Stress induced urinary incontinence in patients with spinocerebellar degeneration.

OBJECTIVES: To examine the pathophysiology of "stress induced urinary incontinence" (urinary incontinence evoked by abdominal straining) in patients with spinocerebellar degeneration. METHODS: Micturitional symptoms of 184 patients with spinocerebellar degeneration who were admitted to hospital were studied repeatedly. Urodynamic studies were made in symptomatic patients, and consisted of uroflowmetry, measurement of residual urine, urethral pressure profilometry, medium fill water cystometry, and external sphincter EMG. RESULTS: Twenty nine (15.8%) patients with spinocerebellar degeneration…

Complications of periurethral collagen injection for stress urinary incontinence.

PURPOSE: Periurethral collagen injection has been advocated as a safe and effective method of treatment for stress urinary incontinence. This study was conducted to determine the complications associated with transurethral collagen injection for treatment of stress urinary incontinence in women. We report the incidence and management of adverse effects. MATERIALS AND METHODS: A total of 337 women with at least a 1-year history, physical findings and urodynamic abnormalities consistent with stress urinary incontinence, who required pads or protective clothing, and who had no or only grade 1 cy…

Myofascial wrap to treat intractable urinary incontinence in children.

OBJECTIVES: The management of intractable urinary incontinence in the patient with cloacal or bladder exstrophy/epispadias, failed bladder neck plasty, or failed augmentation cystoplasty remains a surgical challenge. The myofascial wrap, a modification of the rectus fascial wrap, was developed to treat intractable urinary incontinence due to sphincteric incompetence in these problematic cases. A full-thickness, vascularized pedicle of anterior rectus sheath, rectus abdominis muscle, and posterior rector sheath is incorporated into a bladder neck wrap to provide support, mucosal coaptation, and…

Pelvic floor muscle exercises in genuine urinary stress incontinence.

Pelvic floor muscle exercises, in the treatment of genuine stress incontinence, have been used successfully since 1948. One may expect a significant improvement (warranting no further therapy), or cure rate of about 50%. These exercises have a long-lasting effect. Poorly motivated women should be discouraged to follow exercise sessions. An active co-operation between urogynecologist, physiotherapist and the patient is important in order to avoid undertreated and dissatisfied women. The option to be operated upon must be easily available.

[Transurethral collagen injection for treatment of urinary stress incontinence].

Minimally invasive treatment of urinary incontinence has become a subject of major interest in recent years. We examined the use of transurethral collagen injection for incontinence treatment. A total of 48 patients were selected for this procedure from April 1993 to February 1997-26 male patients (19 post-RPX incontinence and 7 post-TUR incontinence) and 22 female patients (all after previous incontinence surgery) were treated by injection of collagen into the continence region. The whole group underwent an average of 1.8 sessions, and a mean collagen injection volume of 14.5 ml was delivered…

Selecting the Best Surgical Option for Stress Urinary Incontinence.

Although there are many surgical procedures available to treat urinary incontinence, obtaining the best results depends on a thorough preoperative evaluation of the patient. Traditional urodynamic evaluation of stress urinary incontinence (SUI) focusing on maximum urethral closing pressures has been found to be less useful than the abdominal leak point pressure (ALPP) test in detecting intrinsic sphincter deficiency (ISD). An ALPP less than or equal to 60cm H2O indicates a significant degree of ISD, whereas a leak point pressure greater than or equal to 90cm H2O is usually associated with pure…

[Transrectal ultrasonography technique in stress urinary incontinence in women].

OBJECTIVES: This study describes the use of transrectal ultrasound as an alternative to cystography in the assessment of female stress urinary incontinence. The procedure and the different parameters that must be evaluated are also described herein. METHODS: The patients were evaluated in the standing position and saggital sections were obtained with a transrectal transducer. The bladder was filled with saline and the test was done at rest using the Valsalva maneuver with or without a bladder catheter. RESULTS: In these patients with female stress urinary incontinence, bladder neck movement …

Transdermal estrogen for female stress urinary incontinence in postmenopause.

