Lymphatic filariasis (elephantiasis) is a global public health problem caused by the parasitic nematodes Wuchereria bancrofti and Brugia malayi. We have previously reported anthraquinones from daylily roots with potent activity against pathogenic trematode Schistosoma mansoni. Here we report the synthesis of novel anthraquinones A-S and their antifilrarial activity. Anthraquinones A-S were synthesized by a single-step Friedel-Crafts acylation reaction between phthalic anhydrides and substituted benzenes. The antifilarial properties of these synthetic anthraquinones were tested against microfil…

To assess clinical findings associated with Wuchereria bancrofti infection, 192 school children in a filariasis-endemic area of Haiti underwent physical and ultrasonographic examinations and testing for circulating filarial antigen (CFA). The CFA-positive children were more likely than CFA-negative children to have severe interdigital lesions (> or = 1 macerated lesion with involvement of > or = 4 toe web spaces) (P < 0.0001) and inguinal (P = 0.003) or crural (P = 0.004) lymph node pathology. In multivariate analysis, CFA positivity remained a significant predictor for severe interdigital les…

The efficacy of re-treatment with the combination of ivermectin (150-200 micrograms/kg bodyweight) and albendazole (400 mg) on Wuchereria bancrofti microfilaraemia was assessed in 1997-99 in 4 groups of individuals from coastal Ghana, which 1 year previously had received a primary treatment with placebo (n = 38), albendazole (n = 39), ivermectin (n = 34) or combination of albendazole and ivermectin (n = 42), respectively. One year after the re-treatment, an overall mean reduction in microfilarial intensity of 76.2% in relation to the intensity before re-treatment was observed, with no statisti…

Antibodies specific to recombinant filarial antigens Wb-SXP-1 and Bm-SXP-1 have been used to develop a sandwich ELISA for the detection of circulating filarial antigen (CFA) in sera from patients with lymphatic filariasis caused by Wuchereria bancrofti of Brugia malayi. In patients with W. bancrofti infections, a high proportion of microfilaria (mf) positive (MF) and low proportions of patients with chronic pathology (CP) and endemic normals (EN) showed the presence of CFA. Similarly in patients with brugian infections a high proportion of mf positive individuals contained CFA while none of th…

This study was designed to evaluate the effect of a single dose of diethylcarbamazine (DEC, 6 mg/kg) on Wuchereria bancrofti infections in a low-endemicity setting in Egypt (microfilaremia, or MF, 3.7%, median MF 34/mL). Subjects with MF or filarial antigenemia were treated and restudied 1 year later. Treatment with DEC dramatically reduced blood MF counts, with clearance in 69% of subjects. Treatment also reduced filarial antigen levels, but low clearance rates suggest that some adult worms survived treatment in most patients. Mass treatment was administered in one village; 27 months later, M…

Bancroftian filariasis can be detected by using the ICT Filariasis test kit which is composed of specific polyclonal and monoclonal antibodies to Wuchereria bancrofti antigen. Chromatographic reaction with serum or plasma shows a result within 5 minutes. When compared with 454 thick blood films (standard smear method) within the same study, the ICT Filariasis test had sensitivity = 100%, specificity = 96.37%, efficiency = 96.70%, predictive value positive (PVP) = 70.70%, predictive value negative (PVN) = 100%. Compared with 454 membrane filtration technic (MFT), the MFT had sensitivity = 95.10…

Since diethylcarbamazine, the drug recommended for treatment of lymphatic filariasis, seems only partially effective against the adult worm, intense interest persists in identifying a macrofilaricidal drug for this infection. To evaluate directly in vivo the macrofilaricidal activity of repeat high-dose ivermectin, 15 men who had living adult Wuchereria bancrofti detected in the scrotal area by ultrasound were treated with 400 micrograms/kg of ivermectin at 2-week intervals for 6 months (total dose, 4.8 mg/kg). Serial ultrasound examinations were performed before, during, and for 6 months afte…

