Screening and treatment for tuberculosis in a cohort of unaccompanied minor refugees in Berlin, Germany.

INTRODUCTION: In 2015, 4062 unaccompanied minor refugees were registered in Berlin, Germany. According to national policies, basic clinical examination and tuberculosis (TB) screening is a prerequisite to admission to permanent accommodation and schooling for every refugee. This article evaluates the use of an interferon-γ-release-assay (IGRA) during the initial examination and TB screening of 970 unaccompanied minor refugees. RESULTS: IGRA test were obtained during TB screening for 301 (31.0%) of 970 adolescents not previously screened for TB. Positive IGRA results were obtained in 13.9% (42

Hepatic abscess as a paradoxical response to antituberculous chemotherapy for tubercular lymphadenitis.

Tubercular liver abscess developing as a paradoxical reaction (PR) to antitubercular therapy (ATT) is relatively uncommon in the absence of disseminated disease and immunocompromised status of the host even in countries such as India where the disease is rampant. We report a rare case of hepatic abscess that developed paradoxically during antituberculous therapy for tubercular lymphadenitis in a 30-year-old woman who tested negative for HIV. Diagnosis was performed with the help of ultrasonography (USG)-guided aspiration of the abscess followed by cytopathological examination and PCR confirmat

Viral hepatitis and other infectious diseases in a homeless population.

GOALS: To determine the prevalence of four common infectious diseases-hepatitis B, hepatitis C, human immunodeficiency virus (HIV), and tuberculosis-as well as co-infection rates and risk factors in a homeless population. BACKGROUND: The prevalence of infectious diseases, especially viral hepatitis, among the homeless population is largely unknown. STUDY: This study consists of a retrospective analysis of the history and laboratory data collected from all homeless veterans admitted to a Veterans Administration (VA) domiciliary from May 1995 to March 2000. RESULTS: Of the homeless veterans a

Cost-utility analysis of stratified isoniazid dosing by NAT2 genotype compared with isoniazid standard regimen for new patients with pulmonary tuberculosis in Thailand.

BACKGROUND: Isoniazid is metabolized by the N-acetyltransferase 2 (NAT2) enzyme. Individuals classified as slow acetylators are increased risk of developing hepatotoxicity. This study aims to evaluate the clinical and economic impact of incorporating NAT2 genotype-guided dosing of isoniazid to prevent anti-tuberculosis drug-induced hepatitis in the Thai healthcare system. METHODS: A decision tree and Markov model were developed to assess the costs, clinical outcomes, and quality-adjusted life years (QALYs) of NAT2 genotype-guided isoniazid dosing compared to the standard regimen in newly diag

Decreased activities of daily living is a strong risk factor for liver injury by anti-tuberculosis drugs.

BACKGROUND AND OBJECTIVE: We evaluated the association between activities of daily living and drug-induced liver injury by anti-tuberculosis drugs. METHODS: This retrospective cohort study included adult inpatients with newly diagnosed smear-positive lung tuberculosis treated with standard regimen in two hospitals. (n = 346; 63.6 ± 20.3 years old; 106 (30.6%) females). Activities of daily living was divided into 'independent' (Barthel Index (BI) 80-100, 60.4%) and 'decreased' (BI 0-75, 39.6%) categories. Liver injury was defined as the withdrawal or change of treatment regimen on the basis of

Polymyositis associated with hepatitis B: management with entacavir and prednisolone.

We report a 57-year-old woman with a 20-year history of hepatitis B presenting with progressive proximal lower limb weakness for the previous 1 month. Previous medical history included a pericardial and pleural effusion, of which no cause was found and pulmonary tuberculosis which has been adequately treated. Examination revealed multiple telangiactasia over face and nail beds and bilateral proximal lower limb weakness of power 4/5. Biochemical investigation revealed a raised erythrocyte sedimentation rate of 36 mm/h, elevated creatinine kinase levels (14,363 IU/L) and raised liver enzymes (al

Donor selection, retrieval and preparation of donor tissue. Donor selection.

Corneal transplantation safety is widely dependent on clinical donor selection. Donor-to-host transmission of rabies and Creutzfeldt-Jakob disease is well established, and it is lethal for the recipient. Taking into consideration this latter figure, contraindications to ocular tissue transplantation include not only rabies, contact with rabies virus, spongiform encephalitis, family history of spongiform encephalitis, recipients of human pituitary-derived hormones before 1987, surgery using dura mater and brain/spinal surgery before 1992, but also CNS diseases of unknown etiology or those with

[Measures against hospital infection for students in medical schools in Japan--a survey by questionnaires].

We sent questionnaires to 80 medical schools in Japan to know what the problems in hospital infection for students in medical schools are. Seventy-one % were sent back to us. There have been hospital infection from patients to students in 12% of the medical schools, included 3 cases each of measles, chickenpox and mumps, 2 cases each of rubella, hepatitis B and tuberculosis. Fourteen % of the medical schools had reports about the past history of infection and vaccination from students, 70% of the schools determined their antibodies and the 28% did nothing. Ninety-three%, 25%, 23%, 18%, and 15%

Noncirrhotic intrahepatic portal hypertension.

