The changes of the transmucosal potential difference, the ionic flow, and the prostaglandin content were investigated after pylorus ligation, truncus vagotomy, and pylorus ligation with truncus vagotomy in Shay rats. A damage of the mucosa appeared already 10 hours after the pylorus ligation. This was presented in a clear decrease of the transmucosal potential difference and a change of the ionic flow. The paper is devoted to analysis of results of the investigation of the motor-evacuatory function of the stomach at different terms after truncal vagotomy with antrumectomy in patients with complicated duodenal ulcers. Postvagotomy gastrostasis was shown to be twice more frequent after antral resection with anastomosis by Billroth-I than by billroth-II. Truncal vagotomy with antrumectomy is not recommended for patients with duodenal ulcers complicated by decompensated stenosis. Gastric resection is thought to be the operation of choice in such cases.

Gastroduodenostomy (Billroth I) is our reconstruction of choice following gastric resection for gastroduodenal ulcer. The carotid body of the domestic fowl was examined with the electron microscope after either removal of the distal vagal ganglion or midcervical vagotomy. Almost all the axonal elements of the carotid body degenerated within 5-15 days after ganglionectomy. Repeated insulin tests were carried out on 118 patients after vagotomy and antral resection or vagotomy and drainage. The main indication for operation was gastro-duodenal ulcer. The test was performed in both the early (average 13.7 days after the operation) and late (average 3.3 years) postoperative phase. Bombesin and a synthetic bombesin nonapeptide were studied by intravenous infusion at a dose of 0.5 microgram.kg-1.h-1 for 4 h in 7 dogs with esophagostomy and gastric fistula. The influence of the proximal selective vagotomy (PSV) with following skeletisation of parts of the esophagus on its circulation was investigated in an animal experimental study. The Roux-en-Y gastrojejunostomy has become an increasingly used method for gastrointestinal tract reconstruction, and is advocated in the treatment of alkaline reflux gastritis. 22 patients with a negative acid stimulation test 6 to 18 months after total vagotomy and pyloroplasty were followed up with repeated secretory tests over 1 to 10 years. The sensitive, selective and harmless stimulation test with 2 desoxy-D-glucose showed a positive answer in one third of the patients after 1 to 4 years. After 5 to 10 years 10 of 12 patients showed an early positive answer. The absolute values of the PAO2DG remain low so that a late risk of recurrence is not to be expected. Possible explanations of the vagal reinnervation are discussed.

Viscerosensory information travels to the brain via vagal afferents, where it is first integrated within the brainstem nucleus tractus solitarii (nTS), a critical contributor to cardiorespiratory function and site of neuroplasticity.