In a nine-months epidemiological study to assess the status of human filariasis in north-western zone of Bauchi State, Nigeria, 3.5% prevalence was recorded in 8 communities. Of the 1536 persons examined, 54 were microfilaremic for three blood-dwelling filarial parasites. 22 (1.4%) were positive for Mansonella perstans, 10 (0.7%) and 22 (1.4%) had Loa loa and Wuchereria bancrofti respectively. W. bancrofti showed low microfilaremia in females of reproductive age with high degree of heterogeneity among communities. Most infected persons (81.5%) had counts less than 11 mf/20 mm3 of peripheral bl…

Antibodies directed towards the sheaths of microfilariae have been implicated in the elimination of circulating microfilariae, both in experimental and human filariasis. In the present study antisheath antibodies have been detected in human sera by indirect immunoperoxidase assay (IPA) using fixed Wuchereria bancrofti microfilariae as antigen. One hundred and eighteen sera collected from an area endemic for Bancroftian filariasis were tested. While 80% of sera from microfilariae carriers had no demonstrable antisheath antibodies, more than 80% of amicrofilaraemic samples (chronic filariasis ca…

Forty-seven Wuchereria bancrofti microfilaria carriers were treated once weekly for 12 weeks with 6 mg/kg DEC-C. Microfilaria densities ranged from 1 to 101 mf/20 cmm (MfD-50 of 9.75 mf/20 cmm) before treatment. Of 42 persons who completed treatment, 26 (62%) were microfilaria negative in 1-ml venous blood samples. Microfilaria densities in the 16 patients that remained positive ranged from 1 to 50 mf/ml (MfD-50 of 2.59 mf/ml). Weekly spaced doses of DEC-C resulted in a significant reduction in the number of microfilaria carriers posttreatment, and in the level of residual microfilaremia when …

The possible role of microfilarial surface (cuticular) antigens in immuno-diagnosis of human filarial infections has been assessed using microfilariae (mf) of the cattle parasite Onchocerca gibsoni. A Triton X-100 extract of 125I-labeled O. gibsoni mf was reacted with a panel of sera from humans infected with Onchocerca volvulus, Wuchereria bancrofti and Schistosoma japonicum as well as sera from uninfected controls. Results of these immunoprecipitations indicated that sera from humans infected with O. volvulus or W bancrofti contain antibody specificities recognising certain of the radioiodin…

Lymphatic filariasis (LF) is a major public health problem globally and in the Pacific Region. The Global Programme to Eliminate LF has made great progress but LF is persistent and resurgent in some Pacific countries and territories. Samoa remains endemic for LF despite elimination efforts through multiple two-drug mass drug administrations (MDA) since 1965, including renewed elimination efforts started in 1999 under the Pacific Programme for Elimination of LF (PacELF). Despite eight rounds of national and two rounds of subnational MDA under PacELF, Samoa failed transmission assessment surveys

A 35-year-old HIV positive male presented with dyspnoea and chest pain was diagnosed having acute pericardial and pleural effusion. Microfilaria was detected from blood as well as from the pericardial and pleural fluid and from urine. CD4 count was 123 cells microl. The patient was receiving treatment with antiretroviral therapy and Cotrimoxazole for last 4 months. The patient had no opportunistic infection and no symptoms suggestive of filarial infection in the past. This is for the first time we are reporting high microfilarial load (1000/ml) from blood in HIV positive patient, where microfi

Filariasis, a parasitic infection endemic in parts of India, Myanmar, islands of the South Pacific, West and East Africa and Saudi Arabia can be diagnosed from various types of cytopathological specimens. This case documents the detection of filarial infection from hydrocele fluid cytology in a 30-year-old Myanmar migrant worker in Malaysia.

Currently, only three drugs are used to control and treat the mosquito-borne parasitic disease lymphatic filariasis: diethylcarbamazine, ivermectin (Mectizan) and albendazole (Zentel). All interrupt transmission by eliminating microfilaria, the parasite stage that is responsible for transmission between hosts, but do not reliably kill the adult worms that are responsible for much of the pathology seen in the disease. There is an urgent need to develop drugs that will reliably kill adult worms and several compounds are under-going in vitro and animal testing. An alternative strategy - that of t
