Ectopic sphenoidal ACTH-secreting adenoma revealed by 11C Methionine PET scan: case report.

BACKGROUND: Ectopic ACTH pituitary adenomas (EAPA), located outside the sella turcica and deriving from cellular remnants of Rathke's pouch are a very rare cause of Cushing's syndrome (CS). The diagnosis is often difficult and delayed, even after comprehensive work-up. To our knowledge, we report for the first time an ectopic corticotroph tumor of the posterior wall of the sphenoid sinus, leading to false positive results of bilateral inferior petrosal sinus sampling (BIPPS) and which was finally localized by a co-registered11 C Methionine PET/MR imaging. CASE PRESENTATION: A 48-year-old woma

Sphenoid sinus disease. Experience at King Edward VIII Hospital, Durban.

The increasing use of computed tomography (CT) to evaluate sinus disease in general, appears to have led to an increased pickup rate in isolated sphenoid sinus disease. It is probable that subtle disease of the sphenoid sinus was missed before the use of CT as a diagnostic tool. Six patients with sphenoid sinus disease and related pathological conditions seen during the past 2 years, are reviewed; these included 4 mucoceles, 1 case of aspergillosis and 1 of squamous cell carcinoma. Experience with sphenoid disease, with particular reference to the diagnosis, management and complications of thi

CBCT Volumetric Changes in Combined Nasal Cavity and Paranasal Sinuses Following RAMPA-ROA Therapy: A Retrospective Cohort Study with Reference to Longitudinal Growth Data.

Background: The interrelationship between craniofacial morphology and respiratory function is a central focus of orthodontic and dentofacial orthopedic research. This study aimed to evaluate the volumetric changes in the sinonasal complex (combined nasal cavity and paranasal sinuses) following Right Angle Maxillary Protraction Appliance (RAMPA) therapy using cone-beam computed tomography (CBCT) and to compare these outcomes with established longitudinal growth benchmarks. Methods: A retrospective cohort analysis was conducted on 60 pediatric patients (24 males, 36 females; mean age: 86.60 ± 24

Magnetic Resonance Imaging Detects Chronic Rhinosinusitis in Infants and Preschool Children with Cystic Fibrosis.

Rationale: Chronic rhinosinusitis (CRS) contributes to disease burden of patients with cystic fibrosis (CF). However, its onset and progression in infants and preschool children with CF remain poorly understood. Objectives: To determine the prevalence and extent of CRS in young children with CF using magnetic resonance imaging (MRI).Methods: MRI was performed in sedation in 67 infants and preschool children with CF (mean age 2.3 ± 2.1 yr; range 0-6 yr) and 30 non-CF control subjects (3.5 ± 2.0 yr; range 0-6 yr). Paranasal sinus dimensions and structural abnormalities, including mucosal swelling

Hypoosmolar Hyponatremia as Presenting Symptom of a Rare Case of Olfactory Neuroblastoma.

Hyponatremia is a common electrolyte disturbance. The syndrome of inappropriate antidiuresis (SIAD), characterized by nonosmotic release of arginine vasopressin, is a typical cause of hyponatremia. Ectopic secretion of arginine vasopressin by a tumor is often associated with small cell lung cancer but can also be seen in other neoplasms. We present a rare case of a previously healthy 34-year-old woman, evaluated for refractory hypoosmolar hyponatremia consistent with SIAD. After excluding other causes of SIAD, imaging revealed contrast-enhancing mucosal thickening in the left maxillary sinus w

Maxillary sinus mucoceles and other side effects of external-beam radiation in the pediatric patient: A cautionary tale.

A sinonasal mucocele can develop after the obstruction of a sinus ostium, which can occur secondary to trauma, infection, a neoplasm, or an iatrogenic cause. These mucoceles typically arise in the frontal and ethmoid sinuses. Sinonasal mucoceles are epithelium-lined cysts that can gradually expand to fill a paranasal sinus. They can present with ophthalmologic, rhinologic, and neurologic symptoms. There are reports in the literature of sinonasal mucocele development after radiation for nasopharyngeal carcinoma. However, these cases are rare; when they do occur, they tend to arise in the spheno

Cholesterol Granule of the Ethmoid Sinus: A Case Report.

Cholesterol granuloma (CG) is common in the mastoid air cells, less common in the skull base and orbit, and uncommon in the paranasal sinuses. The most commonly affected sinus is the maxillary sinus, and it is very rare in the ethmoid and sphenoid sinus. CG is thought to be due to impair the venous and lymphatic drainage from the sinus cavity. In the early period of the disease, the patient has no symptoms but when the expanding cysts compress the surrounding structures, they cause bony erosion that leads to clinical symptoms such as nasal blockage, eye pain or visual loss. If the patients' pr

The tale of two tumours: an undiagnosed case of papillary thyroid carcinoma.

