Effects of IV fluid restriction according to site-specific intensity of standard fluid treatment-protocol.

BACKGROUND: Variation in usual practice in fluid trials assessing lower versus higher volumes may affect overall comparisons. To address this, we will evaluate the effects of heterogeneity in treatment intensity in the Conservative versus Liberal Approach to Fluid Therapy of Septic Shock in Intensive Care trial. This will reflect the effects of differences in site-specific intensities of standard fluid treatment due to local practice preferences while considering participant characteristics. METHODS: We will assess the effects of heterogeneity in treatment intensity across one primary (all-ca

Repetitive Transcranial Magnetic Stimulation for Generalized Anxiety Disorder: A Systematic Literature Review and Meta-Analysis.

BACKGROUND: Anxiety disorders such as generalized anxiety disorder (GAD) impact 10% of the US population, and many patients do not completely respond to first-line treatments (e.g., selective serotonin reuptake inhibitors, serotonin-norepinephrine reuptake inhibitors, and psychotherapy). Given the dearth of evidence for non-pharmacologic, non-psychotherapeutic interventions, we performed a systematic review and meta-analysis of repetitive transcranial magnetic stimulation (rTMS) in adults with GAD. METHODS: A systematic literature review using the Preferred Reporting Items for Systematic revi

Clinically encountered growth phenotypes of tuberculosis-causing bacilli and their in vitro study: A review.

The clinical manifestations of tuberculosis (TB) vary widely in severity, site of infection, and outcomes of treatment-leading to simultaneous efforts to individualize therapy safely and to search for shorter regimens that can be successfully used across the clinical spectrum. In these endeavors, clinicians and researchers alike employ mycobacterial culture in rich media. However, even within the same patient, individual bacilli among the population can exhibit substantial variability in their culturability. Bacilli in vitro also demonstrate substantial heterogeneity in replication rate and cu

Systematic review of antistaphylococcal antibiotic therapy in cystic fibrosis.

BACKGROUND: The respiratory tract in patients with cystic fibrosis is frequently colonised with Staphylococcus aureus. There is great diversity of clinical practice in this area of cystic fibrosis. A systematic review was conducted to study the evidence relating antistaphylococcal therapy to clinical outcome in patients with cystic fibrosis. METHODS: A search strategy already evaluated for the study of the epidemiology of cystic fibrosis clinical trials was used. This yielded 3188 references from which 13 clinical trials of antistaphylococcal therapy were identified. RESULTS: Substantial het

Causal relationship between serum metabolites and chronic myeloid leukemia: A bidirectional Mendelian randomization study.

Previous studies have found a link between serum metabolite levels and chronic myeloid leukemia (CML), yet their exact causal relationship remains unexplored. Using genome-wide association datasets, we conducted bidirectional Mendelian randomization (MR) analyses to explore the potential causal relationship between 486 serum metabolites and CML. We conducted sensitivity analysis to assess the presence of heterogeneity and pleiotropy. Our Mendelian randomization analysis identified 20 metabolites exerting significant causal effects on CML, including 19 known and 1 unidentified metabolite. Among

Mindfulness-based interventions for cancer patients in standard treatment: A meta-analysis of effects on depression, anxiety, and quality of life.

Mindfulness interventions are indicated to be effective in reducing psychological symptoms faced by patients with cancer. However, there is a lack of revised evidence on their effectiveness during the active treatment phase. To address this research gap, we conducted a meta-analysis of controlled clinical trials with adult patients written in English, published in PubMed, Web of Science, PsycINFO and Scopus databases up to July 25, 2023. Studies with participants who completed oncological treatment more than 6 months before inclusion in the study were excluded. We calculated moderate effects o

Effectiveness of topical corticosteroids in addition to antiviral therapy in the management of recurrent herpes labialis: a systematic review and meta-analysis.

