Emerging therapies for osteoporosis.

This article reviews the conceptual framework for agents that are antiresorptive or anabolic, including pathways that affect bone formation and resorption, and the steps in those pathways that are targets for new therapeutic agents. This article discusses novel antiresorptive and anabolic agents in development. Recent developments that link bone remodeling with serotonin in the gastrointestinal system and the central nervous system via the sympathetic nervous system may change the paradigm for skeletal remodeling. Novel anabolic agents in development include antibodies that target molecules in…

[The type of age-related cataract as available marker of socially significant diseases of the organism].

The aim of the research was to investigate the general somatic status and to investigate the influence of dominating part of vegetative nervous system, following the different types of senile cataract formation in human. Thus, the domination а sympathetic nervous system and correlated systemic dystrophic changes of the tissues in patients with cortical cataract have been found for the first time. Moreover, patients with nuclear cataract had the domination of parasympathetic nervous system and another character of dystrophic changes. Therefore, the type of a forming age-related cataract can be …

Parkinson's disease with involvement of the parasympathetic ganglia.

We report the distribution of Lewy bodies in an 83-year-old man who was diagnosed histopathologically as having Parkinson's disease. Many sections were taken from the brain, spinal cord, spinal dorsal root ganglia and peripheral autonomic systems. In the central nervous system, Lewy bodies were present in many of the areas already reported by previous authors. In the peripheral nervous system, Lewy bodies were present in the sympathetic ganglia, enteric nervous system of the alimentary tract and the submandibular ganglion, which is a peripheral parasympathetic ganglion. The present case indica

Exercise Intervention in Autonomic Function, Immunity, and Cardiovascular Health: A Precision Medicine Approach.

The imbalance in the interaction between the autonomic nervous system and the immune system serves as a central mechanism in the onset and progression of cardiovascular diseases. The excessive activation of the sympathetic nervous system and suppression of vagal function contribute to chronic inflammation and cardiac remodeling. Precision medicine, by integrating multidimensional data such as genomics and metabolomics, offers a novel perspective for the personalized design of exercise interventions. This systematic review explores the bidirectional regulatory mechanisms of exercise interventio…

Hemodynamic patterns and spectral analysis of heart rate variability during dialysis hypotension.

Intradialytic hypotension, a major source of morbidity during hemodialysis and ultrafiltration, is often accompanied by paradoxical bradycar … with and without hypotension. The low frequency (LF) component of HRV is thought to correlate with sympathetic nervous system activity, the high frequency (HF) component with that of the parasympathetic nervous system. In the sessions not complicated by symptomatic hypotension (n = 26), mean arterial BP (MAP) hardly fell, whereas heart rate increased from 77 +/- 2 to 89 +/ …

Efficacy of atropine combined with paroxetine in vagus nerve excitatory panic disorder.

Panic disorder is often associated with the autonomic nervous system pattern - sympathetic activation and parasympathetic (vagal) withdrawal. However, we present one special case here to show a totally reversed pathogenesis - vagal activation occupying the leading role, which requires atropine to cure the patient's symptoms. Through this report, it is reasonably proven that panic disorder may be a heterogeneous condition, whose mechanism might be the imbalance between the sympathetic and parasympathetic tone.

Sympathetic activation in heart failure.

Sympathetic activation has been long appreciated exclusively as a fundamental compensatory mechanism of the failing heart and, thus, welcome and to be supported. In the initial clinical phases of heart failure (HF), the sympathetic nervous system overdrive plays a compensatory function aimed at maintaining an adequate cardiac output despite the inotropic dysfunction affecting the myocardium. However, when the sympathetic reflex response is exaggerated it triggers a sequence of unfavourable remodelling processes causing a further contractile deterioration that unleashes major adverse cardiovasc…

[Evaluation of autonomic nervous function by pupil dynamics recording].

Pupil is richly innervated by autonomic nerves. Therefore, its size or movement well reflects the autonomic nervous function. By analyzing dynamics of pupil movement for light stimulus followed by infrared video-pupillography, the abnormality of autonomic nervous system can be expressed in a quantitative manner. Clinical applications of pupillography and pharmacological examination to Horner syndrome, Adie syndrome, diabetes mellitus, pupillary involvement of Behçet disease, pupil of brain death and patients with chemical sensitivity have been described. Segmental spasm of iris in Horner and A

Antagonistic and Synergistic Activation of Cardiovascular Vagal and Sympathetic Motor Outflows in Trigeminal Reflexes.

