Unusual Cause and Presentation of Collapsing Focal Segmental Glomerulosclerosis.

Collapsing focal segmental glomerulosclerosis (c-FSGS), a structural variant of focal segmental glomeruloslecrosis (FSGS), is considered to be the most aggressive FSGS form. Most patients present with severe nephrotic syndrome and often have rapidly progressing renal failure and progression to end-stage kidney disease. We are reporting a 28-year-old previously healthy woman, who was started on griseofulvin for onchomycosis; she subsequently developed acute renal failure with significant proteinuria. Exposure to the drug caused dramatic decline in the renal function. Renal biopsy was compatible

Use of the SAND balloon catheter in single-incision laparoscopic cholecystectomy for acute cholecystitis.

INTRODUCTION: SILS for acute cholecystitis is technically challenging because of the difficulties in obtaining optical surgical field. MATERIALS AND SURGICAL TECHNIQUE: A 2-cm incision was made through the umbilicus, a single port and trocars were introduced, and the abdomen was then insufflated. A 5-mm SAND balloon punctured the abdominal wall and then the gallbladder wall. The distal and proximal balloons were inflated to prevent bile leakage, and the bile was aspirated. The collapsed gallbladder was then retracted cephalad, the critical structures were exposed, and the cystic artery and du

CREB selectively controls learning-induced structural remodeling of neurons.

The modulation of synaptic strength associated with learning is post-synaptically regulated by changes in density and shape of dendritic spines. The transcription factor CREB (cAMP response element binding protein) is required for memory formation and in vitro dendritic spine rearrangements, but its role in learning-induced remodeling of neurons remains elusive. Using transgenic mice conditionally expressing a dominant-negative CREB (CREBS133A: mCREB) mutant, we found that inhibiting CREB function does not alter spine density, spine morphology, and levels of polymerized actin in naive CA1 neur

Intervertebral disc degeneration: evidence for two distinct phenotypes.

We review the evidence that there are two types of disc degeneration. 'Endplate-driven' disc degeneration involves endplate defects and inwards collapse of the annulus, has a high heritability, mostly affects discs in the upper lumbar and thoracic spine, often starts to develop before age 30 years, usually leads to moderate back pain, and is associated with compressive injuries such as a fall on the buttocks. 'Annulus-driven' disc degeneration involves a radial fissure and/or a disc prolapse, has a low heritability, mostly affects discs in the lower lumbar spine, develops progressively after a

Trabecular microstructure of the human lunate in Kienbock's disease.

The trabecular microstructure of normal lunates and lunates with Kienböck's disease was investigated using micro-computed tomography (micro-CT). Five lunates with advanced Kienböck's disease were obtained during lunate excision and scaphocapitate fusion, and five control lunates were from embalmed cadavers. Microstructural morphometric parameters were measured using micro-CT images. Trabeculations of lunates with Kienböck's disease were 2.67 times denser and 1.84 times thicker than those of normal lunates. Furthermore, bone surface areas were 1.43 times greater and bone volume 2.67 times great

Proximal humeral fractures with a severe varus deformity treated by fixation with a locking plate.

We treated 47 patients with a mean age of 57 years (22 to 88) who had a proximal humeral fracture in which there was a severe varus deformity, using a standard operative protocol of anatomical reduction, fixation with a locking plate and supplementation by structural allografts in unstable fractures. The functional and radiological outcomes were reviewed. At two years after operation the median Constant score was 86 points and the median Disabilities of the Arm, Shoulder and Hand score 17 points. Seven of the patients underwent further surgery, two for failure of fixation, three for dysfunctio

Quadrigeminal cyst management by cystoperitoneal shunt in a 4-year-old Persian cat.

Quadrigeminal cysts represent intracranial cystic accumulations of cerebrospinal fluid within the arachnoid mater at the level of the quadrigeminal cistern. Quadrigeminal cysts are rare in cats, with only one previous report in the veterinary literature. A 4-year-old, male-neutered Persian cat was presented with a 1-year duration of initially episodic, but later progressive, obtundation and collapse. Magnetic resonance imaging of the brain revealed a quadrigeminal cyst with marked compression of the adjacent neural structures, cerebellar herniation and obstructive hydrocephalus. Cystoperitonea

Solid-state NMR evidence for inequivalent GvpA subunits in gas vesicles.

