones. The polyostotic expression is rare, variable and controversial, predominantly high-grade cellular lesions in children; low-grade lesions that affect long bones in adults and others that show a dominant lesion in long bones from which metastases arise. We present the case of a ten-year-old patient with painful tumors in metaphasis and diaphysis of the femur, tibia, fibula, clavicle, humerus, as well as in pelvis, vertebrae and base of the skull of sync

A Rare Presentation of Ameloblastic Carcinoma of the Sinus Cavity and Skull Base.

Ameloblastic carcinoma (AC) is an exceedingly rare odontogenic cancer about which there is limited information in the literature. We present a case of AC originating in the sinus cavity and extending to the skull base in a patient in the first trimester of pregnancy. Diagnostic work up was complicated by this pregnancy, which delayed radiation exposure with imaging. Once scans were obtained, diagnosis was further complicated by the radiographic similarities between possible lung metastases and previously undiagnosed sarcoid nodules. After thorough work up to rule out metastatic disease, the pa

Juvenile nasopharyngeal angiofibroma.

Juvenile nasopharyngeal angiofibroma (JNA) is a rare benign tumor arising predominantly in the nasopharynx of adolescent males. It is an aggressive neoplasm and shows a propensity for destructive local spread often extending to the base of the skull and into the cranium. Clinically, however, it is obscure with painless, progressive unilateral nasal obstruction being the common presenting symptom with or without epistaxis and rhinorrhea. Diagnosis of JNA is made by complete history, clinical examination, radiography, nasal endoscopy and by using specialized imaging techniques such as arteriogra

Primary retroperitoneal paraganglioma simulating a pancreatic mass: a case report and review of the literature.

Paragangliomas are extra-adrenal tumors of the autonomic nervous system and may be found within the skull base, neck, chest, and abdomen. When presenting within the abdominal cavity, they may arise as a primary retroperitoneal neoplasm and can mimic vascular malformations or other conditions related to specific retroperitoneal organs such as the pancreas, kidneys, or adrenals. Retroperitoneal paragangliomas are mostly benign with good prognosis; however, they can present with abdominal pain, palpable mass, or hypertensive episodes. Patients should be initially evaluated with catecholamine leve

A case with sinonasal teratocarcinosarcoma in the nasal cavity and ethmoid sinus.

Sinonasal teratocarcinosarcoma (SNTCS) is a rare and aggressive malignant neoplasm histologically characterized by the combination of one or more epithelial elements and mesenchymal components. We report a 61-year-old man with SNTCS involving left nasal cavity and ethmoid sinus. He complained of left epistaxis for 1 week. Computed tomography and magnetic resonance imaging revealed a soft tissue filling the left middle meatus and ethmoid sinus, and effusion in the left sphenoid sinus but no invasion to the orbit or skull base. Tumor was completely removed with lateral rhinotomy, and post-operat

Correlation between stroboscopy and electromyography in laryngeal paralysis.

Twenty patients with vocal fold motion impairment were reviewed to correlate the findings of electromyography (EMG) and stroboscopy. The causes of motion impairment were idiopathic, previous surgery with recurrent laryngeal nerve injury, neck and skull base trauma, and neoplasm. The EMG studies were analyzed to assess the status of innervation of the immobile vocal fold. The presence or absence of the mucosal wave prior to therapeutic intervention was determined with stroboscopic examination. Eight of 10 patients with EMG evidence of reinnervation or partial denervation were found to have muco

Rare giant cell tumour of the left parieto-occipital skull in an 8-year-old female patient: a case report.

Giant cell tumor (GCT) of bone is a generally benign but locally aggressive neoplasm that most commonly affects the epiphysis of long bones in young adults, while involvement of the cranial bones is exceedingly rare, particularly in the pediatric population. Calvarial GCTs pose significant diagnostic challenges because their clinical and radiologic features overlap with other lytic skull lesions. We report a rare case of a pediatric giant cell tumor arising from the calvarium that presented with progressive scalp swelling and localized pain. Radiologic evaluation demonstrated an expansile oste

Neuroendocrine and undifferentiated sinonasal and skull base tumors: An up-to-date narrative review.

Tumors located in the nasal cavity, paranasal sinuses and the skull base comprise a wide range of histologic subtypes. Among them, neuroendocrine and undifferentiated tumors are rare but noteworthy, because of their distinctive features, aggressive nature, and diagnostic complexities. A literature search was conducted in the PubMed/MEDLINE and the Scopus databases from 2019 until inception. The keywords "neuroendocrine", "undifferentiated", "nose", "sinonasal", "paranasal", "skull base" were used. Thirty-eight articles referring to neuroendocrine and undifferentiated tumors of the nose, parana

Head and neck paragangliomas: clinical and molecular genetic classification.

Head and neck paragangliomas are tumors arising from specialized neural crest cells. Prominent locations are the carotid body along with the vagal, jugular, and tympanic glomus. Head and neck paragangliomas are slowly growing tumors, with some carotid body tumors being reported to exist for many years as a painless lateral mass on the neck. Symptoms depend on the specific locations. In contrast to paraganglial tumors of the adrenals, abdomen and thorax, head and neck paragangliomas seldom release catecholamines and are hence rarely vasoactive. Petrous bone, jugular, and tympanic head and neck