Portal hypertension, widely recognized as a complication of cirrhosis, may also develop as an intrahepatic consequence of numerous hepatic disorders in the absence of cirrhosis. When gastrointestinal bleeding occurs in such cases, ruptured esophageal varices must be considered. Among chronic liver diseases, some, such as schistosomiasis, are commonly associated with portal hypertension and its complications. In others, including tuberculosis, amyloidosis, and polycystic disease, well-documented portal hypertension has been reported in only a small minority of cases. Nevertheless, because of th

Infliximab monotherapy for severe alcoholic hepatitis and predictors of survival: an open label trial.

BACKGROUND/AIMS: Severe alcoholic hepatitis (AH) is associated with very high mortality. Tumor necrosis factor-alpha (TNF-alpha) contributes to the progression of AH and TNF-alpha antagonists like infliximab may help in ameliorating the severity and complications of AH. There is a scarcity of data regarding the safety and efficacy of infliximab monotherapy in the treatment of AH. We evaluated infliximab monotherapy in patients with severe AH. METHODS: Patients with severe AH (Maddrey's score>32) received a single dose of infliximab 5 mg/kg IV. The primary endpoint was survival assessed at one

Autologous mesenchymal stromal cell infusion as adjunct treatment in patients with multidrug and extensively drug-resistant tuberculosis: an open-label phase 1 safety trial.

BACKGROUND: Novel treatment options are urgently needed for multidrug-resistant (MDR) and extensively drug-resistant (XDR) tuberculosis, which are associated with immune dysfunction and poor treatment outcomes. Mesenchymal stromal cells (MSCs) are immunomodulatory and adjunct autologous treatment with bone marrow-derived MSCs might improve clinical outcome by transforming chronic inflammation into productive immune responses. Our aim was to assess the safety of infusion of autologous MSCs as an adjunct treatment in patients with tuberculosis. METHODS: 30 patients with microbiologically confir

[Lymphoid form of intestinal tuberculosis with miliary dissemination: case report].

Tuberculosis with the incidence 28-29/100000 residents still presents a major public health problem in Croatia. Miliary tuberculosis is uncommon cause of fever of unknown origin. Intestinal tuberculosis pose as diagnostic problem that can be identified by colonoscopy and/or explorative laparatomy involving histopathology and microbiology. A case is reported of a 40-year-old HIV negative patient admitted to the Department of Infectious Diseases after two weeks of fever, diarrhea, abdominal pain and weight loss. Biochemistry testing showed mild elevation of the erythrocyte sedimentation rate and

Hepatocystosis in a baboon (Papio anubis).

A 26-lb wild-caught adult female baboon was obtained from a commercial vendor. Routine tests for parasites, pathogenic bacteria, and tuberculosis were negative throughout the 10-week quarantine. However, routine radiographs performed at the end of the quarantine period detected multiple small radioopaque nodules evenly dispersed throughout the liver. A hepatic biopsy was obtained during routine ovariectomy of the baboon, and a diagnosis of granulomatous hepatitis made. Approximately 1 year after her arrival, the baboon was euthanized for reasons unrelated to the hepatitis. The liver contained

Best practices for interpretation and management of common laboratory abnormalities in dermatology. Part II. Complete blood count and infectious diseases.

Part II of this article builds on the concepts discussed in Part I, focusing on the interpretation and management of additional laboratory tests commonly encountered in dermatologic practice. This section covers the complete blood count, tuberculosis testing, hepatitis serologies, and human immunodeficiency virus serologies. It highlights the importance of understanding these tests' results and their implications for patient care. The article provides guidance on when to refer patients to specialists based on abnormal findings and how to assess whether a laboratory result indicates a medical e

Ending diseases together: the case for multi-communicable disease elimination in the Eastern Mediterranean Region.

For decades, communicable disease control in the Eastern Mediterranean Region (EMR) has been largely organised disease by disease. Malaria, tuberculosis, viral hepatitis, and HIV programmes have each developed their own surveillance systems, supply chains, partnerships, and reporting lines at the national level and to WHO. This structure has delivered real, hard-won gains, and diseasespecific programmes remain the backbone of the region's technical capacity. But it has also produced parallel systems that perform, in many settings, functions that could be shared.

[Keratosis lichenoides chronica following trauma. A case report and update of the last literature review].

Since the original description by M. Kaposi in 1895, 40 cases of keratosis lichenoides chronica have been published. Most have been reports of single cases. The clinical appearance is characterized by lichen planus-like papules, linear keratoses, and psoriasis-like plaques. While the etiology is still unknown, an association with a variety of diseases including hepatitis, tuberculosis, glomerulonephritis, and lymphoproliferative diseases has been reported. We present a new case which may have been triggered by trauma or carbamazepine.

[Focal liver diseases--laparoscopic aspects].

A survey of the laparoscopic findings in such important focal diseases of the liver as metastasis, tumours, cysts and abscesses is given. Among the granulomatous changes, sarcoidosis, lymphogranulomatosis, tuberculosis and reticulosis deserve special attention. Definitive differentiation is, as a rule, only possible after carrying out a histological examination. In numerous infectious diseases, small granulomatous changes can be observed in conjunction with a so-called reactive hepatitis. Industrial noxae (e.g. beryllium, asbestos, silicates, and others) can also induce granulomatosis.