Gas vesicles are organelles that provide buoyancy to the aquatic microorganisms that harbor them. The gas vesicle shell consists almost exclusively of the hydrophobic 70-residue gas vesicle protein A, arranged in an ordered array. Solid-state NMR spectra of intact collapsed gas vesicles from the cyanobacterium Anabaena flos-aquae show duplication of certain gas vesicle protein A resonances, indicating that specific sites experience at least two different local environments. Interpretation of these results in terms of an asymmetric dimer repeat unit can reconcile otherwise conflicting features

Scanning electron microscopic studies on egg surface morphology and morphometrics of Culex tritaeniorhynchus and Culex quinquefasciatus (Diptera: Culicidae).

Culex tritaeniorhynchus Giles, 1901 and Culex quinquefasciatus Say, 1823 is an important vector of Japanese encephalitis and Bancroftian filariasis, respectively in India and South East Asian countries. In this paper, we are describing the surface morphology and morphometrics of the egg of C. tritaeniorhynchus in comparison with C. quinquefasciatus for the first time. The results indicated that eggs of both the species appears to be similar to great extent in surface morphology, however, morphometrics provide 56.81% demarking attributes out of 44 attributes at various significant levels (p < 0

Electrophysiological brainstem investigations in obstructive sleep apnoea syndrome.

Phasic inspiratory genioglossus activity prevents pharyngeal airway collapse in healthy subjects during sleep and is diminished or absent in obstructive sleep apnoea syndrome (OSAS), thus leading to pharyngeal obstruction. Case reports of OSAS after pontomedullary lesions indicate that impaired inspiratory genioglossal activity may result from brainstem lesions. We therefore investigated brainstem functions in 18 awake patients with OSAS using brainstem auditory evoked potentials, blink reflex, masseter reflex, masseter inhibitory reflex (in 11 of 18 patients), magnetic evoked potentials of th

Obstructive sleep apnea syndrome. A review.

Obstructive sleep apnea syndrome (OSAS) is a complex disorder characterized by a sleep-related collapse of the upper airway. The most likely candidate for the common pathway linking various abnormalities casually associated with OSAS (such as adenotonsillar hypertrophy, obesity, retro- or micrognathia, acromegaly, or more subtle structural anomalies) is an abnormally small upper airway lumen. Symptoms of OSAS that appear during sleep include snoring, abnormal motor activity, disturbed nocturnal sleep, a sensation of choking, heartburn, nocturia, nocturnal enuresis, and heavy sweating. Daytime

Vascularized bone graft from the iliac crest for the treatment of nonunion of the proximal part of the scaphoid with an avascular fragment.

BACKGROUND: It was hypothesized that nonunion of the proximal third of the scaphoid associated with avascular necrosis could be treated successfully with a free vascularized bone graft obtained from the iliac crest. METHODS: Fifteen patients who had a nonunion of the proximal part of the scaphoid that had been present for an average of two years and three months (range, nine months to seven years) were managed with use of a free vascularized bone graft obtained from the iliac crest. Avascularity of the scaphoid, as assessed on preoperative radiographs, was characterized by loss of trabecular

Pathways of Membrane Solubilization: A Structural Study of Model Lipid Vesicles Exposed to Classical Detergents.

Understanding the pathways of solubilization of lipid membranes is of high importance for their use in biotechnology and industrial applications. Although lipid vesicle solubilization by classical detergents has been widely investigated, there are few systematic structural and kinetic studies where different detergents are compared under varying conditions. This study used small-angle X-ray scattering to determine the structures of lipid/detergent aggregates at different ratios and temperatures and studied the solubilization in time using the stopped-flow technique. Membranes composed of eithe

Use of the anterior interbody fresh-frozen femoral head allograft in circumferential lumbar fusions.

