STUDY OBJECTIVES: We objectively measured body composition, energy expenditure, caloric intake, and sleep in a large, diverse sample of healthy men and women and determined how energy balance and diet associated with sleep physiology.
METHODS: Healthy adults (n = 50; 21-50 years) participated in an in-laboratory study involving two baseline sleep nights (BL1-2, 10 hours time-in-bed/night, 2200-0800 hours). Polysomnography was recorded on BL2. Demographic information, body composition, and energy expenditure measurements were collected at study admittance and on BL1. Daily food/drink intake wa

OBJECTIVE: To investigate the differential nature of disturbed sleep in patients with fibromyalgia (FM) reporting sleep difficulties versus patients with primary insomnia (PI) and patients who do not report disturbed sleep (pain-free controls).
MATERIALS AND METHODS: Patients (FM: n=132; PI: n=109; normals: n=52) were recruited for different studies. FM and PI patients were preselected to meet the sleep disturbance criteria. Patients with sleep or circadian disorders were excluded from all groups. Polysomnography was conducted at screening, during 2 consecutive nights. For this post hoc analy

Inter-patient comparisons of cardiovascular metrics indicative of patient health have been shown to be successful in differentiating patients on a group rather than an individual level. This is in part due to the range of mental (as well as physical) activity-based variations for each patient and the difficulty assessing physical and mental activity during conscious states. In order to provide an objective scale for measuring central nervous system activity during sleep, the heart rate (RR) interval time series is divided into coarse sleep stage segments in which the LF/HF-ratio (the relative

Slow brain rhythms, for example during slow-wave sleep or pathological conditions like seizures and spreading depolarization, can be accompanied by oscillations in extracellular potassium concentration. Such slow brain rhythms typically have a lower frequency than tonic action-potential firing. They are assumed to arise from network-level mechanisms, involving synaptic interactions and delays, or from intrinsically bursting neurons. Neuronal burst generation is commonly attributed to ion channels with slow kinetics. Here, we explore an alternative mechanism generically available to all neurons

Idiopathic hypersomnia is a rare chronic neurological condition that may be characterized by excessive daytime sleepiness, and/or prolonged nocturnal sleep, and also sleep inertia, yet its underlying mechanisms and diagnostic boundaries remain poorly defined. A key challenge is distinguishing excessive sleep duration from excessive daytime sleepiness, as patients may present one or both features, and similar symptoms can arise from diverse medical, psychiatric, and sleep disorders. Current diagnostic criteria rely heavily on the multiple sleep latency test, which frequently underestimates hype

BACKGROUND: Children with Down syndrome (DS) are at increased risk of sleep-disordered breathing (SDB), which is associated with intermittent hypoxia and sleep disruption affecting daytime functioning. We aimed to compare the impact of SDB on sleep quality in children with DS compared to typically developing (TD) children with and without SDB.
METHODS: Children with DS and SDB (n = 44) were age- and sex-matched with TD children without SDB (TD-) and also for SDB severity with TD children with SDB (TD+). Children underwent overnight polysomnography with sleep macro- and micro-architecture asse

Sleep-wake behavior is regulated by a circadian rhythm, homeostatically and by additional mechanisms that determine the timing of slow-wave sleep and rapid eye movement sleep (REMS) episodes. The posterior hypothalamus coordinates the neural and humoral signals with the rest-activity cycle. It contains wake-active neurons, and is a site where stimulation of inhibitory GABAA receptors promotes sleep, whereas their antagonism enhances wakefulness. We explored whether GABAergic mechanisms present in the posterior hypothalamus contribute to the homeostatic and other aspects of sleep-wake regulatio

INTRODUCTION: Sleep duration has been associated with overweight individuals in many epidemiological studies; however, few studies have assessed sleep using objective methods. Our study was designed to evaluate the association between body mass index (BMI) and sleep duration measured by actigraphy (Acti), polysomnography (PSG) and the Pittsburgh sleep quality index questionnaire (PSQIO). Furthermore, we evaluated other biochemical and polysomnographic parameters.
METHODS: A representative sample of 1042 individuals from Sao Paulo, Brazil, including both genders (20-80 yrs), participated in ou

Following a baseline night recording, 8 narcoleptic subjects and 8 sex- and age-matched controls were maintained on a 90 min sleep/wake schedule for 48-72 h. Each cycle consisted of 60 min of enforced wakefulness out of bed, followed by a 30 min "nap" period in which subjects were asked to try and fall asleep. Upon completion of the 90 min sleep/wake protocol, subjects were permitted to sleep ad libitum for 24 h. All sleep periods were monitored polygraphically; in addition, tympanic temperature and subjective sleepiness were recorded during the 90 min sleep/wake schedule. Narcoleptics and con

