Mumps virus infection in adults: three cases of supraglottic edema.

Mumps virus infections primarily involve the parotid glands and most frequently affect school-aged children. We present three unusual adult cases of mumps with dyspnea secondary to severe swelling of the salivary glands and review previously reported cases in the literature. Dyspnea developed progressively after the onset of salivary gland swelling. Laryngoscopy revealed an advanced edematous change in the supraglottis obstructing the airway. In two cases, tracheotomy was needed because of rapid worsening of the supraglottic edema. Questioning regarding breathing problems and laryngoscopic exa…

Endobronchial stenting for severe airway obstruction in relapsing polychondritis.

Airway complications of relapsing polychondritis (RP), including tracheobronchial stenosis, can be fatal. This paper describes a life-saving … sult in improved symptoms, pulmonary function, and a return to daily activities, without the use of tracheotomy and mechanical ventilation.

Airway management in patients with facial trauma.

Management of the airway is a major concern in patients with maxillofacial trauma (gunshot wounds, facial fractures, cervical spine injuries, laryngotracheal injuries) because a compromised airway can lead to death. The method of intubation to use in these patients remains a controversial topic. Although there are many options available, each one has specific indications, and the choice will ultimately depend on the patient's situation and the expertise of the trauma team. In general, endotracheal intubation is usually not a viable option, but nasotracheal intubation and tracheotomies can be p

[The change of difficulty in intubation as growth in patients with Treacher-Collins syndrome and Pierre-Robin syndrome].

BACKGROUND: Previous reports revealed that difficulties in tracheal intubation for patients with Treacher-Collins syndrome (TCS) progresses as growth; however it declines in those with Pierre-Robin syndrome (PRS). We tried to confirm these reports. METHODS: We retrospectively examined the anesthetic records of intubated patients with TCS and PRS without tracheotomy from January 2002 to August 2006. RESULTS: We experienced 10 times of intubation in 5 TCS patients and 6 times in 4 with PRS. No obvious change was observed in its difficulty depending on the growth in both syndromes. CONCLUSIONS…

Tracheal agenesis without esophageal fistula: genetic, resuscitative, and pathological issues.

An exceptional case of tracheal agenesis with no communication with the esophagus is described. This malformation needs surgical airway appr … a case. Usefulness of uncommon resuscitative maneuvers is discussed: a promptly performed surgical tracheotomy is the only mean to ventilate such a baby.

Treatment of bilateral choanal atresia in the premature infant.

An innovative technique for choanal atresia repair was used for a unique case of bilateral choanal atresia in a 28-week premature newborn. Endoscopic transnasal KTP laser was employed as an alternative to standard instrumentation to reach the very small atretic plate of a premature nose, providing good visualization of the operative field, thereby avoiding damage to the adjacent structures. The successful repair allowed for early extubation and avoidance of a tracheotomy. The clinical and surgical aspects of the case are discussed. With the likelihood of encountering more premature infants wit…

Occult supraglottic lymphatic malformation presenting as obstructive sleep apnea.

Sleep disordered breathing and obstructive sleep apnea is commonly encountered in the pediatric population. In many cases, it is the result of oropharyngeal obstruction secondary to adenoidal or adenotonsillar hypertrophy. We describe an unusual case of a child with adenoidal hypertrophy who had an occult supraglottic lymphatic malformation that manifested as obstructive sleep apnea. The management of this lesion is discussed including the use of endoscopy, carbon-dioxide laser, and the decision to avoid a tracheotomy. Occult supraglottic lymphatic malformations (LMs) are a rare cause of obstr…

Long-term complications of artificial airways.

In summary, long-term complications of artificial airways are rare but important sequelae of artificial airways. Many of the potential long-term complications of translaryngeal intubation and tracheotomy are similar and overlapping. Although most patients who undergo these procedures tend to tolerate them without difficulties, significant morbidity and mortality may occur. Identifying the exact cause of the complication may not be possible at times, due to the multiple risk factors involved in the pathogenesis. It is hoped that understanding these potential complications will lead to a more vi…

Laser endoscopic treatment of laryngoceles and laryngeal cysts.

The laryngocele and saccular cyst are uncommon anomalies of the larynx. Two patients, one with an internal laryngocele and one with a saccular cyst, were successfully treated by endoscopic laser marsupialization of their lesions. Since neither patient was found to be a suitable candidate for prolonged general anesthesia, the patients did not require a tracheotomy and were discharged the day after surgery. Both patients are doing well, with remarkable improvement in their voices persisting in their three year follow-up. The signs, symptoms, diagnosis, treatment, and indications for endoscopic l…

Complete mechanical expiratory obstruction as a result of expiratory valve malposition in an anesthetized dog.

A female, mixed-breed dog, aged 8 years, presenting with left temporomandibular joint ankylosis caused by a soft tissue sarcoma was anesthetized for tracheotomy and caudal segmental mandibulectomy. After intramuscular sedation with methadone and dexmedetomidine, oxygen was delivered by face mask, and anesthesia induced and maintained with propofol. A tracheotomy was performed, and a cuffed endotracheal tube was inserted. A circle breathing system that had been leak-tested was connected to the endotracheal tube, and oxygen (2 L minute-1) was administered. End-expiratory carbon dioxide partial p…

Continuous noninvasive ventilatory support outcomes for patients with neuromuscular disease: a multicenter data collaboration.

BACKGROUND: Typically, patients with progressive neuromuscular disorders (NMDs) develop acute respiratory failure (ARF), are intubated, and when failing spontaneous breathing trials (SBTs) undergo a tracheotomy and receive tracheostomy mechanical ventilation (TMV). However, increasing numbers of patients use nasal noninvasive ventilation (NIV), initially for sleep and this is extended to continuous dependence (CNVS). This can be used as a strategy to assist in successful extubation . We retrospectively reviewed 19 centers offering CNVS and mechanical insufflation-exsufflation (MI-E) as an alte…

Eight patients had an uncomplicated recovery with improved dyspnea, with two patients with tracheotomies decannulated. Synchronous airway lesions and outcomes in infants with severe laryngomalacia requiring supraglottoplasty.

OBJECTIVE: To quantify the prevalence and the impact of synchronous airway lesions identified by endoscopy in infants undergoing supraglottoplasty for severe laryngomalacia (LM). DESIGN: Retrospective study. SETTING: Tertiary care pediatric hospital. PATIENTS: Sixty patients who underwent supraglottoplasty for severe LM from 2002 to 2006. Patients who underwent preoperative tracheotomy, had previous airway surgery, or did not have 6 months of follow-up were excluded. Fifty-two patients met inclusion criteria. INTERVENTION: Supraglottoplasty (with carbon dioxide laser). MAIN OUTCOME MEASUR…

[Stenting central airways with tracheobronchial stents].

Tracheobronchial stenting is indicated in the palliative care of cancer patients with central airways obstruction due to primary chest and neck tumors, metastatic and congenital lesions of these organs. Stents, tubular prostheses, solid or wired, removable or not, of different shape, size, material, are used to treat airway obstructions due to endobronchial overgrowing, infiltration, compression, or relaxation of the airway walls. Silicone stents are well tolerated and removable. Their limitations are: the mucociliary clearance impairment, thick walls, displacement possibility. Rigid bronchosc
