Sinonasal adenoid cystic carcinoma: the M. D. Anderson Cancer Center experience.

BACKGROUND: Adenoid cystic carcinoma of the sinonasal tract is a rare cancer that accounts for 10% of all malignancies at this site. The objective of the current study was to evaluate prognostic factors, treatment outcomes, recurrence patterns, and survival rates for sinonasal adenoid cystic carcinoma. METHODS: A retrospective chart review was performed at an academic tertiary referral center. Between 1990 and 2004, 105 patients were evaluated for adenoid cystic carcinoma of the sinonasal tract at a single institution. Demographics, presentation, anatomic site, Tumor, Lymph Node, Metastasis (

Unusual localization of an adenoid cystic carcinoma subsequent to superficial parotidectomy for a benign lesion.

Salivary gland tuberculosis is a rare pathology and does not always have the diagnostic guidelines led by previous tubercular localizations. Tubercular bacillus can reach the salivary gland in different ways, but the condition that most frequently causes a tubercular infection is the decay of the human organism defensive capacity toward the germ.A 15-year-old Romanian boy presented with a swelling in the left submandibular region. Normal extraoral, intraoral examination, and routine diagnostic examinations were performed. In order to make a final diagnosis, we performed a surgical left submand

Thymic tumor with adenoid cystic carcinomalike features: a clinicopathologic study of 4 cases.

Thymic carcinomas are rare malignant neoplasms which comprise several histologic subtypes. Adenoid cystic carcinoma (ACC) is included among these subtypes even if it has never been formally reported. We evaluated the clinical, radiologic, morphologic, immunohistochemical, and genetic features of 4 cases of thymic neoplasms with ACC-like features retrieved from the authors' consult files. Most cases affected adult/elderly males (mean 68.5 y; range: 63 to 77 y; M:F ratio=3:1), and were asymptomatic. The clinical history (no evidence of ACC in other sites), radiologic findings (a mass in the thym

[Prognostic analysis of adenoid cystic carcinoma of major salivary glands of 64 cases].

BACKGROUND & OBJECTIVE: Adenoid cystic carcinoma of salivary glands is a kind of highly malignant tumor, and researches on the tumors originated from major salivary glands are rare. This study was to investigate prognostic factors of adenoid cystic carcinoma of major salivary glands. METHODS: Clinical records of 64 patients with adenoid cystic carcinoma of major salivary glands followed up for more than 7 years were reviewed. Cumulative survival rate was analyzed by the Kaplan-Meier method. The log-rank test was applied to compare survival rates, and the comparison of the percentage of subjec

Dedifferentiation of adenoid cystic carcinoma: report of a case implicating p53 gene mutation.

Adenoid cystic carcinoma is an indolent tumour with an unfavorable long-term prognosis. Dedifferentiation of adenoid cystic carcinoma, which is associated with an accelerated clinical course, has recently been described. We report a case with immunohistochemical and molecular workup to elucidate the likely mechanism of dedifferentiation. The patient, a 64-year-old woman, developed dedifferentiated adenoid cystic carcinoma of the submandibular gland ab initio, accompanied by cervical lymph node metastasis. Histologically, the low-grade adenoid cystic carcinoma merged gradually into an extensive

Primary Adenoidal Hodgkin's Disease: Report of a Case with an Unusual Morphology and Review of the Literature.

The nasopharyngeal lymphoid tissue (adenoids) is an uncommonly reported primary site for Hodgkin's disease. We report a case of primary adenoidal, interfollicular, epithelioid cell-rich variant of mixed cellularity Hodgkin's disease. The combination of an interfollicular pattern and richness of epithelioid histiocytes made it very difficult to make the diagnosis of Hodgkin's disease without the help of immunohistochemistry. To our knowledge, only 39 cases of Hodgkin's disease primarily involving the Waldeyer's ring have been reported in the English literature, 24 of these primarily involving t…

Central (intraosseous) adenoid cystic carcinoma of the mandible: report of a case with periapical involvement.

Primary intraosseous salivary gland tumors are rare, with mucopidermoid carcinoma being the most frequent histotype. The authors present a case of adenoid cystic carcinoma, located in the mandibular incisor region, associated with pain. Endodontic treatment resulted in increased pain and progressive mandibular expansion. An apicoectomy was conducted, and an intraosseous adenoid cystic carcinoma was diagnosed at histological examination. The patient was treated by wide surgical resection, and is alive and well without recurrences or distant metastases 14 yr after the original diagnosis. The cas

[Study on the effects of adenoid-tonsillar operation in infants under 2 years of age].

Infants with upper airway obstruction caused by adenotonsillar hypertrophy often suffer from sudden death. We have performed adenotonsillar operations on patients under 2 years of age. These infants had sleep apnea, dyspnea, poor increase of body weight or cardiac hypertrophy. This is a report on a clinical study on these infants. Between October 1988 and February 1998 eighteen patients under 2 years of age (17 boys and one girl) had an adenotonsillar operation in our hospital. Three had adenotomy and two had adenotomy and one-sided tonsillectomy. The remaining thirteen patients had adenotonsi

Replication of HIV-1 in dendritic cell-derived syncytia at the mucosal surface of the adenoid.