Non-rapid eye movement (NREM) parasomnias are sleep disorders characterized by abnormal, involuntary behaviours during partial arousals from slow-wave sleep. NREM parasomnias are common in childhood and usually remit, but a subset persist into or first emerge in adulthood, where they may cause significant morbidity. Evidence to guide treatment is limited, and no formal treatment guidelines exist. This review summarises evidence for behavioural, psychological, and pharmacological interventions. Ovid (Embase), the Cochrane Library, and PubMed were searched for English-language reports on the tre

Parasomnia overlap disorder (POD) refers to a sleep disorder characterized by the association of REM sleep behavior disorder (RBD) with NREM sleep parasomnia in the same patient. Sexual behaviors during sleep (SBS) can include most wakeful sexual activities and are classified in the ICSD-2 as a variant of confusional arousals and sleepwalking, both NREM parasomnias. A case of SBS associated with sleepwalking and possible RBD has been previously described, but it was not confirmed by polysomnography (PSG). We report two patients with SBS associated with POD documented by PSG. In one patient (60

Topographic EEG investigation with instant voltage mapping showed maximal negativity of 'rolandic' spikes over central or midtemporal electrodes with spread to parietal or upper frontal areas, and a dipole distribution (centrotemporal negativity, frontal positivity). There was no correlation of spike amplitude or duration with spread to adjacent areas. No other focal abnormalities, such as focal slowing, occurred. Spike activity was pronounced during light sleep and was often associated with generalized spike-wave activity, and thus was a sign of functional disturbance rather than the sequelae

Anesthesia, slow-wave sleep, syncope, concussion and reversible coma are behavioral states characterized by loss of consciousness, slow-wave cortical electroencephalogram, and motor and sensory suppression. We identified a focal area in the rat brainstem, the mesopontine tegmental anesthesia area (MPTA), at which microinjection of pentobarbital and other GABA(A) receptor (GABA(A)-R) agonists reversibly induced an anesthesia-like state. This effect was attenuated by local pre-treatment with the GABA(A)-R antagonist bicuculline. Using neuroanatomical tracing we identified four pathways ascending

Clinical and electroencephalographic data and response to drug therapy of 3 boys and 3 girls aged 14-19 years, mean 16, with juvenile myoclonic epilepsy (JME) are described. All presented with violent myoclonic jerks upon awakening, prompted by sleep deprivation and precipitated by deliberate awakening. Following onset of myoclonic jerks, generalized tonic-clonic seizures (GCTS) and absences occurred after 6-18 months (mean 11). The EEG revealed normal background activity in all, along with generalized epileptiform activity in the form of 3-4 Hz polyspike/slow wave or spike/slow wave, commonly

Regression in children with epilepsy may involve loss of cognitive abilities, failure to progress or a slowing of developmental trajectory. A few seizures do not lead to regression. Large numbers of seizures may be associated with regression but the cause is an important cofounder. Individual spike discharges on EEG are associated with transient cognitive impairment and continuous spike discharges with regression. Regression may be global in continuous spike wave in slow sleep (CSWS) or specific (auditory agnosia) in Landau Kleffner syndrome. Regression is mild and transient in Rolandic Epilep

Healthy aging is characterized by a diminished quality of sleep with decreased sleep duration and increased time awake after sleep onset. Older adults awaken more frequently and tend to awaken less from rapid eye movement (REM) sleep and more from non-REM (nREM) sleep than young adults. Sleep architecture also begins changing in middle age leading to a dramatic decrease in the deepest stage of nREM-slow wave sleep (SWS)-as aging progresses. Other less marked nREM changes include reduced numbers of sleep spindles and K-complexes. In contrast, the amount of REM diminishes only slightly. Both cir

Fibromyalgia syndrome (FMS) is a common disorder, characterized by diffuse pain and tenderness, stiffness, fatigue, affective disorders and significant sleep pathology. A new set of diagnostic criteria have been developed which should make it easier for a busy clinician to diagnose the condition. US Food and Drug Administration (FDA) approved medications for the treatment of FMS have, for the most part, been geared to modulate the pain pathways to give the patient some degree of relief. A different kind of pharmacological agent, sodium oxybate (SXB), is described that is currently approved for

A 40-year-old woman with no prior parasomnia developed an acute inflammatory rhombencephalitis with multiple cranial nerve palsies and cerebellar ataxia, followed by myelitis 6 months later, and by an intracranial thrombophlebitis 1 month after. Between and after these episodes, she had a persistent, mild right internuclear ophtalmoplegia, a mild cerebellar ataxia, and a severe REM sleep behavior disorder (RBD) lasting for 2 years. She talked, sang and moved nightly while asleep, and injured her son (cosleeping with her) while asleep. In addition, she walked asleep nightly. During video-polyso
