The immature spine: growth and idiopathic scoliosis.

Untreated progressive scoliosis can have negative effects on the growing spine as asymmetrical forces will act on the growth plates of the vertebral column (>130 growth plates). Spinal growth can be considered as a mixture of hierarchy, synchronization, and harmony: the slightest error can lead to a complex malformation; it is also a very dynamic process although it does not progress linearly: periods of acceleration are followed by periods of deceleration. Remaining growth is a determining factor for the worsening of idiopathic scoliosis (IS): the younger is the child, the higher is the risk …

Thoracic Spine Fracture in the Panscan Era.

STUDY OBJECTIVE: In the current era of frequent chest computed tomography (CT) for adult blunt trauma evaluation, many minor injuries are diagnosed, potentially rendering traditional teachings obsolete. We seek to update teachings in regard to thoracic spine fracture by determining how often such fractures are observed on CT only (ie, not visualized on preceding trauma chest radiograph), the admission rate, mortality, and hospital length of stay of thoracic spine fracture patients, and how often thoracic spine fractures are clinically significant. METHODS: This was a preplanned analysis of pr…

Intradural osteogenic sarcoma in the lumbar spine.

To our knowledge, this is the third reported case of spinal intradural osteogenic sarcoma. The two prior reported cases had a history of iophendylate injection whereas this patient did not. Other cases involved the cranial meninges, not the spine. This is the first reported case of intradural osteosarcoma in the absence of iophendylate injection. We report our workup, diagnosis, and treatment. We also include a video demonstrating the intraoperative invasion of tumor and dural erosion.

A comparison of methods to quantify control of the spine.

Low back pain (LBP) affects many individuals worldwide. The established association between LBP and spine motor control has led to the development of many control assessment techniques. To understand spine control and LBP, it is essential to know the relationship between assessment techniques. Systems identification (SI) and local dynamic stability (LDS) are two methods of quantifying spine control. SI provides a detailed description of control but uses linearity assumptions, whereas LDS provides a "black box" non-linear assessment during dynamic movements. Therefore, the purpose of this proje…

[Etiology and significance of growth disturbances of the spine].

BACKGROUND: The majority of growth disturbances of the spine are acquired and their etiology is still unknown. Both scolioses and sagittal profile disorders are most commonly of idiopathic origin. ETIOLOGY: The etiology is multifactorial and besides genetic, hormonal and mechanical factors also metabolic components seem to be involved. The risk of progression of an existing deformity is particularly high during the pubertal growth spurt. Accordingly, regular clinical and radiological controls should be carried out in this vulnerable period. Recently, spinal deformities have been classified ac…

Paralytic ileus in the patient with tuberculosis of spine.

Background: We reported a patient with spinal tuberculosis and paralytic ileus. A 56-year-old Javanese male presented with lower limb paralysis and bowel obstruction 2 weeks prior to admission. He was found to have hypoalbuminemia and hypesthesia from the T7/T9 levels and below. Other than increased alanine aminotransferase, hematology and blood chemical tests were normal. MRI and plain abdominal radiographs confirmed the diagnosis of spinal tuberculosis at the T5/6 level and paralytic ileus. Tubercles in the lymphoid tissue of the intestinal submucosa were not seen. Conclusion: Paralytic ileus

Considerations in sagittal evaluation of the scoliotic spine.

PURPOSE: To predict the sagittal spinal parameters as measured in a 3D model of the spine using the 2D radiographic measurements. METHODS: Bi-planar low-dose stereoradiography images of 73 right thoracic AIS patients were processed to generate 3D models of the spine and pelvis. T1-T12 kyphosis, L1-S1 lordosis, and pelvic rotation were calculated using these 3D models. With the same X-rays, T1-T12 kyphosis, L1-S1 lordosis, thoracic and lumbar frontal curves, and pelvic rotation (calculated from the frontal and sagittal distances between the femoral heads) were manually measured on the X-rays b…

The Development of Kyphosis and Lordosis in the Growing Spine.

