Sharp cardiac trauma through the sternum caused by an automatic nail gun: A case report.

Automatic nail gun injuries to the hand commonly occur with the use of these machines in construction. However, such injuries to the cardiothoracic area are atypical. Herein, we report a case of emergency surgery to remove a nail, which was accidentally shot through the sternum and reached the heart. A 24-year-old man was working in a narrow space at a construction site, where he tripped over the air hose of an automatic pneumatic nail gun. The trigger was accidentally pulled, while the machine was facing his direction, and a nail entered his sternum. The patient felt chest pain, walked to a n

Revision Roux-en-y hepaticojejunostomy for a post-cholecystectomy complex vasculobiliary injury with complete proper hepatic artery occlusion: A case report and literature review.

INTRODUCTION: Complete proper hepatic arterial [PHA] occlusion due to accidental coil migration during embolization of cystic artery stump pseudoaneurysm resulting from a complex vasculobiliary injurie [CVBI] post laparoscopic cholecystectomy [LC] is an extremely rare complication with less than 15 cases reported. We present a case depicting our strategy to tackle this obstacle in management of CVBI and review the relevant literature. PRESENTATION OF CASE: A 35 year old lady presented on sixth postoperative day with an external biliary fistula following Roux-en-y hepaticojejunostomy [RYHJ] fo

Permanent hypopituitarism is rare after structural traumatic brain injury in early childhood.

BACKGROUND: We sought to determine the incidence of permanent hypopituitarism in a potentially high-risk group: young children after structural traumatic brain injury (TBI). METHODS: We conducted a cross-sectional study with longitudinal follow-up. Dynamic tests of pituitary function (GH and ACTH) were performed in all subjects and potential abnormalities critically evaluated. Puberty was clinically staged; baseline thyroid function, prolactin, IGF-I, serum sodium, and osmolality were compared with age-matched data. Diagnosis of GH deficiency was based on an integrated assessment of stimulate

Homicide with a trident harpoon: a case report.

Spear guns are rarely implicated in homicidal events and are infrequently described in forensic literature, with most published reports focusing on accidental or suicidal injuries and on the surgical management of surviving victims. We report a case of homicide caused by a trident-fishing harpoon, a device designed for underwater use. The fatal injury followed a sudden verbal dispute between two individuals over a parking space on a city street. Autopsy revealed three circular entrance wounds in the precordial region arranged in an oblique pattern, corresponding to the three-pronged configurat

Full-Thickness Corneal Perforation Secondary to Monopolar Cautery Burn During Cosmetic Blepharoplasty in a Young Adult: Successful Management With Cyanoacrylate Glue.

Thermal corneal injuries are uncommon but potentially vision-threatening. Full-thickness corneal perforation caused by monopolar electrocautery during cosmetic blepharoplasty is exceptionally rare and scarcely reported in the literature. We report a 27-year-old healthy man who sustained an accidental central full-thickness corneal perforation during elective upper eyelid blepharoplasty due to inadvertent contact with monopolar cautery. The patient presented with severe ocular pain, markedly reduced vision, and a shallow anterior chamber. Prompt intervention was performed under sterile conditio

Imaging of Intimate Partner Violence, From the AJR Special Series on Emergency Radiology.

Intimate partner violence (IPV) is a highly prevalent public health issue with multiple adverse health effects. Radiologists are well suited to assessing a patient's likelihood of IPV. Recognition of common IPV injury mechanisms and resulting target and defensive injury patterns on imaging and understanding of differences between patients who have experienced IPV and those who have not with respect to use of imaging will aid radiologists in accurate IPV diagnosis. Target injuries often involve the face and neck as a result of blunt trauma or strangulation; defensive injuries often involve an e

Bizarre paediatric facial burns.

Child abuse and neglect account for a significant number of paediatric burn injuries. It is of great importance because of the high mortality, high frequency of repeated abuse, as well as the physical, psychological and social sequelae that it causes. Burn abuse is often under-recognized and under-reported because it is difficult to define non-accidental injury. On the other hand, false accusation of burn abuse is extremely damaging to the family. Bizarre and unusual burn injuries can be caused by accident and should not automatically be assumed to be deliberate injury. Three boys of age 1-7 y

Case Report: Two cases of rare head injuries from Nepal.

