Ovarian papillary cystadenocarcinomas in a green iguana (Iguana iguana).

The clinical, gross morphological and histological characteristics of bilateral ovarian papillary cystadenocarcinomas in an adult female green iguana (Iguana iguana) are documented in this report. Histologically, the tumour consisted of an invasive mass of small glandular structures, irregular arboriform papillae and cystic structures arising from the ovarian surface epithelium. Metastatic implants were extensively disseminated along the serosa of the coelomic viscera, mesentery and fat bodies. Tumour morphology and metastatic behaviour indicated a close resemblance to cystadenocarcinoma in ot…

Detection of Cryptosporidium oocysts in extra-intestinal tissues of sheep and pigs.

Extra-intestinal infections by Cryptosporidium parvum have been detected in pigs and sheep. Detection was carried out by imprints of the mucosa of different organs and viscera in 55 sheep and 57 pigs slaughtered at three abattoirs in Zaragoza (northeast Spain). Imprints were stained by using a modified Ziehl-Neelsen technique. In addition to intestinal infections, cryptosporidial oocysts were found in the gall-bladders of two pigs which were 2 months old, and in some organs of sheep aged 5 days or more, including the gall-bladder (5), mesenteric lymph nodes (2), trachea (7), lung (3) and the u…

Management of the difficult abdominal closure.

Hemorrhagic shock and multiple trunk injuries, especially severe pelvic fracture, may cause massive swelling of intra-abdominal viscera and the abdominal wall, thereby precluding safe, primary abdominal wall closure. Primary closure, under tension in such patients, causes a multitude of problems including respiratory compromise, reduced cardiac output, oliguria, enterocutaneous fistulae, impaired abdominal wall nutrient blood supply, necrotizing fasciitis, evisceration, and death of the patient. Multiple methods have been described to aid the surgeon in circumventing these problems. The author…

Prolonged survival in a child with renovascular hypertension from abdominal-pulmonary Takayasu's arteritis.

Takayasu's disease is an inflammatory condition of the large arteries which comprises circulation to the head, extremities, and, less often, the abdominal viscera or lungs. A 10-year-old girl with renovascular hypertension secondary to Takayasu's arteritis has been followed for over 5 years. Her course is unusual in that (1) her disease is not related to tuberculosis; and (2) despite a persistently active disease course, she has been allowed to experience a fairly normal life-style through continued, extensive medical and surgical management. Vascular involvement has been documented by digital…

The gray scale sonographic appearance of intra-abdominal mesenchymal sarcomas.

Twenty-eight patients with intra-abdominal mesenchymal sarcomas were examined with gray scale ultrasonography. Masses were present in the mesentery, abdominal hollow viscera, liver, and retroperitoneal areas. Four different ultrasonic patterns were observed in the extra hepatic abdominal masses: (a) hyperechoic masses intermixed with anechoic zones; (b) hyperechoic masses with central fluid-filled zones; (c) homogeneous hyperechoic masses; and (d) homogeneous hypoechoic masses. Three of 6 patients with hepatic masses were totally anechoic alongside other more typical metastatic patterns. The p…

Rupture of Hydatid Cyst in the Gallbladder Leading to Acute Cholangitis.

Hydatid disease is a health problem in endemic areas such as the Mediterranean region caused by Echinococcus granulosus which can develop anywhere in the human body, but it is most frequently located at the liver. Liver hydatid cyst may rupture into the biliary tract, thorax, peritoneum, viscera, digestive tract, or skin, but its rupture in the gallbladder remains rare. We report a rare case of rupture of liver hydatid cyst in the gallbladder leading to acute cholangitis. The diagnosis was suspected on radiological imaging, and the patient was taken to open surgery which confirmed the imaging …

Cerebral infarction caused by systemic sclerosis: a case report.

Systemic sclerosis, also known as scleroderma, is a rare multisystem autoimmune disease characterized by vascular lesions caused by collagen deposition in the skin and viscera and damage to the endothelium. Endothelial injury and microvascular occlusion result in Raynaud's phenomenon, finger ischemia, pulmonary hypertension, and scleroderma renal crisis. Scleroderma itself is a rare disease with an incidence ranging from 0.1 to 14 per 100,000 people in the general population. Cerebral involvement is not considered a common manifestation of systemic sclerosis, although studies have shown that t…

Salmonella Typhi - historical perspective of discovery and forgotten contribution of Polish anatomopathology.

Outbreaks of typhoid fever for centuries decimated armies, cities and large hosts of people. Discovery of an agent causing such a grave disease became one of the most important achievements of bacteriology - science, which had experienced rapid development in the last quarter of the 19th century and changed the course of our civilization. The article deals with the discovery of Tadeusz Browicz, Polish anatomopathologist, who in 1874 reported about rod-shaped "parasites" in viscera of typhoid fever victim. His achievement became shaded by the later discoveries of Eberth, Klebs and Gaffky, but as…

Lateral access to paravertebral tumors.

Traditional transabdominal and retroperitoneal approaches for paravertebral tumors can be associated with injury to the viscera and lumbar plexus. The authors provide a technical description of a known approach with a new application for the resection of paraspinal tumors using both open and minimally invasive transpsoas techniques and report on 2 illustrative cases. In both cases, gross-total resection of the tumor was achieved and the patients experienced resolution of their presenting neurological symptoms, although one of the patients required 2 extra days of hospitalization due to an asym…

Splenic Injury in Situs Inversus Totalis - A Surgical Challenge.