STUDY DESIGN: A longitudinal cohort study. OBJECTIVE: The aim of this study was to do the analysis of the development of kyphosis and lordosis in the growing spine. SUMMARY OF BACKGROUND DATA: Previous studies have measured kyphosis and lordosis in different ways with differing techniques. None of the previous literature has a truly longitudinal design and there is disagreement as to whether there exists a difference between the development of kyphosis and lordosis between males and females. METHODS: Repeated measures using Integrated Shape Imaging System Integrated Shape Imaging System 2 s…

[Operative treatment of the degenerative cervical spine].

BACKGROUND: Degenerative alterations of the cervical spine often entail disc herniations and stenoses of the spinal canal and/or neural foramen. Mediolateral or lateral compression of nerve roots causes cervical radiculopathy, which is an indication for surgery in cases of significant motor deficits or refractory pain. Median canal encroachment may result in compression of the spinal cord and cervical myelopathy. Its natural history is typically characterized by episodic deterioration, so that surgical decompression is indicated in cases of clear myelopathic signs. OBJECTIVE: The aim of the p…

Differential Diagnosis of a Pathological Spine Fracture.

A 71-year-old woman with a 10-month history of atraumatic low back pain was referred to physical therapy after an insidious exacerbation of symptoms. Red flags raised suspicions for spinal compression fracture, necessitating the physical therapist to contact her physician and recommend imaging. Radiographs and magnetic resonance imaging were ordered, the latter of which showed an acute fracture at T10, with lesions at T9 and in the liver suggesting metastasis. Following bone scintigraphy and an ultrasound-guided biopsy of a liver lesion, she was diagnosed with metastatic breast cancer. J Ortho

Spine surgery in patients with ankylosing spondylitis.

INTRODUCTION: Ankylosing spondylitis (AS) is an idiopathic seronegative spondyloartropathy that involves mainly the axial skeleton and the sacroiliac joints. AS promotes biomechanical changes in the spine that predispose to fractures, spinal deformity and spondylodiscitis. The aim of this article is to report the clinical and laboratorial characteristics of patients with AS who underwent spinal surgery at our Institution. METHODS: Retrospective review of medical charts of patients who had AS and underwent spinal interventions. RESULTS: Nine patients were found and eight were included in the …

Origins of eponymous instruments in spine surgery.

Every day, spine surgeons call for instruments named after surgical pioneers. Few know the designers or the histories behind their instruments. In this paper the authors provide a historical perspective on the Penfield dissector, Leksell rongeur, Hibbs retractor, Woodson elevator, Kerrison rongeur, McCulloch retractor, Caspar pin retractor system, and Cloward handheld retractor, and a biographical review of their inventors. Historical data were obtained by searching the HathiTrust Digital Library, PubMed, Google Scholar, Google Books, and Google, and personal communications with relatives, col…

New Implant-Based Technologies in the Spine.

Vertebral compression fractures (VCFs) may result in a kyphotic deformity which can cause potential systemic complications secondary to respiratory and gastrointestinal dysfunction. The use of implants in the spine for VCF treatment represents a paradigm shift away from cement injection on its own, aiming to combine the analgesic and stabilizing effect of injecting cement into the vertebral body with vertebral height restoration and kyphotic angle correction. Spine implants which can be used for VCF treatment include stents, jacks, PEEK cages and fracture reduction systems. Lumbar spinal steno…

Hydatid disease of the spine: A rare case.

Hydatid disease or hydatidosis is the most widespread zoonosis caused by Echinococcus granulosus. Liver and lungs are the most common sites. Bone involvement is rare and reported in 0.5%-4% with spinal involvement reported in 50% of these cases. We present a case of spinal hydatidosis in a 35-year-old male presenting with lower extremity weakness and numbness. Magnetic resonance imaging (MRI) of the spine showed multiple cystic lesions at the T9-T11 level with involvement of the paraspinal muscles. The lesion was seen intraspinal, intradural, intramedullary, and epidural. Radiological impressi…
