Salvage medullary perforating artery revascularization using the muscular branch of the occipital artery following occipital artery-posterior inferior cerebellar artery bypass for trapping a ruptured vertebral artery dissecting aneurysm.

Vertebral artery dissecting aneurysms (VADAs) presenting with subarachnoid hemorrhage (SAH) are relatively rare. The re-rupture and mortality rates during the acute phase of SAH remain significantly high. Endovascular coil trapping is one of the definitive treatments to prevent re-rupture; however, it carries a potential risk of medullary infarction derived from occlusion of the medullary perforating arteries (Mperf). Here we present a case with poor-grade SAH resulting from a ruptured left VADA affecting the posterior inferior cerebellar artery (PICA). After obtaining informed consent, open s

Staged, Combined Management of Ruptured Vertebral Artery Dissecting Aneurysms Involving the Posterior Inferior Cerebellar Artery: Report of 4 Cases and Review of the Literature.

BACKGROUND: Ruptured vertebral artery dissecting aneurysms (VADAs) involving the posterior inferior cerebellar artery (PICA) are the most difficult to treat among variations of VADAs but require prompt treatment. The major challenge is to preserve the PICA while occluding the aneurysm. Despite advances in the management of ruptured VADAs involving the PICA, each treatment, whether it is combined or not, is associated with a significant degree of risk. CASE DESCRIPTION: This study presents 4 cases of ruptured VADAs involving the PICA that were successfully treated using a staged, combined meth

Transient Antiphospholipid Antibody Positivity in a Patient with Cerebral Infarction Due to Vertebral Artery Dissection: A Case Report and Literature Review.

We herein report a 60-year-old man with transient lupus anticoagulant positivity in the acute phase of cerebral infarction due to left vertebral artery dissection who experienced repeated occlusion of the affected artery despite mechanical thrombectomy and stent placement. The addition of anticoagulants prevented further cerebral infarctions. In this case, it was believed that vascular injury caused by vertebral artery dissection, combined with the presence of antiphospholipid antibodies, accelerated repeated thrombosis. Interestingly, anti-cardiolipin antibodies were detected eight months lat…

Late Diagnosis of a Patient With Gradual Onset of Lateral Medullary Syndrome Secondary to Spontaneous Vertebral Artery Dissection: A Case Report.

Lateral medullary syndrome (LMS), also known as Wallenberg syndrome, is a cerebrovascular event following ischemia of the lateral part of the medulla oblongata. Some of its etiologies include atherosclerotic changes, hypertension, thromboembolism, vertebral artery dissection (VAD), and aneurysm. We present a case of a 45-year-old male with LMS with a gradual onset of atypical symptoms of LMS, which has led to a late diagnosis of our patient. VAD is a commonly recognized cause of stroke in young people and it is a more frequent cause of LMS than posterior inferior cerebellar artery diseases. Th…

Resolution of traumatic vertebral artery dissection and occlusion after repositioning and posterior C1-ring osteosynthesis of a displaced Jefferson burst fracture.

A 70-year-old male sustained a Jefferson burst fracture with unilateral vertebral artery dissection and occlusion by displaced fragments. We performed reduction and posterior C1-ring osteosynthesis. We present a description of the intraoperative manual assessment of atlantoaxial stability. The vertebral artery was found with a good anterograde flow posteroperatively, and MRA showed reperfusion of the vessel. The patient was free of pain with preserved C1-C2 rotation after 6 weeks. Function-preserving posterior C1-ring osteosynthesis after reduction in a displaced Jefferson burst fracture compl…

Comparison of clinical characteristics and MR angiography appearance in patients with spontaneous intradural vertebral artery dissection with or without subarachnoid hemorrhage.

OBJECT: It is well known that spontaneous intradural vertebral artery dissection (siVAD) is an important cause of nontraumatic subarachnoid hemorrhage (SAH). The factors that influence whether SAH develops, however, remain unclear. The aim of this study was to investigate whether clinical characteristics and imaging findings are different in patients with siVAD with SAH compared to those with siVAD without SAH. METHODS: The authors conducted a retrospective, single-institution study involving patients in whom siVAD developed with or without SAH, between July 2003 and November 2010. Univariate…

Are blood blister-like aneurysms a specific type of dissection? A comparative study of blood blister-like aneurysms and ruptured mizutani type 4 vertebral artery dissections.