BACKGROUND: Recurrent herpes labialis (RHL) is one of the most common viral infections worldwide. The available treatments have limited efficacy in preventing the recurrence of ulcerative lesions and reducing the duration of illness. The objective of this review was to identify the effectiveness of topical corticosteroids in addition to antiviral therapy in the treatment of RHL infection. METHODS: A systematic review of randomized clinical trials comparing the efficacy of combined therapy (topical corticosteroids with antiviral) with placebo or antiviral alone in the management of RHL was con

Clinical Trial Designs for Rare Disorders: A Scoping Review of the Effectiveness of Pharmacologic Interventions in Fragile X Syndrome.

PURPOSE OF REVIEW: Rare disorders (RDs) collectively affect a substantial proportion of the population, yet most lack clinically approved, mechanistically targeted therapies. Fragile X syndrome (FXS), the most common inherited cause of intellectual disability and a major genetic contributor to autism spectrum disorder, exemplifies this gap. Despite more than 3 decades of preclinical research and numerous pharmacologic clinical trials informed by disease mechanisms, no targeted therapy for FXS has received regulatory approval. This scoping review aims to evaluate the design characteristics, out

Short communication: Effect of adding a second prostaglandin F2α injection during the Ovsynch protocol on luteal regression and fertility in lactating dairy cows: A meta-analysis.

Incomplete luteal regression after treatment with a single dose of prostaglandin F2α during an Ovsynch protocol decreases fertility to timed artificial insemination (TAI). To increase the proportion of cows with complete luteal regression and subsequently pregnancy per artificial insemination (P/AI), an additional treatment with PGF2α 24 h after the first has been recommended. A systematic review of the literature and meta-analytical assessment were performed with the objective of evaluating the effects of adding a second PGF2α treatment during the Ovsynch protocol on luteal regression and rep

Preferences of women experiencing infertility for in vitro fertilisation treatment in China: a discrete choice experiment.

INTRODUCTION: In vitro fertilisation (IVF) is a key treatment for infertility, but uptake and continuation may be limited by treatment burden, side effects and out-of-pocket costs. Evidence on IVF treatment preferences among women in China remains limited. This study examined the preferences of Chinese women experiencing infertility for different IVF treatment attributes. METHODS: A discrete choice experiment was conducted using face-to-face surveys to elicit preferences among women experiencing infertility receiving IVF treatment. Five attributes were examined: number of visits per cycle, si

A generalized multilayer reaction-diffusion model for tumor dynamics under hyperthermic therapy with spatial heterogeneity.

Local hyperthermia is an effective treatment for advanced malignant tumors, whose multilayer structure and physiological heterogeneity strongly influence their response to therapy. In this work, a generalized multilayer reaction-diffusion model is developed to study tumor-volume dynamics under localized hyperthermic treatment. The tumor is represented as a set of interacting biological layers, allowing spatial variation in key parameters such as diffusion and proliferation. The model is formulated with appropriate initial, interface and boundary conditions and solved using the finite element m

Associations of tumor necrosis factor-α-308 polymorphism with dengue infection: A systematic review and meta-analysis.

Inconsistency of reported associations between the tumor necrosis factor-alpha-308 (TNFα-308) polymorphism (rs1800629) and dengue virus infection prompted a meta-analysis, to obtain more precise estimates. A literature search yielded 14 case-control studies. We calculated pooled odds ratios (OR) and 95% confidence intervals in three groups according to severity, dengue fever (DF), dengue hemorrhagic fever (DHF) and dengue (DEN) using standard genetic models. Pooled ORs were subjected to modifier treatment where re-analysis was confined to Hardy-Weinberg compliant (HWC) studies. Heterogeneity o

Using Directly Observed Therapy (DOT) for latent tuberculosis treatment - A hit or a miss? A propensity score analysis of treatment completion among 274 homeless adults in Fulton County, GA.

Latent tuberculosis infection (LTBI) treatment in persons at increased risk of disease progression is a key strategy with the strong potential to increase rate of tuberculosis (TB) decline in the United States. However, LTBI treatment in homeless persons, a population at high-risk of active TB disease, is usually associated with poor adherence. We describe the impact of using directly observed treatment (DOT) versus self-administered treatments (SAT) as an adherence-improving intervention to administer four months of daily rifampin regimen for LTBI treatment among homeless adults in Atlanta.
