Simultaneous cochlear implantation during retrosigmoid resection of a large vestibular schwannoma under continuous electrically evoked auditory brainstem response monitoring.

Hearing preservation during surgery for large vestibular schwannomas (VS) remains challenging, and reports of simultaneous cochlear implantation (CI) via the retrosigmoid approach are exceedingly rare. We report, through close collaboration between neurosurgery and otolaryngology, what is, to our knowledge, the first case of simultaneous CI during retrosigmoid resection of a large sporadic VS under continuous intraoperative electrically evoked auditory brainstem response (EABR) monitoring. A 70-year-old woman with a 30-mm Koos grade 4 sporadic VS and contralateral profound hearing loss present

Hearing aids are still beneficial to patients, even if they have a low speech discrimination.

PURPOSE: A speech discrimination score (SDS) is a predictor for the successful use of hearing aids (HAs). This study is to evaluate the efficacy of HAs in patients with a low or poor SDS. METHODS: We enrolled 186 ears using HAs, with an unaided SDS ≤ 64%. They were categorized into four groups by their unaided SDS: 0-16% for Group 1, 20-32% for Group 2, 36-48% for Group 3, and 52-64% for Group 4. Aided SDS was measured 1, 3, 6, and 12 months after the use. The Hearing In Noise Test (HINT), the Hearing Handicap Inventory for the Elderly (HHIE), and the International Outcome Inventory for Heari

Auditory temporal processing deficits in children with reading disabilities.

The role of central auditory processing in reading skill development and reading disorders is unclear. The purpose of this study was to examine whether individuals with specific reading disabilities (SRD) have deficits in processing rapidly presented, serially ordered non-speech auditory signals. To this end, we compared 12 children with SRD and 12 children without SRD on their ability to detect differences and similarities in pure tones of 1000 and 2000 Hz when these signals were presented in pairs (same or different tones, randomly ordered) with short (50 ms) versus long (500 ms) inter-stimu

Reduced Learning of Sound Categories in Dyslexia Is Associated with Reduced Regularity-Induced Auditory Cortex Adaptation.

A main characteristic of dyslexia is poor use of sound categories. We now studied within-session learning of new sound categories in dyslexia, behaviorally and neurally, using fMRI. Human participants (males and females) with and without dyslexia were asked to discriminate which of two serially-presented tones had a higher pitch. The task was administered in two protocols, with and without a repeated reference frequency. The reference condition introduces regularity, and enhances frequency sensitivity in typically developing (TD) individuals. Enhanced sensitivity facilitates the formation of "

Long-Term Behavioral and Neurophysiological Outcomes Following Cochlear Reimplantation in Children: An EEG Study With Predictive Factor Analysis.

OBJECTIVES: Cochlear reimplantation is becoming more common; however, some patients do not achieve the expected outcomes. There is limited evidence regarding cochlear reimplantation-related developmental delays. This study aimed to (1) systematically compare the performance of children with cochlear reimplantation to those with primary cochlear implants using behavioral and EEG measures and (2) identify the influencing factors. DESIGN: This cross-sectional study was conducted at 2 tertiary medical centers specializing in audiology and cochlear implantation from April 2023 to February 2024. Th

Validation of the dyspnea index in adolescents with exercise-induced paradoxical vocal fold motion.

IMPORTANCE: Paradoxical vocal fold motion (PVFM) affects almost 1 million adolescents in the United States. However, to date, no disease-specific objective measure exists to assess symptom severity and response to treatment in adolescents with exercise-induced PVFM. OBJECTIVES: To validate the Dyspnea Index (DI) quality-of-life instrument (previously validated for adults with breathing disorders) in children aged 12 to 18 years with exercise-induced PVFM and to determine the minimum significant DI change corresponding to patient-reported or caregiver-reported improvement or worsening of sympt

Association between monocyte-to-high-density lipoprotein cholesterol ratio and hearing loss: A cross-sectional study based on NHANES.