Many studies in the literature have documented the outcome of circumferential lumbar fusions. However, no study has specifically evaluated the performance of the anterior fresh-frozen femoral head allograft as a structural interbody graft material. All office and hospital records, including charts and radiographs, were reviewed to obtain pertinent clinical and radiographic information. The cases included 23 single-level fusions, 22 two-level fusions, and 5 fusions of three or more levels. In all, 88 fusion levels were analyzed radiographically. The mean follow-up time was 28 months (range, 24

Successful treatment of an aggressive vertebral haemangioma with radiofrequency ablation and laminectomy: a case report.

BACKGROUND AND IMPORTANCE: Aggressive vertebral haemangiomas are rare, hypervascular spinal tumours that can extend beyond the vertebral body, causing spinal cord compression and neurological deficits. These lesions are particularly challenging to manage due to high intraoperative bleeding risk. Although treatments like embolisation, radiotherapy, and surgical decompression have been employed, each carries limitations. Radiofrequency ablation (RFA), commonly used in metastatic spine disease, offers a targeted approach to devascularise lesions prior to surgery and may reduce perioperative morbi

Multilevel meta-regression analysis of per- and polyfluoroalkyl substances toxicity to aquatic macrophytes.

Per- and polyfluoroalkyl substances (PFAS) are widely known for their toxicity and persistence in environmental matrices. Aquatic macrophytes, key components of aquatic ecosystems, are particularly susceptible to these compounds. This study provides the first multilevel meta-regression of PFAS toxicity to aquatic macrophytes, synthesizing 1091 effect sizes across 31 studies and 13 species. PFAS exposure consistently induced significant antioxidant enzyme activation (Hedges 'g = +0.44, p = 0.049) and substantial oxidative damage (Hedges 'g = -0.95, p = 0.020), with decline in total chlorophyll

Global Disparities in Cancer Care for Conflict-Affected Populations: Africa's Neglected Crisis.

Cancer care in conflict-affected settings represents a growing yet underrecognized global health crisis, disproportionately affecting populations in low- and middle-income countries, particularly in Africa. With over 130 million people forcibly displaced worldwide, conflict-driven disruptions to health systems have created profound gaps across the cancer care continuum, from prevention and diagnosis to treatment and palliative care. In these settings, cancer is often deprioritized in favor of acute and communicable diseases, resulting in delayed diagnosis, treatment interruptions, and poor out

Treatment of Steroid-Induced Osteonecrosis of the Distal Femoral Condyle Using Structural Bone Grafting.

BACKGROUND: Steroid-induced osteonecrosis of the distal femoral condyle is an uncommon condition that predominantly affects young patients receiving high-dose corticosteroid therapy. Joint-preserving treatment options for precollapse lesions remain limited, particularly in cases with extensive subchondral involvement. The purpose of this study was to evaluate the clinical and radiological outcomes of structural bone grafting (SBG) using a bicortical iliac bone graft (autograft or allograft) for precollapse osteonecrosis of the distal femoral condyle. METHODS: This retrospective case series in

Breast Cancer Metastases Presenting as Musculoskeletal Complaints.

Breast cancer often metastasizes to the bone, leading to musculoskeletal symptoms that may mimic benign conditions. Patients with undiagnosed or recurrent malignancies may initially present to first-contact providers, including chiropractors, with non-specific spinal or musculoskeletal pain. Here, we extend the series with two additional cases, highlighting risks of delayed diagnosis, the role of imaging, and chiropractic care. In Case 1, a 62-year-old woman with a history of breast carcinoma presented with progressive left-sided sciatica-like symptoms (pain radiating from the sacroiliac regio

Cooperation, Defection, and Collapse: A Multiscale Game Theory Framework for Emphysema Progression.

In the current paper, pulmonary emphysema is hypothesized to emerge from a nonlinear breakdown of cooperation across two tightly coupled systems: the extracellular matrix (ECM) crosslink network and the cellular populations responsible for its maintenance. To formalize this concept, we construct a game-theoretic model that unifies the mechanical failure, inflammatory changes, and percolation-driven tissue collapse that are recognized features of the disease. At the ECM level, elastin and collagen crosslinks are modeled as players in an iterated Prisoner's Dilemma, where cooperation corresponds
