Medical end-of-life decision-making in a small resource-poor Caribbean country: a mortality follow-back study of home deaths.

BACKGROUND: In resource-poor countries, including in Latin American and the Caribbean, empirical information about the characteristics and incidence of medical end-of-life decisions (MELDs)-withholding or withdrawing potentially life-prolonging medical treatments-is largely absent. METHODS: The aim was to describe the incidence and decision-making characteristics of MELDs taken prior to the death of people who died at home in Trinidad and Tobago (T&T). A mortality follow-back study was used where a representative sample of deaths occurring at home in 2018 was drawn from death certificates at

A proposed guideline for vagus nerve stimulator handling in palliative care and after death.

Vagus nerve stimulation (VNS) is often used for patients with drug-resistant epilepsy. Although this intervention may improve seizure control and mood, a number of factors must be considered when patients with VNS near end of life. We reviewed relevant literature to create a proposed guideline for management of patients with VNS in palliative care and after death. VNS has multiple possible side effects, including cough and swallowing difficulties. For patients with neurologic disease in palliative care, such adverse effects can severely affect quality of life and increase the risk for complica

Perinatal hypoxic-ischaemic encephalopathy: a national survey of end-of-life decisions and palliative care.

OBJECTIVE: To explore end-of-life (EoL) decision-making and palliative care in hypoxic-ischaemic encephalopathy (HIE) nationwide. METHODS: A cross-sectional national study on moderate-to-severe HIE in newborns ≥35 weeks' gestational age in 2015, including all 57 level III units that offered hypothermia. Forty-one questions were included to explore how the prognosis is established, as well as timing of the decision-making process, and also how ongoing palliative care is offered. RESULTS: The main difficulties in EoL decisions lie in the scarce time to make an early, accurate prognosis. Only 2

Palliative care in children with spinal muscular atrophy type I: What do they need?

OBJECTIVE: Our aim was to describe the clinical evolution and needs of children with spinal muscular atrophy type I treated in a domiciliary palliative care program. METHOD: We undertook a retrospective chart review of nine consecutive patients. Descriptions of the clinical and demographic profile of children with spinal muscular atrophy (SMA) type I were referred to a pediatric palliative care team (PPCT). RESULTS: Six males and three females were admitted to the PPCT, all before six months of age, except for one afflicted with SMA type I with respiratory distress. The median time of attent

End-of-life care considerations for gay, lesbian, bisexual, and transgender individuals.

Negative social attitudes, discrimination, and homophobia affect gay, lesbian, bisexual, and transgender (GLBT) individuals during their lifetimes. These experiences can affect how these individuals access health services and interact with health professionals, resulting in adverse outcomes compared with their heterosexual counterparts. End-of-life experiences can also be shaped by these factors. There are implications for health professionals in terms of equity of access to targeted health care, preventive screening, and visibility in policy, as well as in principles of inclusiveness, dignity

The (dis)appearance of the dying patient in generalist hospital and care home nurses' talk about the patient.

This article explores interview data from a study of 50 Norwegian generalist nurses' focus group accounts of caring for dying patients in the hospital and care home. An eclectic discourse analytic approach was applied to nurses' accounts of the patient and three discursive contexts of reference to the patient were identified: the 'taken as read' patient, the patient paired with particular characteristics and the patient as psychologically present. Talk about the patient falls mainly into the first two contexts, which position the patient in relation to three closely related discursive processe

Ethico-legal issues in relation to end-of-life care and institutional mental health.

OBJECTIVES: In view of the fact that there is a higher mortality rate for individuals with serious mental illness and that people with mental illness suffer excess mortality due to physical illness, the lack of attention to end-of-life care for patients with a serious mental illness needs addressing. This article seeks to put these issues on the agenda by beginning to explore important ethical-legal issues at the interface of palliative care and institutional mental health. METHODS: Data were collected from eight qualitative interviews conducted with mental health professionals at The Park, C

Palliative sedation in dying patients: "we turn to it when everything else hasn't worked".

Despite skilled palliative care, some dying patients experience distressing symptoms that cannot be adequately relieved. A patient with metastatic breast cancer, receiving high doses of opioids administered to relieve pain, developed myoclonus. After other approaches proved ineffective, palliative sedation was an option of last resort. The doctrine of double effect, the traditional justification for palliative sedation, permits physicians to provide high doses of opioids and sedatives to relieve suffering, provided that the intention is not to cause the patient's death and that certain other c

[Our experience of home parenteral nutrition (HPN) for digestive disease patients].