Human immunodeficiency virus-type 1 (HIV-1) replicates actively in infected individuals, yet cells with intracellular depots of viral protein are observed only infrequently. Many cells expressing the HIV-1 Gag protein were detected at the surface of the nasopharyngeal tonsil or adenoid. This infected mucosal surface contained T cells and dendritic cells, two cell types that together support HIV-1 replication in culture. The infected cells were multinucleated syncytia and expressed the S100 and p55 dendritic cell markers. Eleven of the 13 specimens analyzed were from donors who did not have sym

Successful treatment of concomitant acute myeloblastic leukaemia and adenoid cystic carcinoma of the palate.

The case of a 55-year old man who was affected simultaneously by acute myeloblastic leukaemia (AML) and adenoid cystic carcinoma of the palate is reported. The carcinoma was removed after induction chemotherapy for AML and the patient subjected to consolidation therapy. No reports are evident of other cases in which acute leukaemia and other malignancies were treated simultaneously, and in which the patient was cured of both. It is suggested that this case may serve as a reference for future similar cases.

Rearrangement of 9p13 as the primary chromosomal aberration in adenoid cystic carcinoma of the respiratory tract.

Two adenoid cystic carcinomas, one of the nasal cavity, the other a bronchial tumor, were cytogenetically analyzed. The former had a t(6;9)(q21-22;p 13-21) as the sole karyotypic abnormality. The latter had two related abnormal clones, resulting in the mosaic karyotype 46,XY,t(9;17)(p13;p13)/46,Y,t(X;6)(p22;q23),t(9;17)(p13;p13). The karyotypic profiles of the two cases, the only respiratory tract adenoid cystic carcinomas that have been cytogenetically characterized, differ little from those of previously reported adenoid cystic carcinomas of the major salivary glands, underscoring the fundam

Adenoid cystic carcinoma of salivary glands. A study of 61 cases with clinicopathologic correlation.

Sixty-one cases of adenoid cystic carcinoma of the head and neck region, excluding the ear canal, lacrimal glands, larynx, esophagus, and trachea, were studied, and their different clinicopathologic aspects were analyzed. Adenoid cystic carcinoma occurred more commonly in the minor salivary glands; the palate was affected in 31% of the cases. The fifth decade of life was the age at which patients were most commonly affected, and there was a slight predominance of white women. In most patients a mass was the main complaint; in 63% the duration of symptoms was 1 year or less. Forty-one patients

How should adenoidal enlargement be measured? A radiological study based on interobserver agreement.

Clinical assessment of adenoidal size is difficult, and an objective measurement is desirable. To establish the best radiological method, lateral cephalometric radiographs were taken of 38 children three days before their adenoids were removed. Two radiologists each made five measurements related to adenoidal size on ech radiograph, and the correlations between the radiological measurements and between the measurements and the volume of tissue removed at operation were calculated. Linear measurement of the palatal airway at the narrowest point between the outlines of the adenoid and soft palat…

Yes-associated protein 1 (YAP1) expression in adenoid cystic carcinoma of the lacrimal gland.

PURPOSE: To characterize nuclear expression of Yes-associated protein 1 (YAP1), a key effector of the Hippo pathway, in lacrimal gland adenoid cystic carcinoma (LG ACC) and explore potential associations with clinicopathologic features and clinical outcomes. METHODS: A retrospective, multi-institutional study of 24 LG ACC specimens was performed. Immunohistochemistry (IHC) was used to assess nuclear YAP1 expression. Staining intensity and percentage expression were combined into a composite NY-SCORE. Exploratory analyses evaluated relationships between YAP1 expression scores, clinicopathologi

Bronchopleural fistula following sleeve resection for bronchial adenoid cystic carcinoma-a case report.

BACKGROUND: Tracheobronchial adenoid cystic carcinoma (ACC) is a rare malignant airway tumor characterized by slow growth but marked submucosal extension, which often makes complete resection difficult. When repeated positive margins are encountered intraoperatively, the surgeon must balance oncologic radicality against preservation of lung function and the safety of airway reconstruction. Postoperative bronchopleural fistula (BPF) is an uncommon but potentially life-threatening complication after bronchial reconstruction. CASE PRESENTATION: A 33-year-old female was admitted with a chief comp

Circumferential Extramural Spread of Tracheal Adenoid Cystic Carcinoma Causing Unexpected Positive Margin.

Tracheal adenoid cystic carcinoma (ACC) often shows submucosal extension, leading to positive margins. A male patient presented with dyspnea due to an intraluminal tumour arising from the membranous mid-trachea. Rigid bronchoscopy debulking confirmed ACC. In a second-stage operation, circumferential resection of approximately 3.8 cm of trachea with end-to-end anastomosis achieved macroscopic clearance. Intraoperative frozen sections, however, revealed tumour cells within lymphatic channels of the tracheal sheath at the distal cartilaginous-side margin opposite the membranous lesion, precluding
