The case is reported of a patient who initially presented with a dendritiform corneal ulcer that ultimately failed to heal and in which Mycobacterium chelonei was repeatedly cultured. The organism was sensitive only to kanamycin and amikacin; however, topical administration of these antibiotics failed to achieve a complete cure. Penetrating keratoplasty was ultimately required to eradicate the organism from the cornea.

Photodynamic therapy (PDT) has been found to be effective for the treatment of acne vulgaris. We now wish to present a case series of seven patients who were successfully treated for chronic folliculitis with PDT after photosensitization with topical methyl aminolevulinate. The folliculitis had occurred in areas of acne-prone skin of the patients, mainly after long-term administration of antibiotics.

Three cases of symptomatic bradycardia due to topical ocular timolol administration are reported. Two patients had syncope related to atrioventricular block, and the other one complained of dizziness due to sinus bradycardia. Acute cyanosis and methemoglobinemia developed following topical application and partial ingestion of benzocaine for esophagogastroduodenoscopy. A diagnosis of acute toxic methemoglobinemia should be considered when cyanosis, with or without neurologic symptoms, occurs following the use of local anesthetics in the absence of cardiopulmonary disease. Laboratory tests should include oxygen saturation and methemoglobin concentration. Management includes supplemental oxygen administration and intravenous methylene blue.