Background: There are a number of ways in which one can sustain a head injury. Even if you are doing simple household activities or going out for a morning walk, you cannot be sure of what type of injury awaits you. The source of injury may be a pressure cooker whistle acting as a projectile or a hailstone falling from the sky. Such injuries are common in Nepal, considering the socio-demographic and geographic conditions. In this article, we present two such very rare cases of head injury. Case Reports: The first case is a middle-aged woman who sustained an accidental injury to the face associ

Anesthetic Management of Penetrating Tracheal Injury in a 3-Year-Old Child: A Case Report.

Penetrating tracheal injuries in children are uncommon and present significant diagnostic and airway challenges. We report the case of a 3-year-old boy who sustained an accidental penetrating neck trauma and was taken up for tracheal repair. Initial radiological evaluation did not reveal any tracheal/bronchial injury, but there was extensive pneumomediastinum. The child exhibited signs of airway compromise along with a deep penetrating injury on the anterior neck, extensive subcutaneous and musculofascial emphysema in the neck and anterior chest wall. Anesthetic management required careful ind

Phosgene-Induced acute lung injury: Approaches for mechanism-based treatment strategies.

Phosgene (COCl2) gas is a chemical intermediate of high-volume production with numerous industrial applications worldwide. Due to its high toxicity, accidental exposure to phosgene leads to various chemical injuries, primarily resulting in chemical-induced lung injury due to inhalation. Initially, the illness is mild and presents as coughing, chest tightness, and wheezing; however, within a few hours, symptoms progress to chronic respiratory depression, refractory pulmonary edema, dyspnea, and hypoxemia, which may contribute to acute respiratory distress syndrome or even death in severe cases.

Glass table tops: An unusual source of lethal domestic injury.

A 42-year-old woman who fell through a glass tabletop had her lower back pierced by a long shard of glass. She rapidly exsanguinated. At autopsy, a single penetrating wound was present in her left lower back, with complete transection of her left kidney and a 3.5 L haematoperitoneum. Death was due to exsanguination following accidental transection of the left kidney by a penetrating glass injury of the lower back. Glass-topped tables are a well-recognised source of injury in a domestic setting There are far more non-lethal than lethal injuries, many of which involve children. Quite long shards

Coronal clefts in infants - rare differential diagnosis of traumatic injuries of vertebral bodies in battered children.

Accidental and nonaccidental spinal injuries are generally rarely seen in infants. If affected, vertebral bodies usually present compression fractures due to forced hyperflexion or hypertension. Radiographic examination of the infantile skeleton can reveal a radiolucent band running through a vertebral body. These so called vertebral clefts are mainly visualized in the lateral spinal radiograph. Usually they can be found in the 1st year of life. Radiological appearance of coronal clefts was compared to that of a traumatic vertebral compression fracture. Clefts were mostly localized in the lumb

Management of a full-thickness burn to the foot following sciatic nerve palsy secondary to primary total hip replacement.

Sciatic nerve palsy following total hip replacement is a rare but serious complication. The neurological sequelae that follow range from pure sensory loss to combined motor and sensory loss involving most of the lower limb. The loss of nociceptive feedback predisposes patients to accidental damage to the lower limb. We present the case of a lady with sciatic nerve palsy who sustained full-thickness burn injuries to her foot via a hot water bottle. The dilemma between debridement and grafting following a recent history of surgical trauma (ie, total hip replacement) versus secondary healing via

[Endovascular repair for an acute traumatic aortic transection: a case report].

A thirty-eight-year-old male patient who suffered from 10th and 11th thoracal vertebrae fractures, paraplegia and acute traumatic aortic transection because of accidental fall was referred to our hospital. Open surgical repair carried a very high risk due to severe coexisting injuries. Transection was treated with 30x100 mm Valiant thoracic endograft, which was deployed just distal to the ostium of the left carotid artery. The patient was transferred to the neurosurgery clinic for treatment of paraplegia after an uneventful recovery. Endovascular repair of acute transection confers substantial

Reflections on the syndrome of the "shaken baby".

A full appreciation of the development of current concepts concerning the "shaken baby syndrome" requires a systematic review of historic and more recent interpretations of scientific data that form the bases for diagnosis in these cases. This discussion relates to the dilemma in determining the cause of shaking injuries in this age group that are not obviously accidental or inflicted. The cases in question can present with similar clinical and pathological findings and possibly result from innocent activity. Shaking of infants and children for any reason should be avoided. Debates related to
