Autotransplantion of a displaced mandibular second premolar to its normal position.

The patient was a 13-year-old Korean girl who had a displaced mandibular second premolar. She was reluctant to undergo a lengthy orthodontic treatment and opted instead for transplantation of the premolar to its usual site. On the basis of computed tomography, a replica tooth model was manufactured to shorten the extraoral time, and a root canal treatment was performed because root formation was complete. No negative signs or symptoms were found during a 3-year follow up. Autotransplantation for this patient obviated the need for orthodontic traction and prosthetic therapy.

Rehabilitative treatment of cleft lip and palate: experience of the Hospital for Rehabilitation of Craniofacial Anomalies-USP (HRAC-USP)--part 3: oral and maxillofacial surgery.

This paper presents the treatment protocol of maxillofacial surgery in the rehabilitation process of cleft lip and palate patients adopted at HRAC-USP. Maxillofacial surgeons are responsible for the accomplishment of two main procedures, alveolar bone graft surgery and orthognathic surgery. The primary objective of alveolar bone graft is to provide bone tissue for the cleft site and then allow orthodontic movements for the establishment of an an adequate occlusion. When performed before the eruption of the maxillary permanent canine, it presents high rates of success. Orthognathic surgery aims

Efficacy of collagenated bone substitutes for bone regeneration in two-wall-damaged extraction sockets without barrier membranes.

OBJECTIVES: To evaluate ridge dimensional changes following alveolar ridge augmentation in two-wall-damaged extraction sockets without a barrier membrane, using two types of collagenated bone substitutes i. cross-linked (CL-CB) and ii. non-cross-linked (NCL-CB). MATERIALS AND METHODS: Two-wall defects were created in mandibles of seven beagle dogs and treated in three groups: (i) no grafting (control), (ii) sockets filled with NCL-CB, and (iii) sockets filled with CL-CB. No barrier membrane was used. One animal was sacrificed after 1 week of healing for tissue assessments (n = 1), and the oth

Increased local concentrations of growth factors from leucocyte- and platelet-rich fibrin do not translate into improved alveolar ridge preservation: An intra-individual mechanistic randomized controlled trial.

AIM: Leucocyte- and platelet-rich fibrin (L-PRF) has been tested for enhancing alveolar ridge preservation (ARP), but little is known about the local release profile of growth factors (GF), and the clinical equipoise related to its efficacy remains. This study compared the patterns of GF release, early soft tissue healing, and alveolar ridge resorption following unassisted healing and L-PRF application in non-molar extraction sockets. MATERIALS AND METHODS: Atraumatic tooth extraction of two hopeless teeth per patient was followed by unassisted healing or L-PRF placement to fill the socket in

Predicting bone and soft tissue alterations of immediate implant sites in the esthetic zone using clinical parameters.

BACKGROUND: Immediate implantation is generally a predictable treatment, but sometimes there are significant tissue alterations at the surgical site which compromise clinical outcomes. PURPOSE: This study aimed to investigate the association between tissue alterations and different clinical parameters in esthetic areas following immediate implant placement and provisionalization. MATERIALS AND METHODS: Clinical parameters were measured at 36 non-grafted immediate implant sites enrolled in a randomized controlled trial. Alterations of bone and soft tissue were measured at 12 months after the

Preclinical alveolar ridge preservation using small-sized particles of bone replacement graft in combination with a gelatin cryogel scaffold.

BACKGROUND: Particulate anorganic bovine bone matrix (ABBM) is extensively used for alveolar ridge preservation (ARP). This study evaluates the potential of ABBM/gelatin cryogel scaffold of regular (250 to 1,000 μm) and small (50 to 100 μm) particles for ARP. METHODS: ABBM was either condensed in a gypsum ring to simulate filling of ABBM in a defect or incorporated into a gelatin cryogel scaffold to disperse ABBM. Condensed regular-sized or small-sized ABBM (rABBM or sABBM) (± bone morphogenetic protein [BMP]-2) were subcutaneously implanted in rats to evaluate biocompatibility and osteogenic

Clinical relevance of dimensional bone and soft tissue alterations post-extraction in esthetic sites.

The key to achieving pleasing esthetics in implant dentistry is a thorough understanding of the biological processes driving dimensional bone and soft tissue alterations post-extraction. The aim of the present report is first to characterize the extent of bone and soft tissue changes post-extraction and second to identify potential factors influencing tissue preservation in order to facilitate successful treatment outcomes. The facial bone wall thickness has been identified as the most critical factor influencing bone resorption and can be used as a prognostic tool in order to identify sites a

Clinical Management of Type 3 Recession Defects With Immediate Implant and Provisional Restoration Therapy: A Case Report.