The number of patients receiving home parenteral nutrition (HPN) has been increasing mainly among end-stage cancer patients. In our hospital, twenty-four patients received HPN therapy with digestive disease from April 1995 to May 1998. Most of them were in the terminal stage of advanced gastric cancer. Many patients received other treatments such as narcotics, blood transfusion, chemotherapy, and so on with HPN. Their average age was 71 years old. The duration of HPN varied from 4 to 180 days (mean: 50.9 days). The duration of intravenous hyperalimentation varied from 14 to 300 days (mean: 98.

Oncology-embedded supportive care and end-of-life outcomes: a comparative study of hospice-eligible Muslim patients with cancer and non-cancer diagnoses.

PURPOSE: Oncology-embedded supportive care facilitates comfort-focused end-of-life care in cancer patients, but its effectiveness in Muslim-majority settings is unknown. We compared end-of-life outcomes between hospice-eligible Muslim cancer patients managed within this model and non-cancer patients without such integration. METHODS: This retrospective cohort study (2021-2024) at a Middle Eastern tertiary hospital included hospice-eligible Muslim adults who died during an acute-care admission. Cancer patients were managed by an oncology-embedded supportive service and compared with non-cancer

Evolving end-of-life care patterns for hepatocellular carcinoma in the United States: A 25-year national analysis of disparities in home and hospice utilization.

BACKGROUND AND AIMS: Hepatocellular carcinoma (HCC) remains one of the leading causes of cancer-related mortality worldwide and in the United States. In recent decades, there has been a growing emphasis on end-of-life care delivered in home or hospice settings, yet detailed contemporary insights into how these patterns differ by sex, race/ethnicity, region, and urbanization remain sparse. This study aimed to characterize the trends in the proportion of HCC descendants receiving home or hospice care over a 25-year period, using data derived from the CDC WONDER database. METHODS: Data for HCC-r

Establishing a Health System Policy for Proportionate Palliative Sedation.

Proportionate palliative sedation (PPS) is an important therapeutic option for patients at the end of life who experience intractable suffering despite use of all conventional interventions. Clinical guidelines for PPS differ between institutions, often tailored to local practice patterns and the regulatory environment. While professional organizations encourage institutions to develop tailored PPS guidelines, the literature lacks robust discussion of the development process. In this article, we present two cases from Yale New Haven Hospital, and explore the associated practical and ethical ch

Teaching Where Life Ends: Peer-Assisted Learning in Hospice as a Model for End-of-Life Nursing Education.

Nursing educational programs are expanding clinical learning opportunities to encompass the hospice/palliative/supportive sphere of care, as outlined in the American Association of Colleges of Nursing Essentials to prepare practice-ready nursing graduates. This article outlines one school's journey to integrate end-of-life clinical experiences across the curriculum, starting with beginning nursing students using a peer-assisted learning model at an inpatient hospice facility. Pairing senior and beginning nursing students using the peer-assisted learning model together in the hospice setting re

A Case Report on Home-Based End-of-Life Care with Traditional Korean Medicine Acupuncture and Herbal Medicine for an Elderly Patient with Parkinson's Disease.

BACKGROUND: Managing multiple symptoms in home-based palliative care (HBPC) is challenging. Acupuncture and herbal medicine have been widely used to alleviate symptoms in patients with cancer receiving palliative care, but their role in HBPC for nonmalignant diseases such as Parkinson's disease (PD) remains underexplored. CASE PRESENTATION: We report the case of an 86-year-old woman with end-stage PD who received HBPC. She suffered from seborrheic dermatitis, constipation, and dyspnea, which significantly impaired her quality of life. Conventional treatments provided limited relief, prompting

Pharmacologic pearls for end-of-life care.

As death approaches, a gradual shift in emphasis from curative and life prolonging therapies toward palliative therapies can relieve significant medical burdens and maintain a patient's dignity and comfort. Pain and dyspnea are treated based on severity, with stepped interventions, primarily opioids. Common adverse effects of opioids, such as constipation, must be treated proactively; other adverse effects, such as nausea and mental status changes, usually dissipate with time. Parenteral methylnaltrexone can be considered for intractable cases of opioid bowel dysfunction. Tumor-related bowel o
