Two cases of non-Hodgkin's lymphoma suffered from acute respiratory failure. Both patients were treated with MACOP-B therapy, and received recombinant granulocyte-colony stimulating factor (rG-CSF) during the myelosuppression. They had fever and severe hypoxemia several days after 11 and 12-week's treatment, respectively. The chest X-ray films revealed diffuse fine granular shadows in bilateral lung fields. Each year we treated approximately 40 patients with adult respiratory distress syndrome (ARDS) in our department of anaesthesiology. In these cases we found with bronchoalveolar lavage followed by microbiological examination, a high incidence (40 per cent) of pneumocystis carinii contamination. An infection with Pneumocystis carinii deteriorates the clinical course of ARDS. Effective drugs were the combination of trimethoprim and sulfamethoxazole. Under this therapy we always saw a decrease of infiltrations in the chest-x-ray as well as an improved gas exchange. No relapse was seen in any case

Epiphora is a recognized complication of radiotherapy of medial canthal tumours. We reviewed the records of 36 patients who underwent prophylactic nasolacrimal duct intubation with silicone tubing before superficial x-ray beam therapy and 42 patients who did not undergo intubation before radiotherapy. The mean length of follow-up was 3.4 years in the intubated group and 7.2 years in the nonintubated group. A total of 31% of the patients in the nonintubated group reported chronic epiphora during the follow-up period, compared with 6% in the intubated group (p less than 0.003).

We report the beneficial effect of steroids in a 27-year-old woman with lymphocytic interstitial pneumonitis (LIP) and common variable immunodeficiency (CVID). Exertional dyspnea, x-ray infiltrates, and pulmonary physiologic abnormalities decreased or increased during 3 1/2 years of follow-up in parallel with the administered dose of prednisone. MR is the only noninvasive procedure that can demonstrate changes in the medullary space. If it is combined with bone marrow scintigraphy and conventional x-ray film diagnostics, it can yield a comprehensive diagnosis of bone marrow infiltrations in the regions of the pelvis, thighs and lumbar vertebral column in leukaemia or malignant lymphomas. The T1 relaxation times of the malignant infiltrations are enhanced compared with normal fatty marrow. MR tomography of the bone marrow is particularly suitable for determining the therapy or follow-up control of a malignant systemic disease.

INTRODUCTION: Early diagnosis and treatment of hip dysplasia are widely accepted as major factors for beneficial outcome. However, modalities for reduction and retention as well as for imaging are currently under clinical investigation. Local and general risk factors, e.g., breech presentation and the family's desire to avoid in-hospital treatment are major concerns in the decision-making process and consultation. For treatment of unstable hips in newborns the treatment with the Tübingen splint has proven good results in recent studies. However, mid- and long-term outcome studies are missing.

This study examined the effect of once-weekly parathyroid hormone (PTH) combined with alendronate upon osteoporotic fracture healing after long-term alendronate anti-osteoporosis therapy. Seventy-six 12-week-old female Sprague-Dawley rats were either sham operated or bilaterally ovariectomized (OVX). BACKGROUND: Long-term antiretroviral therapy dramatically reduces HIV-related morbidity and mortality but is also associated with metabolic and morphological changes and requires high levels of adherence.

INTRODUCTION: Malignant lymphoma of the thyroid gland can be defined as a lymphoma arising from the thyroid gland. Lymphomas of the thyroid gland represent less than 5% of primary thyroid neoplasms and are two to three times more common in women than in men, whereas the median age is usually close to 60 years. CASE: A 52-year-old man known for a recent cardiac transplantation presented with 6 weeks of incapacitating left shoulder pain with normal x-rays and CT scans. MRI revealed bone edema of the coracoid and glenoid neck with a supraspinatus abscess. Biopsy of the coracoid demonstrated Aspergillus osteomyelitis of the scapula.
CONCLUSION: Aspergillus osteomyelitis is a rare manifestation of invasive aspergillosis and mostly manifests as spondylodiscitis. In this report, we describe a case of Aspergillus osteomyelitis of the scapula treated with combined antifungal therapy and surgery.

In the article an experience of successful surgical treatment of such a rare pathology in children, acute primary purulent paranephritis is described. In 11 children aged from 2 days to 14 years the standard laboratory, imaging and X-rays studies, including CT and MRI. Upper, posterior, anterior and lower paranephritis was detected in 2, 1, 2, and 6 cases, respectively. The authors concluded that in case of inefficiency of antibacterial therapy the surgical intervention is a main approach to the treatment of acute primary purulent pyelonephritis in children.

Radiotherapy plays an important role in the treatment of skin tumours. For skin carcinomas, external irradiation (kilovoltage X-rays or electrons according to clinical characteristics) is more valuable than interstitial brachytherapy, which is recommended for tumours of the lip and of the nasal vestibule. In mycosis fungoides, total cutaneous electron beam radiation therapy is efficient for patients with limited superficial plaques. In the classical form of Kaposi's sarcoma, radiotherapy can achieve local control whereas it obtains good palliative results in the epidemic form.
