A new device to quantify ocular counterroll using retinal afterimages.

BACKGROUND: The integrity of vertical semicircular canal and otolith function remains difficult to assess in the clinical setting, partly due to difficulties in recording ocular counterroll. Here, we quantify static ocular counterroll from head tilt using a new head-mounted device. METHODS: The device consists of an LED positioned 42 cm in front of one eye and a striated lens which produces a streak of light on the retina. The LED is illuminated at full intensity (80 cd) to generate a retinal afterimage. Subsequently, in darkness, the subject's head is tilted in the roll plane. Finally, the L

[Activity of vestibular organs in women with postmenopausal osteoporosis].

UNLABELLED: Osteoporosis is a systemic disease characterized by low bone mass and alterations in the microstructure of the skeleton. It is a worldwide disease, most prevalent in women over the age of 70. It is estimated that approximately 15-18% of women over 50 years of age are affected by osteoporosis and further 37-50% of women have decreased bone mass. The percentage of men suffering from this disease is about 40-50% lower. Untreated osteoporosis causes pathologic bone fractures, in particular fractures of the femoral neck, deformations and pain. THE AIM OF THIS WORK: was to evaluate the

Analysis of vestibular testing in patients with vestibular schwannoma based on the nerve of origin, the localization, and the size of the tumor.

OBJECTIVES: We aimed to analyze the factors influencing caloric response and vestibular evoked myogenic potential (VEMP) in vestibular schwannoma. SUBJECTS: The subjects comprised 130 patients with unilateral vestibular schwannoma pathologically diagnosed by surgery. METHOD: Caloric response and the amplitude and latency of VEMP were measured and analyzed based on the nerve of origin, localization, and size of the tumor. The tumors were classified into 3 types based on localization: intracanalicular, intermediate, and medial; and into 4 grades based on size: 9 mm or less, 10 to 19 mm, 20 to

Vestibular toxicity due to inhaled tobramycin in a patient with renal insufficiency.

Inhaled tobramycin is being used increasingly in patients with cystic fibrosis and other forms of bronchiectasis for treatment of bronchial colonization with Pseudomonas aeruginosa. The goal of inhaled antibiotics is to provide maximal concentrations at the site of infection without risking systemic toxicity. We report an unusual case of reversible vestibular toxicity due to inhaled tobramycin in a patient with renal failure who was undergoing hemodialysis. Although systemic absorption after inhaled tobramycin is reportedly negligible, no recommendations have been published regarding monitorin

[Vestibular function after a reconstructive surgery improving hearing in patients after radical operation on the middle ear].

The study included 62 patients who had undergone radical surgery (RS) on the middle ear. The patients were later operated to improve hearing according to the original reconstructive technique. Before the reconstructive surgery and 1-2, 3-6, 12 and 36 months after it combined vestibulometric examination, otomicroscopy, audiometry assessed the treatment efficiency. Perception of live speech was also investigated. Vestibular dysfunction occurred in 87.1% of the examinees with the history of RS. One of the main causes of the dysfunction was micropathological foci in the trepanation cavity located

Predictable and pseudo random saccades in patients with acoustic neuroma.

We studied predictive and pseudo random saccades in patients with cerebellopontine angle tumor. Target for the saccades was a laser beam which was reflected on the wall in front of the subject, and the stimulus was controlled by a computer. The fixed target was spaced at a distance of +/- 30 to each side of the subject. The random saccades jumped 20 degrees/40 degrees/60 degrees in an unpredictable manner. The saccade analysis program was based on pattern recognition. In the random saccade test none of the mean values of saccadic reaction time (SRT), saccadic accuracy (SA) and saccadic peak ve

[Vestibular neuritis: a frequently unrecognized cause of diplopia].

The vestibular function plays an essential role in the stabilization of the image on the retina. In addition, when the head is tilted, it contributes to maintain horizontally the plane of the gaze. Vestibular changes can result in oscillopsia and/or diplopia. The latter is related to occurrence of a skew deviation. The authors emphasize the frequent occurrence of diplopia following disorders of the vestibular nerve, specially after vestibular neuritis. In clinical practice, the causal relationship between vestibular neuritis and diplopia is often unrecognized.

[Nystagmus reactions after rotatory vestibular stimulation (modified Veits extended rotation method) in patients with normal equilibrium. Part II: Directional preponderance in nystagmus reactions].

Based on the absolute values of the different nystagmus reactions, measured after having used the rotational test on 70 healthy subjects, the directional preponderance (DP) was calculated in the present study according to the equation of Jongkees and Philippszoon. It could be shown that a DP of less than +/- 30% can be regarded as a normal standard only if it refers to the summarised nystagmic reaction of the per- and first postrotatory phase (Per+PI) of the test. Depending on the different parameters (frequency, amplitude or slow phase velocity of the nystagmus reactions), higher standard val

Identification of the location of vestibular lesions on the basis of vestibulo-ocular reflex measurements.

Quantitative analysis of electro-oculographic recordings of eye movement in response to precise visual and vestibular stimuli makes possible the differentiation of three categories of vestibular syndromes due to pathological changes in three different parts of the visual vestibulo-ocular reflex arc: (1) decreased vestibulo-ocular reflex gain (e.g., decrease in slow component velocity), but normal fast components and visual-vestibular interaction (labyrinth and eighth nerve); (2) normal slow component velocity but abnormal fast components to all stimuli (pontine or medullar reticular formation)

Sacculo-collic pathway dysfunction accompanying auditory neuropathy.

CONCLUSIONS: In a patient with bilateral auditory neuropathy (AN), the vestibular-evoked myogenic potential (VEMP) was probably absent because of a neuropathy involving the inferior vestibular nerve and/or its end organ, the saccule. Our result can therefore be interpreted as a concomitant unilateral sacculo-collic neuropathy. We suggest the use of more precise terms to characterize AN patients with involvement of different parts of the inner ear and its innervations. We encourage detailed vestibular assessment in patients with AN in order to assess the co-existence of any symptomatic or asymp

Vestibular function in benign intracranial hypertension.

Investigation of vestibular function in 34 patients with benign intracranial hypertension was performed and a follow-up examination 3-20 years after the primary examination, when the symptoms of intracranial hypertension had disappeared. A primarily affected vestibular function was found in 19 patients (56%), viz. hyporeactivity in 9, preponderance in 9, and hyperreactivity in 2. Except for 2 patients, all had normal vestibular reactions at follow-up. These patients exhibited no evidence of what is called 'congestive inner ear'. An account is given also of the role of vestibular testing in con

Clinical Application of the Vestibular Stimulation Effect on Balance Disorders with Dementia.

BACKGROUND: In a previous study on Alzheimer's disease (AD), we showed that vestibular dysfunction derived from cerebral disorders contributes to balance disorders. No previous clinical study has attempted to prevent the progression of balance disorders in dementia patients through vestibular stimulation using an air caloric device. OBJECTIVE: The purpose of this pilot study was to delay the progression of balance disorders by inducing vestibular compensation, specifically by utilizing the effect of vestibular stimulation to activate the cerebrum. METHODS: Fifteen individuals were randomized
