Central skull base lymphoma in children: MR and CT features.

BACKGROUND: Central skull base tumours are rare in children. OBJECTIVE: To characterize the imaging features of central skull base lymphoma … ncement, and changes in adjacent structures were evaluated. RESULTS: All four tumours involved the sphenoid bone. In three patients, the tumour extended to the ethmoid sinus and both cavernous sinuses. All lesions were isointense solid masses on T2-weighted images and showed avid enhancement, except for one lesion with a focal necrosis. All tumours were associ …

Intraosseous hemangioma of the lateral orbital wall.

PURPOSE: To report the clinical and radiologic features of two patients with intraosseous orbital hemangiomas of the lateral wall. METHODS: Case reports and literature review. RESULTS: The first patient presented with an indolent mass over the lateral orbital rim. Intraosseous hemangioma was not considered in the differential diagnosis. A biopsy specimen was taken from the tumor, which caused significant bleeding. After biopsy, an orbital CT scan revealed a lesion involving the left lateral orbital rim and soft tissue of the temporal fossa. The rim was grossly enlarged, with several cavities

Craniofacial growth disturbance after skull base and associated suture synostoses in the newborn chacma baboon: a preliminary report.

The purpose of this study was to create craniofacial growth disturbances via synostoses of sutures of the cranial base, as well as of other various selected sutures of the anterior cranial and posterior facial structures. Twelve sutures, single or in combination, were obliterated by means of autogenous bone transplantation in twenty-five baby baboons (Papio ursinus). The data obtained from the sacrificed animals were qualitatively analyzed. The results indicated that synostosis of any of the selected sutures does lead to craniofacial growth disturbances. Furthermore, no specific synostosed sut

Widespread metastases from acinic cell carcinoma of parotid gland.

Acinic cell carcinoma metastasising to the spine is rare and has been described only once before in the literature. Its metastasis to other organs such as the regional lymph nodes, lungs and orbit have all been described, but the simultaneous occurrence of lymph nodal, pulmonary and spinal spread in a single patient has not been recorded. We believe this 40-year-old man to be the first reported case of incompletely resected acinic cell carcinoma of the parotid gland metastasising simultaneously to regional lymph nodes, upper lobes of both lungs, sphenoid bone and dorsal spine with neurological…

[Orbital and intracranial complications of acute sinusitis in childhood--status of endoscopic paranasal sinus surgery based on case examples].

BACKGROUND: Orbital or endocranial complications of sinusitis are rare clinical syndromes in infancy. METHODS: We retrospectively examined the complications that occurred and the role of functional endoscopic sinus surgery (FES). RESULTS: Most of the complications were caused by acute inflammation of the ethmoid or sphenoid cells with direct extension through dehiscent areas (flat bone) or neurovascular foramina. MRT had the highest sensitivity in detecting acute inflammatory foci. Early elimination of the cause of inflammation by FESS prevents spread of the complication. Advantages of FESS

Modified Lynch procedure for chronic frontal sinus diseases: rationale, technique, and long-term results.

The modified Lynch operation (Neel-Lake) differs in several ways from the operation described by Lynch. The operation begins with an intranasal anterior ethmoidectomy. The agger nasi cells are removed by curetting forward between the frontal process of the maxilla and the septum. The middle turbinate, normal-appearing mucosa of the frontal-ethmoid complex, and frontal process of the superior maxilla are preserved. Bone removal is limited in most cases to the anterior floor of the frontal sinus, a portion of the lacrimal bone, and the bone over the anterior ethmoid cells. Another important diff

Fungal central skull-base osteomyelitis: atypical presentation and management issues.

Mucormycosis is a very commonly encountered disease in the sinonasal region in patients with diabetes and immunocompromised status specially … geal abscess. The patient was taken up for emergency surgery; a necrotic lesion with destruction of sphenoid bone, pterygoids and clivus was seen originating in the nasopharynx. The lesion on fungal staining came out to be broad aseptate hyphae suggestive of mucormycosis.

Treatment of a sphenoid sinus mucocele using an endoscope combined with a navigating system: a case report.

A sphenoid sinus mucocele extending into the middle cranial fossa and the pterygomaxillary fissure was decompressed using an endoscope in combination with a navigation system. A 59-year-old woman was admitted with a 3 month history of retro-ocular pain and right sided exophthalmos. To evaluate the cystic lesion, a fibreoptic endoscope was inserted into the cyst through an erosive defect in the temporal bone. The histopathological appearance of cyst wall tissue confirmed the diagnosis of mucocele. After 4 months, the mucocele had refilled and the exophthalmos had recurred. At reoperation, the c

Intracranial extension and spontaneous hemorrhage of a sphenoid plasmacytoma.

A 71-year-old woman with IgG multiple myeloma presented initially with a compression fracture of the thoracic spine. She responded to radiation therapy and chemotherapy with melphalan and prednisone. One year later, she died from a plasmacytoma eroding intracranially from the greater wing of the sphenoid bone with an associated subdural hematoma. Uncal herniation had been preceded for several weeks by facial numbness. Cranial and intracranial plasmacytomas may occur as isolated lesions or as part of multiple myeloma and can present clinically in a variety of ways, most frequently with cranial …

Congenital Ocular Dystopia from Orbitofrontal Bone Dysplasia.

