Recurrent syncope in a child and video assisted thoracoscopic surgery - The long and short of it.

Long QT syndrome (LQTS) is a myocardial repolarisation disorder caused by cardiac ion channelopathy and one of its common presentations is recurrent syncope. This reduced repolarisation reserve in LQTS can be unmasked by perioperative factors like electrolyte imbalance, drugs, hypothermia and changes in cardiac autonomic tone. We report the anaesthetic management of left thoracoscopic sympathectomy in a 5-year-old child with LQTS and epicardial pacemaker in situ. It is very challenging to isolate the lung on one hand and prevent the predisposition to torsadogenic potential on the other.

Less Common Side Effects of Sympathetic Surgery.

Because of video-assisted thoracic technology and increased patient awareness of treatment options for palmar hyperhidrosis, endoscopic thoracic sympathectomy (ETS) has become a well-accepted treatment for this disorder. Video assistance affords excellent visualization of thoracic anatomy, which allows the procedure to be done quickly with few complications. However, despite the ease of performing ETS, complications can occur unless thoracic anatomy and physiology are well-understood. Awareness of possible intraoperative and postoperative complications is essential if this procedure is gong to…

Anesthetic Management of Urgent Cesarean Section with Undiagnosed Transposition of Great Arteries.

We consider that general anesthesia will be an appropriate and safe method as regional methods could lead to significant time loss in the prevention or minimization of complications. High blockage required for regional anesthetic methods in pregnant women with TGA and the maintenance of hemodynamic stability may become difficult due to neuroaxial sympathectomy even if a successful neuroaxial block has been provided. Agents with minimal effect on cardiovascular functions should be preferred for anesthesia induction and maintenance and close hemodynamic monitoring should be done during intraoper…

[Medical treatment of critical leg ischemia: current status and future perspectives of gene and cell therapy].

Critical limb ischemia (CLI) is associated with a high risk of amputation and death. For patients who cannot be surgically revascularized, medical options include prostanoids, spinal cord stimulation and lumbar sympathectomy, but none of these treatments has a demonstrated impact on the amputation rate at six months. Gene and cell therapy, aimed at stimulating angiogenesis, have mainly been tested in phase I and II clinical trials. These approaches appear to be feasible and safe in the short-term, but large randomized studies are necessary to demonstrate their clinical benefits and long-term s…

The protective effect of brachial plexus palsy in purpura fulminans.

Acute infectious purpura fulminans is reported in a 16-month-old male with a history of posttraumatic asplenia and complete left brachial plexus palsy. This patient developed peripheral necrosis of both lower extremities and the right upper extremity, whereas the left upper extremity was completely spared from ischemia and tissue damage. Amputation of four digits on the right hand and debridement of both lower extremities were required. This patient demonstrated the protective effect of a traumatic sympathectomy, which suggests the requirement of an intact sympathetic reflex in the development…

Comparison of the effectiveness and safety between ramicotomy and sympathetic chain interruption in the treatment of primary hyperhidrosis: a meta-analysis.

OBJECTIVE: This meta-analysis evaluates the effectiveness and safety of ramicotomy versus sympathetic chain interruption (SCI) in treating primary hyperhidrosis (PH). METHODS: A comprehensive literature search was conducted across multiple databases, including PubMed, Embase, Web of Science, Ovid, Cochrane Library, CNKI, and Wanfang, covering studies from their inception through October 2024. A total of 10 studies involving 970 patients were included, with 504 patients undergoing ramicotomy and 466 receiving SCI. RESULTS: The analysis revealed that patients undergoing ramicotomy experienced

Sympathicotomy under local anesthesia: a simple way to treat primary hyperhidrosis.

General anesthesia has been the standard approach for sympathicotomy to treat primary hyperhidrosis. Sympathicotomy through thoracoscopic guidance is a straightforward procedure with a short operation time. Therefore we propose using local anesthesia for the procedure. The advantages of local anesthesia include avoiding endotracheal intubation and systemic anesthetics, and allowing immediate assessment by means of verbal communication with the patient. We effectively treated primary hyperhidrosis in 3 patients by thoracoscopic sympathicotomy under local anesthesia.

Quality of Life Changes Following Surgery for Hyperhidrosis.

The best way to evaluate the impact of primary hyperhidrosis on quality of life (QL) is through specific questionnaires, avoiding generic models that do not appropriately evaluate individuals. QL improves significantly in the short term after sympathectomy. In the longer term, a sustained and stable improvement is seen, although there is a small decline in the numbers; after 5 and even at 10 years of follow-up it shows virtually the same numerical distribution. Compensatory hyperhidrosis is a major side effect and the main aggravating factor in postoperative QL, requiring attention to its mana…

Restoring autonomic balance: future therapeutic targets.

Our aim was to highlight the very recent and rapid expansion of a number of contemporary, pioneering and projected interventional approaches for resistant hypertension. Our discussions of each approach centre on a new model of the cardiovascular system that emphasises the importance of the sympathetic nervous system, its differential control and reflex activation. Four interventional therapeutic strategies are discussed including targeted sympathectomies and carotid body ablation. Harnessing endogenous homeostatic control systems looks promising as a means to correct autonomic balance. The cha

Familial primary hyperhidrosis: a disabling social disease.

The incidence of primary hyperhidrosis ranges between 2-5%. This condition is more common than previously thought of and has been linked to a familial predisposition. It is socially disabling to the affected individuals. In the current era of minimally invasive surgery, the video assisted thoracoscopic sympathectomy has become the definitive treatment with minimal complications. We report on a case of two siblings with primary hyperhidrosis that was treated successfully at our institution with the aim to share our experience and attract awareness among medical practitioners of the availability…

Tibial artery false aneurysm: uncommon result of blunt injury occurring during athletics.

Two young men presented with symptoms following lower extremity injuries sustained in the normal course of participation in sports. One played baseball while the other competed in Tae Kwon Do. One case presented with digital ischemia, the other developed a pulsating hematoma. Each came to angiography, and each proved to have a false aneurysm of a tibial artery. The tibial artery was ligated in each case, without further complications. The patient with digital ischemia was thought to have sustained microemboli, and also underwent lumbar sympathectomy.

Right stellate ganglion stimulation modulates arrhythmogenesis in acute left lateral ventricular ischaemia.

AIMS: Acute myocardial ischaemia causes fatal arrhythmias as result of a flow of 'injury current'. Left stellate ganglion stimulation (LSGS) … SGS is contingent on the location of ischaemic zone supporting the clinical findings that bilateral sympathectomy is superior to left sympathectomy alone.

The history and development of sympathetic trunk reconstruction: a 40-year journey from open surgery to robotic-assisted microsurgery.

Endoscopic thoracic sympathectomy (ETS) is widely used to treat primary hyperhidrosis; however, compensatory sweating (CS) has emerged as a significant complication. Various surgical approaches for managing CS have evolved over the past 4 decades, yet their development and outcomes remain poorly documented. This study aims to examine the historical evolution and advances in techniques for reconstruction of the thoracic sympathetic trunk. A review was conducted using PubMed, Embase, and the Cochrane library, focusing on currently published studies related to the reversal surgery of thoracic sym…
