Apical Hypertrophic Cardiomyopathy: Diagnostic Challenges in a Patient with Hypertension and Obstructive Sleep Apnea.

BACKGROUND Apical hypertrophic cardiomyopathy (ApHCM) is an uncommon phenotype of hypertrophic cardiomyopathy (HCM) characterized by myocardial thickening predominantly affecting the left ventricular apex, typically without significant obstruction of the left ventricular outflow tract. This report describes a 53-year-old man with chronic hypertension and obstructive sleep apnea (OSA) in whom characteristic electrocardiographic (ECG) and cardiac magnetic resonance imaging (MRI) findings established ApHCM, highlighting practical implications for differential diagnosis and follow-up. CASE REPORT

Contemporary Natural History and Management of Nonobstructive Hypertrophic Cardiomyopathy.

BACKGROUND: Left ventricular outflow tract gradients are absent in an important proportion of patients with hypertrophic cardiomyopathy (HCM). However, the natural course of this important patient subgroup remains largely unresolved. OBJECTIVES: The authors systematically employed exercise (stress) echocardiography to define those patients without obstruction to left ventricular outflow at rest and/or under physiological exercise and to examine their natural history and clinical course to create a more robust understanding of this complex disease. METHODS: We prospectively studied 573 consec

[Comprehensive left ventricular outflow tract reconstruction in hypertrophic cardiomyopathy].

OBJECTIVE: To describe the concept of comprehensive left ventricular outflow tract reconstruction based on eliminating all obstructive mechanisms. MATERIAL AND METHODS: The authors analyzed own experience of left ventricular outflow tract reconstruction in patients with hypertrophic cardiomyopathy (over 400 surgeries). RESULTS: Complex left ventricular outflow tract reconstruction involves resection of interventricular septum and correction of papillary muscles, chordae tendineae and accessory muscle bundles. The described approach demonstrates high efficacy and safety and can be considered

Outcomes in Patients With Obstructive Hypertrophic Cardiomyopathy and Concomitant Aortic Stenosis Undergoing Surgical Myectomy and Aortic Valve Replacement.

Background Hypertrophic cardiomyopathy (HCM) and aortic stenosis can cause obstruction to the flow of blood out of the left ventricular outflow tract into the aorta, with obstructive HCM resulting in dynamic left ventricular outflow tract obstruction and moderate or severe aortic stenosis causing fixed obstruction caused by calcific degeneration. We sought to report the characteristics and longer-term outcomes of patients with severe obstructive HCM who also had concomitant moderate or severe aortic stenosis requiring surgical myectomy and aortic valve replacement. Methods and Results We studi

Advances in Transcatheter Electrosurgery for Treating Valvular Heart Disease.

Delivery of electrosurgery energy through catheters and guidewires enables interventionists to 'cut' through obstructive intravascular lesions or across cardiac chambers. A novel application of transcatheter electrosurgery is to make controlled lacerations in heart valve leaflets. This review describes three applications of transcatheter electrosurgery of aortic and mitral valve leaflets to enable transcatheter heart valve implantation. Intentional laceration of the anterior mitral leaflet to prevent left ventricular outflow obstruction splits and splays the anterior mitral valve and enables t…

Hemodynamic Assessment of Dual Obstructive Left Ventricular Assist Device Lesions.

Obstructive left ventricular assist device (LVAD) lesions are uncommon but are being increasingly recognized, particularly with the increased use of advanced imaging modalities. While heart failure symptoms and LVAD power fluctuations have a broad differential, obstructive lesions in the LVAD circuit should be considered. We present a unique case of a patient supported on HeartWare HVAD (Medtronic Inc., Dublin, Ireland) therapy, who experienced postural dizziness with objective orthostatic hypotension and occasional ventricular tachycardia. With fluctuations in LVAD flow and power, a CT scan w

Right-to-left shunting through the unidirectional valved patch after closure of ventricular septal defect.

Postoperative transesophageal echocardiography images of a patient undergoing unidirectional valved patch closure of ventricular septal defect in the setting of severe pulmonary hypertension are presented. The images and videos elegantly demonstrate a functioning valve without any obstruction to the left ventricular outflow.

Invasive exercise hemodynamics in obstructive and nonobstructive hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is a heterogeneous disease that may lead to exertional dyspnea through dynamic left ventricular outflow tract obstruction (LVOTO), left ventricular (LV) diastolic dysfunction, and/or left atrial myopathy. Deciphering the relative contribution of these alterations to exercise pathophysiology may be clinically relevant. We sought to characterize the hemodynamic adaptation to exercise of patients with HCM, using supine left and right heart catheterization. Twenty-five patients with HCM underwent rest and exercise cardiac catheterization. Patients were subdivided

Outcomes After Stenting of Right Ventricular Outflow Tract for Tetralogy of Fallot and Its Variants.

