Reaching vulnerable women through maternity waiting homes in Malawi.

OBJECTIVE: To determine whether two maternity waiting homes (MWHs) supported by the Safe Motherhood Initiative are reaching vulnerable women during the early phase of their implementation. METHODS: A cross-sectional interview-based study was conducted among women who attended two centers in Malawi with attached MWHs (Area 25 Health Centre, Lilongwe; and Kasungu District Hospital, Kasungu). Between April and June 2015, exit interviews were conducted among MWH users and non-users. RESULTS: Compared with non-users, MWH users at Area 25 were significantly more likely to report a prior spontaneou

Climate change and planetary health collaborations in the context of an occupational therapy telehealth intervention for families of children and youth with special health care needs.

Families of children and youth with special health care needs (CYSHCN) have been insufficiently considered in research about the health effe … ssional dialogue and knowledge-transfer activities pertaining to climate-related concerns affecting vulnerable populations. Practice-engaged research methods (e.g., case-based dialogues, reflection, surveys, visual models and diagrams) and telehealth modalities were used to iteratively assess needs, share resources, and facilitate transdisciplinary co-learning. Bridging …

Development of a hydrogen peroxide-inactivated vaccine that protects against viscerotropic yellow fever in a non-human primate model.

Yellow fever virus (YFV) is endemic in >40 countries and causes viscerotropic disease with up to 20%-60% mortality. Successful live-attenuated yellow fever (YF) vaccines were developed in the mid-1930s, but their use is restricted or formally contraindicated in vulnerable populations including infants, the elderly, and people with compromised immune systems. In these studies, we describe the development of a next-generation hydrogen peroxide-inactivated YF vaccine and determine immune correlates of protection based on log neutralizing index (LNI) and neutralizing titer-50% (NT50) studies. In a…

The negative consequences of sports betting opportunities on human capital formation: Evidence from Spain.

The proliferation of on-site betting shops has received enormous public attention, becoming one of the most alarming health policy issues in contemporary cities. However, there is little evidence on whether its growing presence nearby vulnerable populations produce social harm beyond its known adverse individual effects. This study provides new evidence on the negative societal effects of betting houses. Our research design takes advantage of a new wave of openings in Madrid (Spain), which created a sudden increase in the supply of on-site gambling. Using a differences-in-differences design, w…

Seeking menstrual products: a qualitative exploration of the unmet menstrual needs of individuals experiencing homelessness in New York City.

BACKGROUND: There has been increasing recognition that certain vulnerable populations in the United States of America struggle to meet their menstruation-related needs, including people experiencing homelessness. Media and policy attention on this subject has focused on the provision of free menstrual products to vulnerable populations, including a New York City legislative bill, which guarantees access to menstrual products for Department of Homeless Services shelter residents (Intros 1123-A). METHODS: This qualitative study explored the challenges people experiencing homelessness in New Yor…

Formalising remote consulting in primary care services for underserved populations with long-term conditions in sub-Saharan Africa: a process evaluation and stakeholder engagement.

INTRODUCTION: Remote consulting can improve healthcare access and equity. When delivered by trained health workers, it is safe, trustworthy and efficient. Alongside two independent clinical trials, we studied the implementation of a cascaded health worker training intervention (REaCH), plus airtime, on remote consulting in primary healthcare facilities in Nigeria (n=35) and Tanzania (n=21). Additionally, we assessed health service readiness for sustainable remote consulting in Ghana, Kenya, Nigeria, Malawi, Sierra Leone, Tanzania and Uganda. METHODS: We explored what worked for whom, how, and

Communicative Justice in Later Life: A Critical Discourse Study and Bioethics-of-Vulnerability Analysis of Older Adults' Primary Care in Spain.

Older adults' experiences of primary care are shaped not only by service capacity but by the language and routines through which access and care are organised. Using a qualitative Critical Discourse Studies (CDS) approach informed by the bioethics of vulnerability, we analysed interviews and focus groups with 12 community-dwelling older adults living alone in southern Spain. Critical Discourse Analysis (CDA) linked micro-level talk to meso-level scripts and macro-rationales. Participants portrayed appointment systems and digital channels as gatekeeping devices that shifted agency to family 'br

Financial incentives and social messaging for repeat SARS-CoV-2 antibody testing among the underserved: A randomized trial.

Financial incentives may influence health behavior beyond their expected monetary value, and their effectiveness may depend on how the behavior is framed. Behavioral theories of decision making suggest that individuals may value protection against small-stakes losses more than expected utility predicts, while theories of family-centered health behavior suggest that messages emphasizing benefits to family members may strengthen participation in preventive health activities. We tested these ideas in a 2×2 factorial randomized trial involving 625 households recruited from a Federally Qualified He

Co-Producing Equitable Perinatal Mental Health Care: Facilitators and Barriers to Access Among Underserved Women in the PRAMS Study.

INTRODUCTION: Underserved women experience disproportionately high rates of perinatal mental health (PMH) difficulties but face substantial barriers to accessing appropriate care. Evidence on how services can be redesigned to improve equitable access remains limited. The Perinatal Redesign for Accessing Mental Health Services (PRAMS) study aimed to co-produce an intervention to improve access to PMH support for underserved women and birthing people. METHODS: PRAMS is a mixed-methods study informed by Accelerated Experience-Based Co-Design and the Medical Research Council framework for complex

Impact of Limited Access to Surgical Care in Medically Underserved Communities.

Summary/BackgroundMedically underserved communities and ethnic minorities constitute a significant portion of the vulnerable population within the United States. Recent changes in the health care structure, rising inflation with a decline in median household income, and the SARS-CoV-2 pandemic have disproportionately impacted communities of low socioeconomic status. Healthcare providers and federal organizations must be aware of how these factors influence access to surgical care to tailor treatment, interventions, and policies better to meet the needs of these populations.MethodWe systematica

Trajectories and predictors of psychological recovery post zero-COVID among Chinese adolescents in disadvantaged regions.

With the end of the COVID-19 pandemic and the relaxation of social restrictions, it seems intuitive to expect significant reductions in learning difficulties and psychological distress among adolescents. However, their psychological recovery could be hindered by the increased stress arising from rapidly changing environments, particularly for those from less developed regions. A three-wave study focusing on Chinese adolescents (N = 962) from economically disadvantaged regions was conducted to examine their psychological distress and academic inefficacy changes before and after the relaxation o

Mobile Intervention for Increasing COVID-19 Testing in K-12 Schools Serving Disadvantaged Communities: Randomized Controlled Trial of SCALE-UP Counts.

BACKGROUND: A key challenge for schools throughout the COVID-19 pandemic was finding ways to monitor and prevent COVID-19 cases. While diagnostic testing and connecting students and their families to appropriate resources to mitigate the spread of COVID-19 were recommended, few schools had scalable infrastructure, including information technology systems, to implement these types of measures. OBJECTIVE: This study tested a new approach to COVID-19 testing (SCALE-UP Counts) in school settings that used automated bidirectional text messages provided to the school community that alerted parents

We examined neighborhood-level demographic, economic, and social characteristics and food and health-services access to gauge the vulnerability of older-adult neighborhoods in New York State (NYS), which is understudied and is significant given the rapid aging of populations worldwide. We conducted descriptive ecological analyses using data from the American Community Survey, historical redlining maps, Social Capital Instruments, U.S. Department of Agriculture food access atlas, ESRI businesses, and Social Determinants of Health. We compared census tracts classified as having high and low leve