Type 3 extraction sockets present a unique challenge in that they possess gingival recession indicative of facial hard- and soft-tissue loss. When teeth present with prior disease requiring removal and implant replacement, the treatment strategy incorporates palatal implant positioning as well as proper restorative contour management to compensate for the recession defect, thereby allowing the gingival tissues to heal in the correct 3-dimensional position. This article describes the case of a patient with a nonrestorable maxillary right central incisor with internal resorption. The case demons

Part II. Minimizing alveolar bone loss during and after extractions. Protocol and techniques for alveolar bone preservation.

Alveolar ridge resorption accelerates following extraction of teeth and the residual defect varies from socket to socket. This article proposes a new treatment oriented classification of extraction defects. It also reviews several graft materials and membranes that aid in the decision for selecting an appropriate socket preservation technique. The algorithm developed by the authors helps design a potentially successful treatment plan based on the classification of extraction defects, with choices ranging from no treatment to complex grafting procedures (i.e. allogenic block grafts). In additio

[Alveolar ridge preservation following tooth extraction].

Alveolar bone resorption following tooth extraction is not uncommon. The dimensions of the alveolar bone are of great importance from both esthetic and implant planning reasons. This paper reviews the rational of ridge preservation following tooth extraction in order to minimize bone loss. The different treatment modalities for that purpose will also be discussed. When extracting a tooth, the dental professional should consider performing a ridge preservation procedure according to the accepted indications. The knowledge of the various options as well as their pros and cones will help the clin

Alveolar wound healing alteration under uranyl nitrate intoxication.

The deposit of uranium compounds in calcifying zones has been demonstrated in bone. Nevertheless, no studies on the effect of uranium on osteogenesis have been performed. A histologic and histometric study of the effect of a single intraperitoneal injection of 2 mg/kg of body weight of uranyl nitrate on bone formation is presented. It was performed on rats' healing sockets, 14 days after tooth extraction. The alveolar bone volume (15 X 10(5) micron vs. 34 X 10(5) micron2), total bone formation areas (4.85% vs. 19.55%), and volume density of bone in the alveolar apical third (0.26 vs. 0.40) wer

Multiple myeloma presenting as an unhealed extraction socket: Report of a case with brief review of literature.

Multiple myeloma (MM) is a relatively rare malignant hematological disease, which is characterized by a monoclonal malignant proliferation of plasma cells that causes osteolytic lesions. Maxillofacial presentations in patients with MM are not uncommon, but because the symptoms are varied, it is very difficult to diagnose MM in this region especially in patients with initial oral involvement. Furthermore, maxillofacial manifestations as an initial sign or symptom are scarce. We report a case of a 40-year-old male patient who presented with an unhealed socket in lower left back tooth region for

The "washing line" suture technique for securing the Subepithelial Connective Tissue Graft.

Following tooth extraction, resorption of the buccal wall of the socket will occur; this will be true for both the maxilla and the mandible. Where the extraction site is surrounded by natural dentition, the loss of the buccal alveolar plate can degrade the visual aesthetics of an implant-supported prosthetic rehabilitation. To aid the harmonization of the hard and soft tissue morphology, both hard and soft tissue augmentation can be carried out either consecutively with an extraction/immediate implant placement or prior to an implant placement in the delayed scenario. The contemporary method o

Rotated pedicled palatal flaps for primary closure in immediate post-extractive guided bone regeneration at maxillary molar locations: A case report of a new technique.

BACKGROUND: Replacing severely compromised teeth with endo-periodontal lesions continues to pose a clinical challenge because of accompanying deficiencies in hard and soft tissues. While traditional staged protocols, involving vertical ridge augmentation and soft tissue grafting, are effective, they are complex, time-intensive, and often not well-received by patients. This case report presents a simplified approach that combines immediate guided bone regeneration (GBR) using a rotated pedicled palatal flap to achieve primary closure without requiring buccal flap advancement, thereby preserving

Is Alveolar Ridge Preservation Necessary in Molar Sites? A Systematic Review.

PURPOSE: Alveolar ridge preservation (ARP) aims to minimize alveolar bone resorption after extraction. The benefit of ARP in reducing the need for additional bone augmentation procedures to facilitate implant placement has been demonstrated, particularly in single-rooted tooth sites. However, its efficacy in molar sites remains unclear. This systematic review aimed to investigate if ARP in molar sites, compared to spontaneous healing (SH), adequately preserves ridge dimensions to facilitate successful implant placement while reducing the need for additional bone augmentation procedures such as

[Prolong bleeding after extraction from green pit venom].

A case of prolonged socket bleeding after extraction is reported to have been caused by a recent green pit viper snake bite. The snake is a common tropical creature. Several cases bitten are seen in general medical clinic yearly. Pain and local symptoms lead most patient to doctor's attention; while coagulopathy, the other clinical features are usually not being aware of. The 66-year-old patient who had been bitten by the snake a day earlier, of having tooth extraction was referred to Rachaburi Hospital Dental clinic of having post-extraction delayed haemostasis. A routine heamostasis as of ge