Situs inversus totalis is a rare congenital anomaly in which position of the heart and all abdominal viscera is reversed. Situs abnormalities usually go unnoticed but may be recognized by radiography or ultrasonography as an incidental finding or during evaluation for congenital heart diseases. We present such an extremely rare and to the best of our knowledge the third reported case of an injured spleen in the right hypochondrium, following seemingly trivial blunt trauma in a patient with situs inversus totalis who underwent splenectomy. The presence of associated congenital heart defects, vi…

Ischemia-reperfusion injury in an aortic dissection patient.

Aortic dissection is a life-threatening emergency. Well-established risk factors include systemic hypertension, hereditary connective tissue diseases (Marfan syndrome and Ehlers-Danlos syndrome), coarctation of the aorta, bicuspid aortic valve, aortitis, and arch hypoplasia. Ischemia of the viscera, the kidneys, the spinal cord, or the lower extremities due to malperfusion constitutes life-threatening complications that have to be considered in the treatment strategy. We report a rare case of symptomatic ischemia of the lower extremities due to aortic dissection. This case demonstrates that the…

Fistulation as a complication of intra-abdominal soft-tissue sarcomas; a case series.

Soft-tissue sarcomas are rare, accounting for only one percent of all cancers. They can occur in retroperitoneal and intraperitoneal sites, including gastrointestinal stromal tumours (GIST), and have the potential to cause complications secondary to interaction with other abdominal viscera. Fistulation, or an abnormal communication between two epithelium-lined surfaces that do not usually connect, is a rare example of such a complication. We present a series of cases of fistulation due to the presence of an intra-abdominal soft-tissue sarcoma and contrast three different approaches to manageme…

Sparganosis in a patient with diffuse large B cell lymphoma.

Sparganosis is the human infection by plerocercoid, the larvae of sparganum. Clinically, subjective symptoms do not occur in the incipient stage, but as the worm migrates, pruritus or tenderness may occur. On physical examination, soft, palpable, and sometimes migratory, subcutaneous nodules are found in sparganosis patients. As rare cases; sparganosis from the orbit, the abdominal viscera, and the breast have been reported. However, there have been no reports relating such disease to the patients' immunocompromised status. We experienced a case of sparganosis from a patient with lymphoma whose…

Laparoscopic dual mesh repair of a diaphragmatic hernia of Bochdalek in a symptomatic elderly patient.

Bochdalek hernia is a congenital posterolateral diaphragmatic defect. It is usually diagnosed in newborns and children; the mortality rate is high due to further congenital anomalies such as pulmonary hypoplasia and pulmonary hypertension. In adulthood, the diagnosis is often incidental, while sometimes it is related to gastrointestinal or respiratory symptoms, or to herniated viscera complications. Thus, surgical treatment is advocated. We herein report a case of an 86-year-old woman with dyspnea and dysphagia. After the diagnosis by barium enema and CT scan of the herniated stomach and the g…

Splenic lymphangiomatosis associated with skeletal involvement (Gorham's disease): a new case and review of literature.

Lymphangiomatosis is a well-recognized congenital benign tumour, frequently seen in infancy and childhood, characterized by the presence of multiple lymphangiomas. Diffuse lymphangiomatosis also involving bony tissue is called Gorham's disease. This condition generally affects somatic soft tissue, where lymphatics are normally found. A predilection of this affection for bone, thoracic and neck involvement is well known, while involvement of abdominal viscera is very unusual. In bone this non malignant proliferation of lymphatic channels results in destruction and resorption of the osseous matr…

Traumatic liver injury complicating cardio-pulmonary resuscitation. The value of a major intensive care facility: a report of two cases.

Complications associated with external cardiac compression include trauma to the heart, chest wall, lungs and gastrointestinal viscera, with gastrointestinal visceral injury including, ruptured stomach, liver, oesophagus, spleen and colon. However, the use of thrombolytics and anticoagulants in patients with an acute myocardial infarction increases the incidence of a visceral haemorrhage when these patients need cardiopulmonary resuscitation. We report two out-of-hospital cardiac arrest patients whose immediate post-resuscitation phase was complicated by hepatic injury and significant haemoper…

Iliac vessel injuries.

Trauma to the iliac vasculature continues to pose a significant challenge to management. In several large series, mortality for penetrating injuries is reported as approaching 40%. Uncontrollable hemorrhage originating from an anatomically inaccessible source and multiple associated injuries often contribute to this high mortality rate. This article discusses the current existing management strategies and the controversial role of PTFE in vascular reconstruction within a contaminated field. Concomitant injuries to the enteric viscera and genitourinary system are also addressed. Postoperative m…

Sacral reflexes: physiology and clinical application.

Sacral reflexes consist of motor responses in the pelvic floor and sphincter muscles evoked by stimulation of sensory receptors in pelvic skin, anus, rectum, or pelvic viscera. These responses may be elicited by physical or electrical stimuli. They have been used in research studies of the pathophysiology of pelvic floor and anorectal disorders and many have been recommended for diagnostic use. These reflexes are described and discussed in this review. More rigorous evaluation of their value in the clinical assessment and care of patients with pelvic floor and sphincter disorders is required. …
