Potential efficacy of crizanlizumab in treating priapism in sickle cell disease: A case report.

This report documents the treatment of a 41-year-old male with sickle cell disease (SCD) and repeated stuttering priapism using crizanlizumab, which alleviated the priapism but induced a significant vaso-occlusive crisis during the second infusion. Encouragingly, no subsequent vaso-occlusive crises occurred. However, the potential for infusion-related adverse events warrants close supervision. Further research is necessary to explore its full benefits on priapism management.

Priapism in children: a comprehensive review and clinical guideline.

OBJECTIVE: We review the English literature between 1980 and 2013 and summarize the clinical classification, aetiology, physiology, and pathophysiology of paediatric priapism. We propose a clinical guideline for the management of priapism in children. PATIENTS: Male patients aged ≤ 18 years. RESULTS: Priapism, a prolonged penile erection lasting >4 h, is a rare condition in childhood. There are 3 widely accepted types of priapism: 1) ischaemic priapism, the commonest type seen in children; 2) stuttering priapism, recurrent, self-limiting prolonged erections; and 3) non-ischaemic priapism, ra

[Callosal disconnection syndrome due to acute disseminated enchephalomyelitis].

We have reported a very rare case of a patient with callosal disconnection syndrome due to acute disseminated enchephalomyelitis (ADEM). A 54-year-old right-handed woman developed sudden consciousness disturbance and fever after 2 weeks when she had common cold. She did not have a history of stuttering. On admission, the neurological finding showed consciousness disturbance, and exaggerated muscle stretch reflex in four extremities without meningeal irritation. The cerebrospinal fluid examination revealed the cell counts of 273/mm3, the protein of 348 mg/dl and the myelin basic protein 18.3 pg

Response to Blomgren, Roy, Callister, and Merrill (2005).

PURPOSE: The purpose of this letter is to register concerns about a study reported by M. Blomgren, N. Roy, T. Callister, and R. Merrill (2005), entitled "Intensive Stuttering Modification Therapy: A Multidimentional Assessment of Treatment Outcomes." Concerns include questions about the description of the treatment, the duration of treatment time, and the results of the treatment. CONCLUSIONS: The treatment program did not inherently lend itself to clear description (would not be easily replicable), the participants did not improve their speech fluency, and the treatment time was extensive. The value of single-case research.

This paper presents an argument in support of clinic based research. The role that the clinic-based speech and language therapist has in advancing our knowledge of communication disorders and their treatment is emphasised. The value of single-case research within the field of speech and language therapy is discussed and the arguments are presented with respect to stammering. A single case study of pre-school dysfluency is used to illustrate the potential for conducting research using this methodology in a clinical environment.

The rise and fall of operant programs for the treatment of stammering.

In the sixties and seventies operant technology was used for the development of stammering treatment programs. Initial successes could not be maintained, which led to a marked decline. Some of the reasons for this are: lower effectivity than originally stated; limited clinical relevance; rigid structure which is uninspiring for the therapist; little room for individualizing the programs despite heterogeneity of clients. More recent treatments reduce these shortcomings with higher flexibility and use of cognitive techniques.

The neurological underpinnings of cluttering: Some initial findings.

BACKGROUND: Cluttering is a fluency disorder characterised by overly rapid or jerky speech patterns that compromise intelligibility. The neural correlates of cluttering are unknown but theoretical accounts implicate the basal ganglia and medial prefrontal cortex. Dysfunction in these brain areas would be consistent with difficulties in selection and control of speech motor programs that are characteristic of speech disfluencies in cluttering. There is a surprising lack of investigation into this disorder using modern imaging techniques. Here, we used functional MRI to investigate the neural co

A child with severe head banging.

We present a 7-year-old boy with a developmental disorder presenting with severe head banging. Clinical evolution was consistent with diagnosis of autistic spectrum disorder, obsessive compulsive disorder, stuttering, and Tourette's syndrome. This report emphasizes the overlap between developmental disorder phenotypes. There is a need to understand the natural history and relationship of specific symptoms that occur in developmental disorders to devise effective and appropriate intervention strategies.

The clinical analysis layer of transcription.

The clinical analysis layer of transcription is part of a 'toolkit' approach for detailed analysis of communication samples. This article provides a rationale for using the clinical analysis strategy and is supported by several examples of how a clinical analysis layer can add to the understanding of communication errors or other clinical areas of interest. The examples used in this paper are drawn from the areas of stuttering and aural rehabilitation and demonstrate the utility of this clinical analysis layer of transcription.

An update on mechanisms and treatment options for priapism.

Priapism is an abnormal prolonged penile erection that persists in the absence of any sexual stimulation. Priapism can be subcategorized into three types: ischaemic (low-flow or veno-occlusive), non-ischaemic (high-flow or arterial) and stuttering priapism. Ischaemic priapism is the most common subtype and is associated with multiple aetiologies, most commonly haemoglobinopathies and antipsychotic medications. The mechanisms underlying stuttering priapism are complex, and involve dysregulation of the nitric oxide signalling pathway, Rho-Rho kinase pathway, adenosine, opiorphins, oxidative stre

Validation of fragment analysis by capillary electrophoresis to resolve mixed infections by Cryptosporidium parvum subpopulations.

The potential of capillary electrophoresis (CE)-based DNA fragment analysis to identify mixed infections by Cryptosporidium parvum subpopulations was validated using high-resolution slab-gel electrophoresis. A selection of genomic DNA samples from C. parvum isolates with CE electropherogram profiles indicative of two concurrent alleles at one or more of six mini and microsatellite loci (MSB, MS5, ML1, ML2, TP14, 5B12) were analysed. These loci were PCR-amplified and products separated on precast Spreadex EL600 slab gels. ML1 PCR products differing by as little as 3 bp in length were visible af

[Speech disorders in ENT practice].

The most frequent speech and language disorders which ENT doctors are confronted with are generally known to be and presented as: delayed speech and language development, dystalia, dysglossia, rhinolalia, dysarthria, and verbal fluency disorders (stuttering, cluttering). The diagnostic portion in comparison to the therapeutic part is always greater and quite different. The close cooperation with representatives of phoniatrics and pedaudiology, as well as logopedics and other specialities such as neurology, and internal medicine is highly necessary.

Botulin ophthalmoplegia. Clinical and oculographic observations.

Two patients with botulin ophthalmoplegia had rapid quivering eye motions during attempts to refixate laterally placed objects; these abnormal eye movements occurred in conjunction with a disjunctive limitation of range of eye movements. Oculographic examination showed that the quivering motions were composed of multiple hypometric saccades, many of which had subnormal and stuttering velocities. By blocking acetylcholine release into myoneural junctions, botulin toxin seems to limit the duration of saccadic burst innervation reaching ocular muscle.

Denaturing high performance liquid chromatography for the detection of microsatellite instability using bethesda and pentaplex marker panels.

Microsatellite instability (MSI) is a characteristic molecular phenotype of tumors from the hereditary nonpolyposis colorectal cancer (Lynch) syndrome. Routine MSI screening of tumors in younger patients is an efficient prescreening tool for the population-based detection of Lynch syndrome in the absence of family cancer history. We describe here the optimization of a denaturing high performance liquid chromatography (DHPLC) assay for MSI analysis with the "Bethesda" panel of markers recommended by the National Cancer Institute and with a more recently proposed "pentaplex" panel of 5 mononucle