OBJECTIVE: To evaluate the effect of transdermal estrogen for stress urinary incontinence in postmenopause. STUDY DESIGN: An open within patient, dose-finding study with transdermal 17-beta-estradiol combined with cyclic medroxyprogesterone acetate was conducted over 9 months in 21 patients (mean age 57.3 years) suffering from urodynamically verified mild to moderate stress incontinence without detrusor instability. RESULTS: Subjective improvement was noted in 16 out of 21 patients (76%). The dose level of 50 micrograms was better tolerated than 100 micrograms and sufficient enough to achiev…

Early experience with intraurethral collagen injections for urinary incontinence.

We report on 41 patients (10 men and 31 women) who underwent collagen injections for urethral incompetence. Mean followup was 6 months (range 3 to 11 months) in men cured or improved, 8.4 months (range 3 to 15 months) in women who were cured and 4.5 months (range 2 to 10 months) in women who were improved. In women the procedure was usually performed through a periurethral approach while they were under local anesthesia and in men it was performed transurethrally while under either general or local anesthesia. Of the 31 women 28 (90.3%) were cured (15) or improved (13). Mean maximum Valsalva p

[Late vesico-vaginal fistula after colporrhaphy for urinary incontinence].

We present a case of a 65 year-old woman with a late vesicovaginal fistula after colporrhaphy. Too late proper diagnosis of that complication caused perivesical tissue inflammation and led to damage to the urinary bladder, occlusion of the urethra as well as created a massive inflammatory tumor in the minor pelvis, which suggested a cancerous process. Bilateral hydronephrosis forced us to make bilateral nephrostomy. After intensive antiinflammatory therapy, anterior exenteration and Bricker's operation were performed with a finally good effect.

[Radio-urodynamic evaluation of stress urinary incontinence in women].

This work is aimed at demonstrating the validity of conventional radiological procedures, correlated with urodynamics, in the study of female urinary stress incontinence. In a study population of 110 patients with a clinical-urodynamic diagnosis of stress incontinence, radiological evaluation was performed by means of retrograde cystography, bead chain cystourethrography, and voiding cystourethrography. Radiographic findings were correlated with urodynamic data, and in particular with urethral pressure profile (functional length of the urethra, maximum closing pressure, maximum urethral pressu

Micturition symptoms and urinary incontinence after non-radical hysterectomy.

The effects of 554 non-radical vaginal and abdominal hysterectomies on micturition symptoms and urinary incontinence were studied. From the urological point of view, two groups of women may be distinguished prior to hysterectomy: one without urological symptoms and the second with urological complaints (present in no less than 57% of all women and differing significantly from the distribution of urological symptoms in the normal population). In the first group, frequency developed significantly more often than nocturia, dysuria, stress incontinence and nocturnal and diurnal urge incontinence. …

[Is Teflon treatment of female urinary incontinence still justified?].

An analysis is made in this paper of therapeutic results obtained at Charité Berlin from 14 cases of pure stress incontinence and 6 cases of combined stress-urge incontinence, with reference being made to general benefits and setbacks of transurethral Teflon injection to cope with female urinary incontinence. Five women underwent teflon therapy as the first operation performed on them. Seven women had had one operation before and the rest more than one. Continence was restored in 6 patients (30 per cent), and 3 patients (15 per cent) were improved. The presurgical situation was of minor influe…

Urinary incontinence and its correlates in very old age.

The study describes the prevalence of urinary incontinence and factors associated with incontinence among the population of Tampere aged 84/85 years (n = 320). 18% of the males were occasionally incontinent and 21% were daily incontinent. The corresponding figures for females were 20 and 21% respectively. Incontinence was not associated with sex, living alone, parity, or outdoor toilet facilities. On the other hand, incontinence was found to be related to use of home nursing, home help or day hospital services, living in institutions, limitations in mobility, and the use of diuretics.
