A perspective on SIDS pathogenesis. the hypotheses: plausibility and evidence.

Several theories of the underlying mechanisms of Sudden Infant Death Syndrome (SIDS) have been proposed. These theories have born relatively narrow beach-head research programs attracting generous research funding sustained for many years at expense to the public purse. This perspective endeavors to critically examine the evidence and bases of these theories and determine their plausibility; and questions whether or not a safe and reasoned hypothesis lies at their foundation. The Opinion sets specific criteria by asking the following questions: 1. Does the hypothesis take into account the key …

Sleep and respiration in children: time to wake up!

The interest in paediatric sleep disorders over the last few decades has had its main focus on the sudden infant death syndrome (SIDS) - healthy infants who go to sleep and never wake up again. Overall, this is the most dramatic form of paediatric sleep disordered breathing. By contrast, classical presentations of sleep disordered breathing in children, such as snoring and obstructive sleep apnoea as well as their clinical implications have been greatly neglected and underestimated in the past. In contrast to snoring in adults, snoring in children has so far generally been regarded as noisy br…

Is There a Role for the Microbiome and Sudden Death? A Systematic Review.

BACKGROUND: Sudden unexpected death (SUD) is one of the most important and worthy investigation case profiles in emergency medicine and forensic pathology. Sudden unexpected deaths in adults (SUDA) are frequently caused by cardiac events, while infections usually cause those in infants younger than one year (SUDI), and to a lesser extent, in children older than one year (SUDC). However, in some instances of children under the age of one dying (SIDS), a cause is not discovered despite a thorough investigation that includes a review of clinical history, examination of the death scene, and a comp

The pineal gland.

The pineal gland is located posterior to the midbrain and is the site of melatonin production. Research on pineal gland function in neonates is very limited. This article will discuss pineal gland development and the possible relationship between melatonin production and sudden infant death syndrome. Further research on pineal gland function is needed in order to establish its significance for the neonate.

Complete heart block in nonfamilial hyperekplexia.

An infant with nonfamilial hyperekplexia had multiple episodes of tonic spasms that mimicked tonic seizures. They were accompanied by complete heart block and apnea. These episodes did not correlate with electroencephalographic epileptiform changes and were partially responsive to clonazepam, valproic acid, and cardiac pacemaker. Sudden death in hyperekplexia may be related to complete heart block and apnea during seizurelike episodes.

Chemoreceptor function and sleep state in apnea.

Resting ventilation and ventilatory responses to 100% oxygen and to 5% carbon dioxide in air were measured in REM and non-REM sleep in post-neonatal infants. Normal controls were compared to infants with prolonged apnea and to siblings of sudden infant death victims. No significant differences in ventilatory responses were found between the groups. We conclude that apnea may occur in infants whose central and peripheral chemoreceptor activity is normal while they are breathing.

The ventilatory response to carbon dioxide in high risk infants.

PURPOSE: To examine the ventilatory response to inspired carbon dioxide in infants considered to be at risk for sudden infant death syndrome or apnea. DESIGN: Clinical data measurement. SETTING: Infant apnea evaluation program of a university division of neonatology. PATIENTS: Fifty nine infants were full term characterized by the following diagnoses; siblings of infants who had died from sudden infant death syndrome (SIDS) (n = 7), apparent life threatening event (ALTE) (n = 24), apnea/cyanosis in the newborn nursery (n = 21), and controls. Sixty-nine infants were preterm and consisted of …

The role of alcohol in infant cosleeping deaths.

The well-established epidemiological and neuropathological differences between infants who die alone compared to those in shared sleeping environments indicate that these cohorts have differences. Risk factors in both situations depend on both exogenous and endogenous factors. Although parental consumption of alcohol continues to be cited as a risk factor for sudden infant death syndrome (SIDS) it is difficult to understand how a substance which impairs parental consciousness rather than infant arousal could contribute to SIDS. Surely a more logical mechanism to explain these deaths would be s…

Undiagnosed Congenital Heart Defects as a Cause of Sudden, Unexpected Death in Children.

Despite advances in the diagnosis and treatment of congenital heart defects (CHDs), these defects are still an important cause of sudden, unexpected death in young children. This retrospective study identified 64 cases of CHDs presenting as a cause of sudden, unexpected death in a busy, urban Medical Examiner's Office pediatric population between 2006 and 2016. The majority of cases (52 of 64, 81%) were infants. Interestingly, 52% of cases were undiagnosed prior to autopsy. Ventricular septal defects and atrioventricular septal defects were the most common simple (14%) and complex (17%) malfor

Secular trends in excess fetal and infant mortality using perinatal periods of risk analysis.

Perinatal periods of risk (PPOR) provide an alternative analytical approach to studying infant mortality. Results can be used to focus community activities to improve infant and maternal health. This article demonstrates the use of PPOR to monitor trends in excess fetal and infant mortality related to disparities associated with race and ethnicity in Kansas City, MO (KC). Based on a comparison of PPOR analyses for 1996-2000 and 2001-2005, there was a 30% reduction in excess fetal and infant mortality in Kansas City and reductions for both non-Hispanic blacks (17%) and non-Hispanic whites (66.7

Histological Changes in the Thyroid Gland in Cases of Infant and Early Childhood Asphyxia-A Preliminary Study.

A retrospective blinded study of thyroid gland histology was undertaken in 50 infants and young children aged from 1 to 24 months. Deaths were due to (i) suffocation (N = 7), hanging (4), wedging (3), and chest and/or neck compression (4), and (ii) SIDS (20), noncervical trauma (7), organic disease, (4) and drug toxicity (1). In the asphyxia group (N = 18), thyroid gland congestion ranged from 0 to 3+ with 39% of cases (7/18) having moderate/marked congestion. In three cases, focal aggregates of red blood cells (blood islands) were observed within the intrafollicular colloid. These deaths invo

Sudden unexpected death in infancy (SUDI) and myocarditis: State of the art and medico-legal implications. A systematic review.

Sudden infant deaths should be recognized as a public health concern, and understanding their underlying causes is crucial to elucidate this still poorly understood phenomenon. Despite efforts and standardized diagnostic protocols including autopsies and laboratory tests, the causes of sudden infant death often remain unclear in the 75 % of the cases. The aim of this review is to analyze the available data on SUDI associated with myocarditis, offering forensic pathologists a comprehensive overview of this topic, with a particular focus not only on pathophysiological and clinical implications b…

Decreased arousals in infants who sleep with the face covered by bedclothes.

OBJECTIVE: The risk of becoming a victim of sudden infant death syndrome is increased in infants who sleep with their face under bedding items. The present study was designed to evaluate auditory arousal thresholds of infants who sleep with their face covered by bedclothes. METHODS: Twenty healthy infants with a median age of 11.5 weeks (range: 4-22 weeks) were recorded polygraphically for 1 night. Although they slept in their usual supine position, a bed sheet was placed over their face for 60 minutes. Fifteen of the 20 infants were chosen at random and were exposed to white noises of increa…

Cardiorespiratory patterns in infants presenting with apparent life-threatening episodes.

A total of 305 infants presenting with apparent life-threatening events (ALE) were referred by their paediatricians and underwent 24-hour tape recordings of electrocardiogram and abdominal breathing movements (from a pressure capsule transducer). Seventy-seven of these infants, all full-term (greater than or equal to 37 weeks of gestation), were randomly selected, followed up for clinical outcome, and their recordings subjected to a detailed analysis of heart and respiratory rates and breathing patterns. Recordings on 157 age-matched, full-term controls were similarly analysed for comparison p