BACKGROUND: This study aimed to unravel the association of the monocyte-to-high-density lipoprotein cholesterol (HDL-C) ratio (MHR) with different hearing loss (HL) types among American adults and assess the potential of MHR as a predictive HL biomarker. METHODS: 2001-2006, 2009-2012, and 2015-2020 NHANES data were analyzed. The possible relations of MHR to speech-frequency hearing loss (SFHL) and high-frequency hearing loss (HFHL) were unveiled leveraging weighted univariate and multivariate logistic regression (LR) models. Possible nonlinear relations of MHR to SFHL and HFHL were explored u

Effects of sensorineural hearing loss on formant-frequency discrimination: Measurements and models.

This study concerns the effect of hearing loss on discrimination of formant frequencies in vowels. In the response of the healthy ear to a harmonic sound, auditory-nerve (AN) rate functions fluctuate at the fundamental frequency, F0. Responses of inner-hair-cells (IHCs) tuned near spectral peaks are captured (or dominated) by a single harmonic, resulting in lower fluctuation depths than responses of IHCs tuned between spectral peaks. Therefore, the depth of neural fluctuations (NFs) varies along the tonotopic axis and encodes spectral peaks, including formant frequencies of vowels. This NF cod

Medical treatment of otosclerosis: rationale for use of bisphosphonates.

Otosclerosis is a bony dyscrasia characterized by histopathological findings of osteoclast production. Osteoclastogenesis explains the pathogenesis of otosclerosis. Basic science research in the experimental animal otic capsule has given insight into the process of evolution of otosclerosis. The normal otic capsule is preserved with very little bone turnover as a result of the production of osteoprotegerin (OPG) by the membranous inner ear that prevents the activation of osteoclasts. Animals genetically unable to produce OPG demonstrated the production of hearing loss and histopathology of the

Hearing results with clothespin ossiculoplasty: preliminary report on the Kraus Modified Schuring Ossicle-Cup Prosthesis (Clothespin Prosthesis).

The clothespin partial ossicular replacement prosthesis (PORP) is designed to increase joint stability during incus replacement ossiculoplasty. Fundamental modifications have been made in the Schuring ossicle-cup, which include a forked well, increased well wall thickness and length, and a flexible tip disk added to the shaft. The forked well functions like a straight clothespin rather than an inverted cup. The forked well enables the clothespin prothesis to slide down over the stapes superstructure, between the facial nerve canal and the promontory, to form a stable, mortise-and-tenon prosthe

Comparative Efficacy of Laser Acupuncture and Direct Photobiomodulation for Inferior Alveolar Nerve Hypoesthesia After Mandibular Third Molar Extraction: A Randomized, Double-Blind, Sham-Controlled Clinical Trial.

BACKGROUND: Inferior alveolar nerve (IAN) sensory disturbance after mandibular third molar extraction can persist and affect speech, mastication, and quality of life. Photobiomodulation (PBM) has been used to promote peripheral nerve recovery, but the most suitable irradiation strategy remains unclear. Laser acupuncture (LA) delivers PBM to predefined acupoints and may provide both local and central neuromodulatory effects. OBJECTIVE: To compare standardized LA with direct nerve-trajectory PBM for post-extraction IAN hypoesthesia in a randomized, double-blind, sham-controlled trial. METHODS:

Estimated prevalence of noise-induced hearing threshold shifts among children 6 to 19 years of age: the Third National Health and Nutrition Examination Survey, 1988-1994, United States.

OBJECTIVE: This analysis estimates the first nationally representative prevalence of noise-induced hearing threshold shifts (NITS) among US children. Historically, NITS has not been considered a common cause of childhood hearing problems. Among children, NITS can be a progressive problem with continued exposure to excessive noise, which can lead to high-frequency sound discrimination difficulties (eg, speech consonants and whistles). METHODS: The Third National Health and Nutrition Examination Survey (NHANES III) was conducted from 1988 to 1994. NHANES III is a national population-based cross
