We present four cases of disseminated zygomycosis that illustrate the characteristic clinical syndromes and natural history of this infection. The zygomycetes affect individuals immunosuppressed by age, drug therapy, or underlying disease. A previous splenectomy or deferoxamine therapy may be an additional factor in the development of disseminated zygomycosis. Clinical syndromes reflect vascular invasion and organ infarction. Diagnosis requires consideration of risk factors followed by biopsy of the affected organ for culture and for demonstration of broad nonseptate hyphae in tissue sections.

Cardiac transplantation moved from the realm of an investigational procedure to a therapeutic modality in the early 1980s. While one-year survival of 80 percent or greater can be expected, close follow-up is required to monitor for rejection, infection, and other late complications. With the need for close surveillance, both short and long-term, improved patient access to the transplantation center based on geographic proximity allows enhanced care by the original transplant team.

Forty subjects participated in a study of four handwashing agents evaluated for their efficacy in removing non-transient bacteria: 70% isopropanol, 0.05% stabilized iodine, 4% chlorhexidine gluconate, and 1% para-chloro-meta-xylenol. Infectious mononucleosis (IM) and beta-hemolytic streptococcal pharyngitis may present similar clinical pictures and may occur concurrently; however, recent studies have differed sharply with older ones that cited high rates of concurrent disease. Our study of 100 consecutive new patients meeting strict criteria for a diagnosis of IM found only 4% with positive throat cultures for group A beta-hemolytic streptococci at the time of diagnosis. Use of antibiotics for presumed streptococcal pharyngitis in patients with IM is unjustified. Only patients with positive cultures should be treated.

BACKGROUND AND OBJECTIVES: Many cannabis laws include medical use with pain as an indication. However, it is not known the extent of cannabis use for pain, the types of pain treated, and the perceived effectiveness of cannabis for chronic pain in this region. The purpose of this study was to understand cannabis use for pain in West Virginia patients.

Neonatal abstinence syndrome (NAS) is a withdrawal syndrome observed in neonates exposed to drugs in utero, typically opioids, which is associated with symptoms affecting the central and autonomic nervous systems and the gastrointestinal system. BACKGROUND: Gastrointestinal complications after cardiac surgery are associated with high morbidity and mortality. We sought to determine the granular impact of individual gastrointestinal complications after cardiac surgery and assess contemporary outcomes.
MATERIALS AND METHODS: Patients undergoing cardiac surgery from 2010 to 2017 (6070 patients) were identified from an institutional Society of Thoracic Surgeons database. Current research demonstrates that pediatric symptom management care is often initiated in the late stages of disease once clinicians are no longer able to meaningfully impact symptom burden.
