The incidence, characteristics, and complications of pregnant women who delivered stillbirths under different child policies in central China.

BACKGROUND: China's evolving fertility policies (one-child to three-child) have shaped maternal and neonatal outcomes, but specific gaps in stillbirth epidemiology during policy transitions. METHODS: This retrospective cohort study analyzed 721,860 singleton pregnancies in 2011-2023, from 18 maternal near-miss surveillance hospitals in Hunan. Stillbirth rates were assessed across four policy periods: one-child (2011-2013), partial two-child (2013-2015), universal two-child (2016-2020), and three-child (2021-2023). Multivariable logistic regression identified risk factors, adjusting for fertil

Preconception exposures and adverse pregnancy, birth and postpartum outcomes: Umbrella review of systematic reviews.

BACKGROUND: Preconception exposures have been associated with adverse pregnancy, birth and postpartum outcomes. However, the reports, statements and guidelines of national and international health organisations vary in what they recommend individuals should monitor, avoid, reduce or practise in the preconception period. OBJECTIVES: To synthesise and evaluate the evidence across systematic reviews for associations between exposures before conception and adverse pregnancy, birth and postpartum outcomes. DATA SOURCES: MEDLINE, Embase, Epistemonikos (to May 2020) and reference lists of included

Assessed early-pregnancy exposures included advanced maternal age, smoking, depressive symptoms, physical inactivity, insufficient sleep, and pre-pregnancy overweight or obesity. Risk Factors for Prolonged Intertwin Delivery Interval and Associated Maternal and Second Twin Morbidity.

OBJECTIVE: This work aimed to identify possible risk factors and the morbidity associated with prolonged intertwin delivery interval (IDI). … and with an increased rate of cesarean delivery, placental abruption, and hemoglobin drop ≥ 3 g/dL. Advanced maternal age, nulliparity, and twin birth weight ratio ≥ 1.2 are associated with prolonged IDI. KEY POINTS: · Prolonged was found to be associated with higher neonatal acidemia and lower 5-minute Apgar score.. · Prolonged IDI is also associated with increased r …

A multifactorial fracture risk analysis and vacuum-assisted delivery simulation across gestational ages using CT-derived finite element modeling with probabilistic material analysis.

Fetal tissue properties, such as cranial stiffness, increase with gestational age, creating a potential risk of injury during vacuum-assisted delivery (VAD) for both the neonate and the mother. Using nonlinear CT-derived finite element models combined with uncertainty quantification, this study evaluated the biomechanical responses across a wide range of fetal tissue and scalp properties, as well as maternal pelvic tissues. Simulations indicate that as the fetal head stiffens, maximum principal stress on the cranium rises, suggesting an increased likelihood of fracture, particularly at term. S

DESIGN: A secondary analysis. SETTING: Data from the seventh round of the DHS data of four South Asian countries; Pakistan, Nepal, India and Maldives. Effect of cervical changes on the cesarean scar topography after primary cesarean sections: an observational study.

BACKGROUND: The increasing global rate of cesarean sections (CS) has raised concern about associated long-term complications, particularly uterine scar niches. A niche represents a localized myometrial defect at the hysterotomy site, which may lead to abnormal bleeding, pelvic pain, or subfertility. OBJECTIVE: to assess the scar area, niche formation and isolated myometrial defect creation after CS in women undergoing their 1st CS at various stages of 1st stage of labor, irrespective of gestational age. METHODS: This prospective observational cohort study was conducted at Ain Shams Universit

The biochemical pregnancy loss rate remains stable up irrespective of age and differs in pattern from clinical miscarriages.

INTRODUCTION: As women age, the increasing rate of aneuploidy lead to an augmentation in the incidence of clinical miscarriages. It was anticipated that biochemical pregnancy rates would also rise with maternal age. However, no study has previously evaluated the effect of maternal age on biochemical pregnancy rates. MATERIAL AND METHODS: A retrospective cohort study of 2177 subjects who underwent single embryo transfer (SET) as part of a fresh or thawed IVF cycle were recruited from 2008 through 2012, resulting in 952 pregnancies. Data was stratified for age and compared using analysis of var

[Pregnancy in women aged 43 years or older: maternal and perinatal risks].

OBJECTIVES: The proportion of women with advanced age at the time of delivery increases constantly, partly due to assisted reproductive technologies. Maternal morbidity is known to increase with maternal age, whereas data on perinatal outcomes are controversial. Risks of maternal and perinatal complications may be elevated among older pregnant women. The purpose of this study is to quantify these risks among 43 years or older women in comparison with women aged 25 to 35 years, the age known to be optimal for a pregnancy. PATIENTS AND METHODS: A retrospective study was conducted during three y

Tuberculosis in pregnancy: the challenges for South Asian countries.

AIM: Despite tuberculosis (TB) being a global problem, maternal TB remains an unrecognized and underestimated tragedy, especially in South Asian countries. Therefore, we performed a non-systematic review regarding implications of maternal TB on obstetric and perinatal outcomes in the South Asian context. MATERIAL AND METHODS: We reviewed original studies, both descriptive and analytical, that originated from South Asian countries following an electronic search supplemented by a manual search. Although relevant studies from developed countries were reviewed, they were not included in the tabul

Maternal Risk Factors Associated With Neonatal Outcome in Primiparous, Machala, Ecuador.

Introduction The resulting neonatal, weight of the newborn (NB) is considered as a health indicator since the nutritional status of the neonate can have repercussions on the growth and development of the child until adulthood. Secondly, preterm delivery is associated with several maternal risk factors, such as the presence of anemia, adolescence, or advanced age. The aim of the study was to determine the maternal risk factors related to neonatal outcomes in primiparous. Methods A descriptive, observational, quantitative, longitudinal, and non-experimental study was conducted. Data were collect

De novo mutations producing unstable hemoglobins or hemoglobins M : I. Establishment of a Depository and use of data to test for an association of de novo mutation with advanced parental age.

Hemoglobins M and unstable hemoglobins cause clinical syndromes that are transmitted in autosomal dominant fashion. Pedigrees of 50 probands with de novo mutations producing unstable Hb disease or Hb M disease were compiled. Cases were ascertained (1) by screening the relevant literature published from 1950 through 1980 and (2) through personal communication. Additional pedigree data on several published cases were collected, and a depository containing all available information rekated to de novo Hb mutants was established. The 50 probands were born in 14 countries between 1922 and 1976. Pate

The gonadal niche safeguards human fetal germline cell development following maternal SARS-CoV-2 infection.

During pregnancy, germline development is vital for maintaining the continuation of species. Recent studies have shown increased pregnancy risks in COVID-19 patients at the perinatal stage. However, the potential consequence of infection for reproductive quality in developing fetuses remains unclear. Here, we analyze the transcriptome and DNA methylome of the fetal germline following maternal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. We find that infection at early gestational age, a critical period of human primordial germ cell specification and epigenetic reprog