The trigeminal nerve and heart are strongly related through somato-autonomic nervous reflexes that induce rapid changes in cardiovascular function. Several trigeminal reflexes have been described, but the diving and trigeminocardiac reflexes are the most studied. The heart is a target organ dually innervated by the sympathetic and parasympathetic systems. Thus, how cardiac function is regulated during the trigeminal reflexes is the result of the combination of an increased parasympathetic response and increased, decreased, or unaltered sympathetic activity. Various hemodynamic changes occur as

When do the symptoms of autonomic nervous system malfunction appear in patients with Parkinson's disease?

BACKGROUND/AIM: Dysautonomia appears in almost all patients with Parkinson's disease (PD) in a certain stage of their condition. The aim of … malfunction was defined by Ewing diagnostic criteria. The tests, indicators of sympathetic and parasympathetic nervous systems, were significantly different in the PD patients as compared with the controls, suggesting the failure of both systems. However, it was shown, in the selected groups of patients, that the malfunction of both systems was present in two treated group …

Sympathetic rhythms during hyperventilation-induced apnea.

The effect of hyperventilation-induced apnea on the respiratory rhythmicity of sympathetic nerve activity was determined using spectral analysis of sympathetic nerve frequencies. Left phrenic, external intercostal, and inferior cardiac sympathetic nerves were recorded in alpha-chloralose-anesthetized, vagotomized, paralyzed, artificially ventilated cats. The respiratory modulation of sympathetic activity during normoventilation was indicated by spectral peaks of sympathetic activity coinciding with respiratory frequencies determined from the phrenic nerve activity of each cat. The spectral pea

Spleen glia are a transcriptionally unique glial subtype interposed between immune cells and sympathetic axons.

Glia are known to play important roles in the brain, the gut, and around the sciatic nerve. While the gut has its own specialized nervous system, other viscera are innervated solely by autonomic nerves. The functions of glia that accompany autonomic innervation are not well known, even though they are one of the most abundant cell types in the peripheral nervous system. Here, we focused on non-myelinating Schwann cells in the spleen, spleen glia. The spleen is a major immune organ innervated by the sympathetic nervous system, which modulates immune function. This interaction is known as neuroi…

Sympathetic activity at rest and motor brain areas: FDG-PET study.

Although recent studies identified brain areas which are involved in short term activation of the sympathetic nervous system, little is known about brain mechanisms which generate the individual variability of basal autonomic activity. In this fluorodeoxyglucose positron emission tomography study (FDG-PET), we aimed to identify brain regions, which covary with function parameters of the autonomic nervous system at rest. Therefore, FDG-PET (Siemens, Germany) was performed twice in 14 healthy resting subjects (7 m, 7 f; mean age 29.5 years) while different parameters of autonomic function were a…

Comparison of [123I]metaiodobenzylguanidine kinetics with heart rate variability and plasma norepinephrine level.

BACKGROUND: [123I]Metaiodobenzylguanidine (MIBG) imaging has been used to assess cardiac sympathetic nerve abnormalities. We evaluated the clinical significance of myocardial MIBG imaging as a measure of cardiac sympathetic nervous system function by comparing it to heart rate variability and plasma norepinephrine level. METHODS AND RESULTS: In 211 subjects, we analyzed heart rate variability with 24-hour electrocardiography, performed scintigraphy with MIBG, and measured plasma norepinephrine levels. Time and frequency domain measures of heart rate variability were calculated with the Marque…

A case of severe trigeminal neuralgia: recovery by means of stellate ganglion block with procaine. Discussion of possible mechanisms of action.

Drug and invasive treatment options for trigeminal neuralgia (TN) are often fraught with problems. Knowledge of the involvement of the autonomic nervous system, especially the sympathetic portion, in the pain process has grown rapidly in recent years. Both nociceptive and neuropathic pain can be maintained by the sympathetic nervous system, known as 'sympathetically maintained pain' (SMP). This current case report describes a patient with refractory TN that was treated with a stellate ganglion block (SGB). After the first SGB, the patient experienced significant pain relief that became long-la…

Control of postganglionic neurone phenotype by the rat pineal gland.

As neurones develop they are faced with choices as to which genes to express, to match their final phenotype to their role in the nervous system. A number of processes can guide these decisions. Within the autonomic and sensory nervous systems, there are a handful of examples that suggest that one mechanism that may match phenotype to function is the presence of target-derived differentiation factors. We tested whether the rat pineal gland controls the expression of a neuropeptide (neuropeptide Y) and a calcium-binding protein (calbindin) in sympathetic postganglionic neurones that innervate i