We reviewed our institutional database to examine the effect of right ventricular outflow tract (RVOT) stenting on branch pulmonary artery (PA) sizes and their outcomes. RVOT stenting was performed following standard technique. Since 2021, a conscious effort was made to spare the pulmonary valve (PV) whenever feasible. PA annulus and branch PA sizes were serially monitored by echocardiography. Sixty patients, (4.5 [1.75-24] months, weight 6.4 [3.5-9 kg]), were included (Time period:2012-2025). Commonest indications included cyanotic spell (65%) and hypoplastic branch PAs (30%). Pulmonary valve

Robotic trans-atrial and trans-mitral ventricular septal resection.

Localized ventricular septal hypertrophy, also known as idiopathic hypertrophic subaortic stenosis or idiopathic hypertrophic subaortic septal obstruction (IHSS), can create severe ventricular outflow obstruction. This often results in a high sub-aortic pressure gradient with potentially lethal symptoms. In 1960, Braunwald described the hemodynamic characteristics of IHSS, and thereafter, Morrow developed a trans-aortic approach to resect a large part of the ventricular septum, enlarging the outflow tract. The Morrow operation has remained the gold standard for surgically treating this patholo…

Hypertrophic obstructive cardiomyopathy. Effects of acute and chronic verapamil treatment on left ventricular systolic and diastolic function.

Changes in left ventricular systolic and diastolic function and outflow gradient were evaluated in patients with obstructive hypertrophic cardiomyopathy after intravenous acute treatment with verapamil (15 patients) and after six months of oral chronic treatment (11 patients). All patients had severe symptoms despite beta blockade, and the condition of all but two improved appreciably after chronic treatment with verapamil. Resting left ventricular outflow tract gradient decreased in six of 15 patients after intravenous verapamil, and in five of 11 patients after long term treatment, but there

Right Ventricular Outflow Conduit Stenosis After Childhood Ross-Konno Procedure.

BACKGROUND: Right ventricle-pulmonary artery (RV-PA) conduit stenosis is a relatively uncommon complication after a childhood Ross-Konno procedure. CASE SUMMARY: A man in his 20s presented with progressive dyspnea for 1 year. He had previously undergone modified Konno and Ross-Konno procedures for congenital subaortic stenosis followed by pulmonic balloon valvuloplasty for RV-PA stenosis. Echocardiography demonstrated reduced left ventricular ejection fraction, severe pulmonary hypertension, and right ventricular outflow tract obstruction. Cardiac computed tomography revealed right ventricula

Hypertrophic obstructive cardiomyopathy complicated with acute myocardial infarction and diffuse fibrosis: surgery or not?

BACKGROUND: Hypertrophic cardiomyopathy with extreme hypertrophy, biventricular obstruction and diffuse myocardial fibrosis complicated by myocardial infarction in the absence of obstructive coronary artery disease (MINOCA) is a rare phenotype. Evidence and guideline recommendations are still lacking for a treatment strategy. CASE PRESENTATION: Emergency coronary angiography was performed in a 38-year-old man with a 2-year history of nonobstructive hypertrophic cardiomyopathy (HCM) presenting with acute myocardial infarction. The coronary angiogram yielded no stenotic lesions but showed a dif

Aortoventriculoplasty with the pulmonary autograft: the "Ross-Konno" procedure.

BACKGROUND: For patients with complex left ventricular outflow tract obstruction, including hypoplastic aortic anulus with or without severe diffuse subaortic stenosis, various aortoventriculoplasty procedures (e.g., Konno procedure and its modifications; extended aortic allograft root replacement) are important management options. In younger patients, however, reoperation for valve replacement is inevitably required, and anticoagulation issues pose additional problems. The pulmonary autograft provides a promising option for aortic valve replacement as part of the aortoventriculoplasty procedu

Right ventricular bypass for palliation of cardiac sarcoma.

A 33-year-old woman with a nonresectable right ventricular sarcoma and pulmonary outflow tract obstruction underwent a right ventricular bypass operation for symptomatic relief. The patient had an uneventful recovery and was asymptomatic on discharge, without jugular plethora or hepatomegaly. We consider this procedure to be an excellent palliative treatment of malignant right ventricular obstructive symptoms for improving the patient's quality of life.

Multimodality Cardiac Imaging and Multidisciplinary Collaboration in Monitoring Intracardiac Sarcoma Response to Immunotherapy.

BACKGROUND: Intracardiac metastases causing right ventricular outflow tract obstruction are rare and carry a dismal prognosis. Immune checkpoint inhibitor (ICI) therapy risks immune-mediated pseudoprogression that could worsen hemodynamic obstruction. CASE SUMMARY: A 40-year-old man with metastatic alveolar soft part sarcoma presented with dyspnea, pericardial effusion, and intracardiac masses causing significant right ventricular outflow tract obstruction. Cardiac magnetic resonance tissue characterization established a noninvasive diagnosis without endomyocardial biopsy. ICI therapy was del
