Alternatives to the treatment of major injuries to the proximal interphalangeal (PIP) joint include fusion, implant arthroplasty, perichondrial grafting, single autogenous free vascularized transfer, and double autogenous free vascularized transfer. A patient presented a gunshot wound to the index finger with loss of skin and extensor tendon and PIP joint disruption. The finger was reconstructed with a composite free flap of skin and extensor tendon and PIP and distal interphalangeal joints of the second toe. A follow-up of 10 months is presented, which demonstrates PIP joint motion and finger

Structural and mechanical changes in muscles and muscle cells were studied after spinal cord injury with tetraplegia as well as secondary to … mechanics has been critical for development of several therapeutic treatments including both muscle tendon transfer surgery and rehabilitation. Successful tendon transfer procedures improve patient function, which facilitates employability and allows patient to regain independence and reduce the need for a constant care.

A one-stage flap repair is described for complicated dorsal injuries of the hand involving loss of skin and tendon based on the principle of the "Chinese" radial artery forearm flap in which vascularised tendons are transferred to reconstruct the missing extensor tendons.

Fractures of ossified tibialis anterior tendon have been successfully managed by the excision of the ossified tendon and transfer of the extensor hallucis longus tendon. A 64-year-old man sustained an injury during mountain hiking 2 weeks prior to presentation, falling down on his heels with his ankle fully plantarflexed. Two tender and bony hard masses were palpable along the course of the tibialis anterior tendon, one at the anteromedial aspect of the ankle and another at the dorsum of the talonavicular joint. Radiographs of the ankle demonstrated an approximately 2x1-cm ovoid-shaped bony ma

An 8-year-old boy presented with inability to extend his fingers. Examination revealed congenital hypoplasia of the extensor tendons. He was treated with tendon transfers. We present a review of the literature and management of such cases.

Two novel operations for cerebral palsy are described. Both procedures, originally proposed and performed by Gage, have been advocated as biomechanically more effective than previous procedures in achieving the objectives of eliminating hip and knee contractures in cerebral palsy. Description of the operative technique and the rationale of use are stated to assist the operator.

The technique of congenital clubfoot surgical treatment based on independently described method by Mc Kay and Simons is delineated. We have modified skin incision making it ca 2 cm above the calcanean tuber in order to facilitate an access for elongation of the calcaneal tendon. In some cases the procedure was supplemented with cuneonavicular and first tarsal-metatarsal capsulotomy and tibialis anterior muscle tendon transfer to the dorsum of the first metatarsal bone.

Thirteen patients were operated on for hand palsies in cases of C7 to T1 or C8, T1 root avulsions. Finger flexion and intrinsic function were paralyzed in all patients. Finger extension was paralyzed in 12 patients. Wrist flexion and extension were present in all patients. Tendon transfers were performed to restore the different functions. The extensor carpi radialis longus was transferred to the flexor digitorum profundus. The brachioradialis tendon was transferred to the flexor pollicis longus tendon for thumb flexion, with a tendon translocation procedure in 6 patients. Intrinsic function w…

Posterior tibial tendon dysfunction (PTTD) is a progressive disorder secondary to advanced degeneration of the posterior tibial tendon, leading to the abduction of the forefoot, valgus rotation of the hindfoot, and collapse of the medial longitudinal arch. Eventually, the disease becomes so advanced that it begins to affect the deltoid ligament over time. This attenuation and eventual tear of the deltoid ligament leads to valgus deformity of the ankle. Surgical correction of PTTD is performed to protect the ankle joint at all costs. Generally, this is performed using osteotomies of the calcane

INTRODUCTION: Tibialis posterior tendon insufficiency in adult acquired flat foot deformity (AAFFD) is treated by reinforcing the posterior tibial tendon (PTT) using grafts from flexor hallucis longus (FHL) and flexor digitorum longus (FDL). The communication between FHL and FDL will influence the length of the graft that can be harvested from FHL and FDL. In this study, we aim to study the patterns of communications between FHL and FDL tendons and the location of Master Knot of Henry (MKH) and point of division of FDL tendons in Indian population.
MATERIALS AND METHODS: In this observational

An understanding of the processes of tendon healing and tendon-to-bone healing is important for the intraoperative and postoperative management of patients with tendon ruptures or of patients requiring tendon transfers in foot and ankle surgery. Knowledge of the normal process allows clinicians to develop strategies when normal healing fails. This article reviews the important work behind the identification of the normal phases and control of tendon healing. It outlines the failed response in tendinopathy and describes tendon-to-bone healing in view of its importance in foot and ankle surgery.