BACKGROUND: Several patients with ocular dystopia and craniofacial differences have been found to have orbital dysplasia and a frontal bone defect. This deformity is characteristically different from differential diagnoses of encephalocele, sphenoid dysplasia, craniofacial dysostoses, or atypical clefting. METHODS: The authors retrospectively reviewed the craniofacial registries of two pediatric centers for patients presenting with ocular dystopia or orbitofrontal anomalies between 2000 and 2014. The features and treatment of these patients were analyzed. RESULTS: Four patients with congenit

Extradural and Intradural Anterior Clinoidectomy: Technical Nuances and Video Illustration.

An anterior clinoidectomy is an important skull-base technique to have in the armamentarium when managing pathology of the paraclinoid region. Drilling the anterior clinoid (AC) provides access to the clinoidal internal carotid artery and early decompression of the optic nerve. This technique is essential in the management of central skull base tumors and aneurysms, especially from the opticocarotid region. The intricate neurovascular anatomy associated with the AC can be difficult to master. There are 2 main techniques for drilling the AC, intradural and extradural, although hybrid techniques

In this case report a 54-year-old woman had an anterior clinoid process meningioma. She was initially diagnosed as having a cerebrovascular disease, however, her stroke-like symptoms were most likely caused by internal carotid artery compression or vasospasm due to meningiomal involvement, but initially overlooked. Meningiomas are rarely reported as a cause of a stroke. A detailed evaluation can provide a high degree of confidence in differentiating stroke and non-stroke medical conditions, known as stroke mimics or chameleons, to be considered when a diagnosis of stroke has not been confirmed

Anatomical Study and Clinical Application of Optic Canal Decompression Via Transethmoid-sphenoid Approach Under Endoscope.

This study aimed to evaluate the safety and accuracy of the endoscopic transethmoid-sphenoid approach for optic canal decompression. Twelve sides of 6 adult cadaveric heads fixed in formalin were selected to simulate optic canal decompression using the endoscopic transethmoid-sphenoid approach. Furthermore, this approach was used for optic canal decompression in 10 patients (11 eyes) with optic nerve canal injury. Related anatomical structures were observed using a 0-degree endoscope, and the anatomical characteristics as well as the surgical data were collected. The maximum effective widths o

The Endoscopic Trans-Sinusoidal Trans-Pterygopalatine Route to the Foramen Rotundum Approach in Trigeminal Neuralgia Treatment.

AIM: To describe a new, minimally invasive, and safe access to foramen rotundum via the endoscopic endonasal trans-sinusoidal route for trigeminal nerve blockage. MATERIAL AND METHODS: We studied 5 fresh cadavers bilaterally, and 10 pterygopalatine fossae and maxillary nerves using the endoscopic endonasal trans-sinusoidal trans-pterygopalatine approach. RESULTS: The proposed approach enabled the maxillary nerve to be visualized after revealing the foramen rotundum with the help of some craniometric measurements. Distance between sphenoid sinus lateral wall and maxillary sinus posterior wall

Ossification of Pterygospinous and Pterygoalar Ligaments and Their Clinical Significance: An Anatomic Study.

Pterygospinous and pterygoalar ligaments, present in close proximity of foramen ovale at the base of skull, ossify creating pterygospinous and pterygoalar foramina, which causes problems in accessing retro and parapharyngeal spaces, trigeminal ganglion for treating trigeminal neuralgia by thermocoagulation and blocking it. Besides, these anatomical entities may compress nearby neurovascular structures causing discomfort to patients. Considering immense clinical significance associated with ossification of pterygospinous and pterygoalar ligaments, the study has been carried out. Aim of the stud

The Shape of the Foramen Ovale: A Visualization Aid for Cannulation Procedures.

Several neurosurgical procedures, including rhizotomy for the management of trigeminal neuralgia, cannulate the foramen ovale (FO) to access intracranial structures. Cannulation procedures, including those utilizing neuronavigational technology, are occasionally complicated by anatomical variation of the FO, sometimes resulting in miscannulation and subsequent adverse events. The FO, while commonly thought of as oval-shaped, has also been described as "almond," "banana," "D shape," "pear," and "triangular." Because of the importance of the FO in neurosurgical procedures and the misunderstandin

An anatomical study of the pathophysiology of carotid cavernous sinus fistula associated with Le Fort III osteotomy.

UNLABELLED: Carotid-cavernous sinus fistula (CCF) is a rare complication occurring after the Le Fort osteotomy. We aimed to elucidate the cause of CCF in the Le Fort osteotomy. METHODS: Eleven fresh cadavers were used. After craniotomy, a Le Fort III bipartition osteotomy was then performed. On the left side, both the lateral wall of the maxilla and the pterygomaxillary junction (PMJ) were divided, in addition to the conventional osteotomy line. On the right side, those parts were kept intact. After the osteotomy, a tensiometer was fixed to the skull base. The sensor was linked with the wall

Norman Dott, Gerard Guiot, and Jules Hardy: key players in the resurrection and preservation of transsphenoidal surgery.

Developed over a century ago, the transsphenoidal approach to access lesions of the pituitary gland and sella turcica has transformed the field of neurosurgery, largely due to the work of Oskar Hirsch and Harvey Cushing. Furthermore, its use and modification in the early 1900s was perhaps one of Cushing's greatest legacies to skull base surgery. However, Cushing, who had worked relentlessly to improve the transsphenoidal route to the pituitary region, abandoned the approach by 1929 in his pursuit to master transcranial approaches to the suprasellar region. Hirsch and a few other surgeons conti