Pituitary metastasis is an infrequent clinical problem occurring in 1%-5% of various autopsy series. Differentiated thyroid carcinoma as the primary malignancy was reported in only 2.1% of the cases. A 53-year-old Filipina presented with 7 months history of progressive loss of vision and headaches. She underwent thyroidectomy 2 years prior to admission for an enlarging neck mass. After then, she was lost to follow-up. Physical examination revealed visual field loss, galactorrhea and a 3×4 cm firm suprasternal mass. Imaging showed a 4.5×5×5 cm mass in the sphenoid and ethmoid sinuses with exten

Unsuccessful frontal balloon sinuplasty for recurrent sinus barotrauma.

The standard of care treatment for diffuse recurrent sinus barotrauma (RSB) is an endoscopic sphenoethmoidectomy with a complete frontal dissection. Successful healing leaves the RSB patient with no ethmoid sinuses and endoscopically patent frontal, sphenoid, and maxillary ostia. In persistent cases, patients with small frontal ostia will go on to require a frontal drillout. Patients presenting for surgical management of RSB generally have minimal sinus disease despite significant symptoms during flight and the prospect of extensive surgical management can be unappealing. With the advent of ba

Craniospinal dissemination of clival chondroid chordoma.

Chondroid chordoma commonly presents as clival osseous and extradural mass. A 15-year-old boy presented with progressive visual deficit, headaches and diplopia since three years. Computed tomography (CT scan) showed a skull base tumour, but was wrongly reported at the time as chronic sphenoidal sinusitis and nasal polyps. In the past three months, he developed dysphagia, urinary retention and constipation. Terminally, he had weakness of all limbs. Fundoscopy showed optic atrophy. Temporal and spatial variation in symptoms led to a clinical diagnosis of multiple sclerosis with optic neuritis. P

Angiomatous Nasal Polyps in a 43-Year-Old Female From North-Central Nigeria: A Case Report.

Angiomatous nasal polyps (ANPs) are uncommon variants of nasal polyps, which, like their more common counterparts, are a significant and common cause of morbidity for patients, regardless of demographic variations. As a group, nasal polyps are non-neoplastic and commonly arise within the maxillary antrum, sphenoid, and ethmoid sinuses, protruding via the ostia into the nasal cavity. Their precise cause is unknown, but several factors related to genetics, local infections, family history, and environment have been implicated in their etiopathogenesis. ANPs are commonly diagnosed in the younger

Pituitary Gland Macroadenoma with Associated Hearing Loss.

Pituitary macroadenomas are benign neoplasms that can cause a range of symptoms due to a mass effect on surrounding structures. This case report describes a 51-year-old male presenting with progressive bilateral hearing impairment and significant deterioration in speech discrimination over several years. Examination revealed normal tympanic membranes and no neurological deficits, while pure tone audiometry indicated mild to moderate sensorineural hearing loss. Magnetic resonance imaging identified a 6 × 5.5 × 7 cm pituitary macroadenoma extending into the suprasellar region and invading the sp

Mucormycosis causing oral and nasal osteonecrosis in a case of acute myeloid leukemia.

AIMS: Mucormycosis is a rare and aggressive fungal infection with a high mortality rate because of its rapidly progressive and destructive nature. The oral cavity is often affected under opportunistic conditions. We report a 34-year-old woman diagnosed with acute myeloid leukemia complained of slight swelling on the right side of her face with toothache and gingival swelling. An incisional biopsy was performed, and the specimen analysis revealed broad aseptate hyphae with a ribbon-like appearance, which is characteristic of opportunistic Mucorales infection. METHODS AND RESULTS: The oral lesi

Frontal sinus inverted papilloma managed with Draf III.

Inverted papilloma (IP) is a rare benign tumor that originates from nasal and paranasal sinus mucosa. Although it is considered a benign lesion, it is locally aggressive and has risk between 7 and 15% for malignant transformation. The management of frontal sinus (FS) IP is still challenging due to its rarity and local invasiveness. We report a rare case of IP originating from FS, and to review the literature to delineate the possible surgical strategy. A 52-year-old male presented with long-standing history of bilateral nasal obstruction and left eye proptosis. Sinus navigation computed tomogr

Persistent CSF Rhinorrhoea, Pneumocephalus, and Recurrent Meningitis Following Misdiagnosis of Olfactory Neuroblastoma.

A 41-year-old female patient was admitted with streptococcal meningitis on a background of 5-month history of CSF rhinorrhoea. Imaging revealed an extensive skull base lesion involving the sphenoid and ethmoid sinuses, the pituitary fossa with suprasellar extension and bony destruction. Histological examination of an endonasal transethmoidal biopsy suggested a diagnosis of olfactory neuroblastoma. A profuse CSF leak occurred and the patient developed coliform meningitis. A second endonasal endoscopic biopsy was undertaken which demonstrated the tumour to be a prolactinoma. Following endonasal
