The clinical correlation of temporal bone fractures and spiral computed tomographic scan: a prospective and consecutive study at a level I trauma center.

BACKGROUND: Undetected temporal bone fractures (TBFs) can lead to complications such as hearing loss, facial nerve paralysis, otorrhea, or otorhinorrhea, and can be the cause of life-threatening bacterial meningitis and can easily be missed. METHODS: We prospectively studied patients with head trauma between January 2000 and January 2001. All patients were examined by the attending trauma physician and then underwent cranial helical computed tomographic (CT) scan independent of clinical findings and Glasgow Coma Scale (GCS) score to determine the proportion of TBFs that would have been missed

History of high-velocity impact water trauma at Letterman Army Medical Center: a 54-year experience with the Golden Gate Bridge.

Three time frames were studied during the 54-year history of the Golden Gate Bridge from 1937 to 1991. During that period of time, there were 918 documented jumps from this majestic structure to the water 250 feet below. The last 15 years provided us with 297 consecutive patients, all brought to one institution, which were retrospectively reviewed and categorized as to site and type of injury for survivors and fatalities. This is the largest high-velocity water impact trauma series in the world. Certain unique characteristics of the Golden Gate Bridge and the San Francisco Bay lend itself to t

Epidemiological profile and factors associated with length of stay in patients with acetabular fractures at a level 1 trauma center in Nepal: a retrospective analytical study.

BACKGROUND: Acetabular fractures, predominantly caused by high-energy trauma, pose a substantial healthcare burden in low- and middle-income countries (LMICs) such as Nepal. This study aimed to characterize the epidemiological and clinical profile of patients with acetabular fractures at a tertiary Nepalese center and to identify independent determinants of prolonged hospitalization using multivariable analysis. METHODS: A retrospective analytical study was conducted using electronic medical records of 591 patients treated for acetabular fractures (ICD-10: S32.4–S32.5) between 2006 and 2023. T

Developing a Prediction Model for Suicidality Among COVID-19 Patients in Korea Using Timely Data From the National Center for Disaster and Trauma.

BACKGROUND: To date, no study has developed a predictive model based on the timely data (diagnosed within a month) of patients with coronavirus disease 2019 (COVID-19). This study aimed to develop screening tools and a model for predicting suicidality based on data from patients diagnosed with COVID-19 within the past month. METHODS: We analyzed data from 96,694 COVID-19 patients collected by the Korean National Center for Disaster and Trauma. Using classification and regression tree (CART) and random forest models, we predicted suicidality based on 39 features, including demographic informat

Trends, patterns, and outcomes of hip fractures over 14 years: insights from a tertiary trauma center in Beijing, China (2009-2022).

UNLABELLED: Over 14 years, hip fracture patients grew older, with shorter hospital stays. Femoral neck fractures were more common than intertrochanteric fractures, but the latter had worse functional outcomes and higher complications. Surgery was standard, with nailing for intertrochanteric and arthroplasty for femoral neck fractures. Older age increased adverse outcomes. AIM: We aimed to investigate trends in patient characteristics and outcomes in patients after hip fractures. METHODS: We analyzed data from hip fracture patients treated at our trauma center between January 2009 and Decembe

Pre-morbid Commonalities Among Patients Undergoing Emergent Cricothyrotomy at a Tertiary Care Level 1 Trauma Center: Categorizing Our Experience.

INTRODUCTION: Emergent cricothyrotomy is a lifesaving surgical procedure employed when conventional intubation fails. Scenarios requiring cricothyrotomy are uncommon, and many trainees have limited experience with the procedure. As "cannot intubate, cannot oxygenate" (CICO) scenarios demand rapid intervention, it is crucial that providers are able to determine which patients are at risk for failed intubation and, when necessary, quickly proceed to a surgical airway. This study investigates pre-morbid commonalities among patients who underwent cricothyrotomy at a tertiary care level 1 trauma ce

Outcomes of Video-assisted Thoracic Surgery-guided Early Evacuation of Traumatic Hemothorax: A Randomized Pilot Study at Level I Trauma Center.

INTRODUCTION: Traumatic hemothorax is accounted for about 20% of traumatic chest injuries. Although majority can be managed with the timely placement of intercostal tube (ICT) drainage, the remaining pose a challenge owing to high complication rates associated with retained hemothorax. Although various treatment modalities including intrapleural instillation of fibrinolytics, radioimage guided drainage, VATS guided evacuation and thoractomy do exist to address the retained hemothorax, but indications along with timing to employ a specific treatment option is still unclear and ambiguous. The changing epidemiology of traumatic spine injuries: a trends analysis of 26 years of patients at a major level 1 trauma center in the United States.

BACKGROUND CONTEXT: Traumatic spinal injuries (TSI) are associated with high morbidity, mortality, and resource utilization. The epidemiology of TSI varies greatly across different countries and regions and is impacted by national income levels, infrastructure, and cultural factors. Further, there may be changes over time. It is essential to investigate TSI to gain useful epidemiologic information. However, there have been no recent studies on trends for TSI in the US, despite the changing population demographics, healthcare policy, and technology. As a result, reexamination is warranted to re
