Buddy Wire-Supported Distal Access Catheter Advancement Technique (B-DAT) for Difficult Vertebral Artery Access: A Technical Note.

OBJECTIVE: Endovascular treatment (EVT) of posterior circulation lesions can be technically challenging when access to the vertebral artery (VA) is difficult because of tortuosity, stenosis, or a narrow caliber. Stable advancement of the distal access catheter (DAC) is often required for safe and effective EVT; however, selective VA cannulation with a guiding catheter (GC) may increase the risk of vascular injury or compromise flow in an unfavorable anatomy. We developed a buddy wire-supported DAC advancement technique (B-DAT) to facilitate direct DAC advancement into the VA. We evaluated the

An acceptability pilot of the facilitating active management in lung illness with engaged surrogates (FAMILIES) study.

Approximately half of the surrogate decision makers of critically ill adults are at risk for negative emotional burden. Decision support and effective surrogate-clinician communication buffers against such experiences. The objective of this study is to evaluate the acceptability of a new surrogate-targeted educational tool that promotes engagement with clinicians and advocacy for 2 evidence-based practices in the provision of mechanical ventilation for acute respiratory failure: spontaneous awakening and breathing trials.A panel of 44 former patients and surrogates of a 20-bed medical intensiv

Object features and T4/T5 motion detectors modulate the dynamics of bar tracking by Drosophila.

Visual objects can be discriminated by static spatial features such as luminance or dynamic features such as relative movement. Flies track a solid dark vertical bar moving on a bright background, a behavioral reaction so strong that for a rigidly tethered fly, the steering trajectory is phase advanced relative to the moving bar, apparently in anticipation of its future position. By contrast, flickering bars that generate no coherent motion or have a surface texture that moves in the direction opposite to the bar generate steering responses that lag behind the stimulus. It remains unclear how

Fingertip reconstruction using two variations of direct-flow homodigital neurovascular island flaps.

The fingertip is an extremely intricate area of digital sensibility that plays an important role in fine perception and hand function. Thus, sensate fingertip reconstruction is essential to the recovery of most hand functions. The authors used two methods of direct-flow homodigital neurovascular island flap coverage to reconstruct distal finger amputations-namely, the triangular-advancement flap technique and the step-advancement flap method. The authors present their experience with these two variations of direct-flow homodigital neurovascular island flaps and their use in reconstructing 18 f

Record health care directives before terminal illness!

Physicians share the grief and tension of families who must make decisions for incompetent, dying members. The author recorded the wishes of 1000 competent patients to guide families and the health care team during terminal illness. A questionnaire, which became part of the medical record, was used to determine views of organ donation, religious beliefs, and other directives. Patients were eager to record and discuss their wishes during an office or hospital visit; 94% of all patients wanted to participate in health care decisions. When directives for terminal care are available, families will

Why does HIV infection not lead to disseminated strongyloidiasis?

We investigated the hypothesis that host immunosuppression due to advancing human immunodeficiency virus (HIV) disease favors the direct development of infective larvae of Strongyloides stercoralis, which may facilitate hyperinfection and, hence, disseminated strongyloidiasis. To do this, we sought correlations between the immune status of the subjects and the development of S. stercoralis infections. Among 35 adults, there were significant negative rank correlations between CD4+ cell counts and the proportions of free-living male and female worms. Thus, in individuals with preserved immune fu

The actual use of directional steering and shorter pulse width in selected patients undergoing deep brain stimulation.

INTRODUCTION: Directional deep brain stimulation (DBS) and pulse with <60μs increase side-effects threshold, enlarging the therapeutic window. However, new systems allowing these advanced features are more expensive and often available only for a limited number of patients in some centers. It is unknown how many and which DBS patients actually need the advanced features because of an insufficient improvement with standard parameters. METHODS: We included in the analysis all patients with Parkinson's disease, dystonia and tremor who were selected to receive implantation of advanced DBS systems

Comprehensive analysis of soft tissue changes in response to orthognathic surgery: mandibular versus bimaxillary advancement.

This study was performed to compare soft tissue changes in response to mandibular and bimaxillary advancement osteotomy. Preoperative and postoperative cone beam computed tomography scans of 24 cases were analysed: 12 underwent bimaxillary advancement and 12 underwent mandibular advancement. The skeletal surgical movements were measured and soft tissue changes were displayed on a three-dimensional colour map. The intensity and shade of the colour indicated the magnitude and direction of the changes. In the bimaxillary advancement group, maxillary advancement was 5.5±2.7mm with anterior vertica

Coagulation factor Xa inhibition: biological background and rationale.

Ischemic heart disease and cerebrovascular disease are the leading causes of death in the world. Surprisingly, these diseases are treated by relatively antiquated drugs. However, due to our improved understanding of the underlying pathology of these diseases, and a number of technological advances in tools for drug discovery and chemical optimization, an exciting new wave of antithrombotic compounds is beginning to emerge in clinical trials. These agents, referred to as direct coagulation factor Xa inhibitors, appear to provide an enhanced risk-benefit margin compared to conventional therapy.

Organ transplantation: contemporary Sunni Muslim legal and ethical perspectives.

The problems that organ transplantation poses to the Muslim mind may be summarized as follows: firstly, a Muslim believes that whatever he owns or possesses has been given to him as an amanah (trust) from Allah. Would it not be a breach of trust to give consent for the removal of parts of one's body, while still alive, for transplantation to benefit one's child, sibling or parent? Secondly, the Shari'ah (Islamic Law) emphasizes the sacredness of the human body. Would it not then be an act of aggression against the human body, tantamount to its mutilation, if organs were to be removed after dea

Directional dark field for nanoscale full-field transmission X-ray microscopy.

Dark-field X-ray imaging visualizes structural inhomogeneities through small-angle scattering, but existing directional methods are confined to the micrometer scale. While recent advances have extended dark field capabilities to nanoscale transmission X-ray microscopy, directional scattering retrieval - critical for characterizing anisotropic nanostructures - has remained inaccessible for imaging resolutions in the sub-micrometer scale. Here, we demonstrate the first directional dark-field setup for nanoimaging, achieving orientation mapping of scattering features below the spatial resolution

Medical Management of Heart Failure With Reduced Ejection Fraction in Patients With Advanced Renal Disease.

Large randomized clinical trials (RCT) supporting guidelines for the management of heart failure with reduced ejection fraction (HFrEF) have typically excluded patients with advanced chronic kidney disease (CKD). Patients with concomitant advanced CKD and HFrEF experience poor cardiovascular outcomes and mortality relative to either disease in isolation and have been shown to consistently receive lower rates of HFrEF guideline-directed medical therapy (GDMT). This review evaluated recent evidence for the use of GDMT in patients with HFrEF and advanced CKD approaching dialysis from RCTs and obs

Evidence-based Strategies for Advanced Heart Failure.

Heart failure is a complex disease that interacts with other organ systems, such as the lungs, kidneys, and the liver, in a complex manner. The stages of heart failure often define the acuity and onset of disease symptoms and progression as well as treatment measures, in addition to the patient's functional class. As the neurohormonal and compensatory mechanisms become resistant and fail guideline-directed medical therapy, more advanced therapies are required. This article outlines treatments for advanced heart failure and provides a review of guideline-directed medical therapies leading into
