Nonsteroidal anti-inflammatory drugs in ophthalmology.

Nonsteroidal anti-inflammatory drugs (NSAIDs) are increasingly employed in ophthalmology to reduce miosis and inflammation, manage scleritis, and prevent and treat cystoid macular edema associated with cataract surgery. In addition, they may decrease postoperative pain and photophobia associated with refractive surgery and may reduce the itching associated with allergic conjunctivitis. In recent years, the U.S. Food and Drug Administration has approved new topical NSAIDs, and previously approved NSAIDs have been reformulated. These additions and changes result in different pharmacokinetics and

[Topical inhibition of angiogenesis at the cornea. Safety and efficacy].

BACKGROUND: The efficacy and safety of novel topical inhibitors of corneal neovascularisation will be discussed. METHODS: A literature review after a PUBMED search and own clinical and experimental results are presented. RESULTS: The off-label use of Avastin eye drops and GS101 eye drops against insulin receptor substrate (IRS)-1, which have been tested in phase II trial, both seem to be relatively efficient and safe ways to inhibit progressive corneal neovascularisation. Other VEGF antagonists, such as ranibizumab and pegaptanib eye drops also inhibit corneal neovascularisation. CONCLUSION

Topical ocular delivery of NSAIDs.

In ocular tissue, arachidonic acid is metabolized by cyclooxygenase to prostaglandins which are the most important lipid derived mediators of inflammation. Presently nonsteroidal anti-inflammatory drugs (NSAIDs) which are cyclooxygenase (COX) inhibitors are being used for the treatment of inflammatory disorders. NSAIDs used in ophthalmology, topically, are salicylic-, indole acetic-, aryl acetic-, aryl propionic- and enolic acid derivatives. NSAIDs are weak acids with pKa mostly between 3.5 and 4.5, and are poorly soluble in water. Aqueous ophthalmic solutions of NSAIDs have been made using so

Effective treatment with topical cyclosporin A of a patient with Cogan syndrome.

The purpose of this report is to describe the effective treatment of severe anterior segment inflammation due to Cogan syndrome through the use of topical administration of cyclosporin A. A 47-year-old female patient had been experiencing headaches and difficulties with her vision. Subsequent examination revealed the sudden onset of bilateral conjunctival injection and swelling of bilateral auricles. Despite the multiple treatment (systemic and topical corticosteroid and antibiotic therapy), necrotizing scleritis had appeared bilaterally and the scleral wall was thinning. Topical administratio

Additivity of prostaglandin F2 alpha-1-isopropyl ester to timolol in glaucoma patients.

The effect on intraocular pressure (IOP) of adding prostaglandin F2 alpha-1-isopropyl ester (PGF2 alpha-IE) to timolol was studied in 21 eyes of 13 patients with newly diagnosed primary open-angle glaucoma that was inadequately controlled with timolol alone. After at least 2 weeks of twice daily timolol 0.5% therapy, PGF2 alpha-IE, 0.5 microgram in 30 microliters, was topically applied twice daily at 8 AM and 8 PM, 5 minutes before each timolol dose, for 7 days. Intraocular pressures were measured before timolol treatment, and at 8 AM, 8:30 AM, 12 PM, 2 PM, and 4 PM on the day before the addit

Effects of various concentrations of diethylcarbamazine citrate applied as eye drops in ocular onchocerciasis, and the possibilities of improved therapy from continuous non-pulsed delivery.

Diethylcarbamazine was given as eye drops in varying concentrations in a half-log dilution series from 1.0 to 0.0001% to patients with ocular onchocerciasis. Migration of microfilariae into the cornea, followed by their straightening and disintegration, was observed with delivery rates as low as 0.1 microgram/hour. Dose-related adverse inflammatory reactions, including the development of globular limbal infiltrates with itching and redness, were seen with delivery rates as low as 0.6 microgram/hour, but substantial inflammatory reactions, including severe vasculitis, were seen only with delive

Phlyctenular keratoconjunctivitis in a patient with Staphylococcal blepharitis and ocular rosacea.

BACKGROUND: Phlyctenular keratoconjunctivitis is a type IV hypersensitivity reaction to an inciting agent. The presentation is usually bilateral. The diagnosis can be difficult, especially if compounded by an underlying skin disorder such as rosacea. The 2 types of phlyctenules are differentiated based on the location on the eye, i.e., conjunctival or corneal. CASE REPORT: A 22-year-old man presented with painful, bilateral, elevated vascularized corneal lesions. Ultimately, the diagnosis was bilateral phlyctenular keratoconjunctivitis secondary to concurrent posterior blepharitis and recurre