OBJECTIVE: Blood blister-like aneurysms (BBAs) resemble arterial dissections. The purpose of this study was to investigate the relationship between these two disease entities and highlight commonalities and distinct features. METHODS: Among 871 consecutive patients with aneurysmal subarachnoid hemorrhage, 11 BBAs of internal carotid artery and seven vertebral artery dissections (VADs) with a short segmental eccentric dilatation (Mizutani type 4), which is morphologically similar to a BBA, were selected. The following clinical factors were studied in each group : age, gender, risk factors, Hun…

Anatomic Feature and Preventive Strategy of Anterior Spinal Artery Occlusion in Endovascular Internal Trapping of Vertebral Artery Dissecting Aneurysms Distal to the Bifurcation of the Posterior Inferior Cerebellar Artery.

BACKGROUND AND OBJECTIVES: In endovascular internal trapping for intracranial vertebral artery dissecting aneurysms (VADAs) distal to the posterior inferior cerebellar artery bifurcation, anterior spinal artery (ASA) occlusion is a serious complication although it is relatively infrequent because of the presence of collateral blood vessels. In this article, we investigated the correlation between vascular anatomy and ischemic complications of the ASA. METHODS: We retrospectively evaluated 21 patients with ruptured PICA-distal type VADA treated by internal trapping at our affiliated institutio

How I do it? Posterior inferior cerebellar artery-intracranial vertebral artery reimplantation bypass and trapping of dissecting aneurysm involving the proximal posterior inferior cerebellar artery.

BACKGROUND: Proximal posterior inferior cerebellar artery (PICA) aneurysms are surgically challenging due to the high variability in the anatomy of the PICA origin, their deep-seated nature, and their entanglement with the lower cranial nerves. Direct or reconstructive clipping may not be achievable if the aneurysm is large, or dissecting, or exhibits atherosclerosis or calcification. METHOD: We present a case of a proximal PICA lateral medullary segment (P2) aneurysm that was successfully cured by trapping the aneurysm and reconstructing the PICA using the PICA-intracranial vertebral artery

Coil embolization strategy after flow diverter deployment in patients with intracranial vertebral artery dissection aneurysms: a study from a hemodynamic viewpoint.

Flow diverter (FD) deployment combined with coil therapy is effective and considered superior to FD deployment alone for treating large, complex anterior circulation aneurysms. However, the optimal strategy for coil usage in posterior circulation aneurysms, particularly intracranial vertebral artery dissection aneurysms (IVADAs), remains unclear. This study used patient-specific aneurysm models and finite element analysis to determine the ideal packing density (PD) of coils following FD placement in IVADAs. We prospectively analyzed 22 patients with 24 aneurysms, all treated with FD at our hos…

Treatment of unruptured intracranial vertebral artery dissection aneurysms with Flow Diverter compared with conventional stent-assisted coiling-a single-center study.

AIM: Flow diverters (FDs) are being increasingly used off-label for treatment of intracranial vertebral artery dissection aneurysms (IVADAs). However, the safety and efficacy of FDs for unruptured IVADAs remain unclear. This study was performed to investigate whether FDs-alone are safer and more effective than conventional stent-asisted coiling. METHODS: We retrospectively analyzed 152 patients who underwent endovascular stenting from December 2011 to December 2022. The baseline data, aneurysm characteristics, surgical details, perioperative complications, follow-up angiography, and clinical …

Massive Brainstem and Cerebellum Infarction Due to Traumatic Extracranial Vertebral Artery Dissection in a Motor Traffic Accident: An Autopsy Case Report.

Traumatic unilateral vertebral artery (VA) injury can cause focal thrombosis and may block the basilar artery which can further lead to ischemic stroke and massive cerebral infarction and subarachnoid hemorrhage and even rapid death. Here, we present an autopsy case with a traumatic extracranial VA dissection in a motor traffic accident, and the identification of cause of death underwent 2 autopsies. A 62-year-old male pedestrian collided with the right rearview mirror of a car and fell down to the ground. He complained pains in the head and neck. Head computed tomography (CT) showed a right l
