Non-alcoholic beer consumption among alcohol consumers in the US.

OBJECTIVES: Non-alcoholic beer may help adults reduce alcohol intake, but it could also undermine alcohol reduction efforts. Few studies have investigated how and why adults consume non-alcoholic beer. We aimed to establish nationally representative estimates of the prevalence, patterns, motivations, and correlates of non-alcoholic beer consumption among US regular alcohol consumers. METHODS: In October 2024, we conducted a cross-sectional, nationally representative survey of US adults ages 21 + years who drank alcohol at least once a week in the past four weeks. We assessed past-year non-alc

Addressing Alcohol Use Disorder in Alcohol-Associated Liver Disease.

Alcohol use disorder (AUD) is a prevalent public health challenge currently. Alcohol-associated liver disease, the commonest end-organ damage, is increasing at an alarming rate. It is currently the leading cause worldwide of hospitalization, liver transplantation, and death from cirrhosis of the liver. Furthermore, AUD in patients with liver disease remains untreated, which consequently accelerates liver injury and worsens patient-related outcomes and healthcare burden. It is therefore very critical for all clinicians and healthcare providers to screen for and address AUD at any healthcare enc

The use of alcohol-free and low-alcohol drinks in pregnancy in the UK.

Alcohol-free drinks [beers, ciders, wines, and spirits containing <0.05% alcohol by volume (ABV)], and low-alcohol drinks (between 0.05% and 1.2% ABV) are increasingly available and may be used as a harm reduction measure. However, it is not known what pregnant women think and feel about these drinks and how regularly they are consumed before and during pregnancy. A cross-sectional online survey was developed and piloted. Women ≥18 years in the UK who were pregnant, or recently pregnant, were recruited via targeted social media advertising. Of the 2092 respondents, 47.8% (n = 1001) were curren

Pharmacotherapy to Prevent Alcohol Relapse in Alcohol-Associated Liver Disease.

PURPOSE OF REVIEW: Alcohol use disorder (AUD) drives alcohol-associated liver disease (ALD), and relapsing after abstinence remains a significant challenge before and after transplantation. This review summarizes evidence for pharmacotherapies in relapse prevention and their integration into ALD care. RECENT FINDINGS: Naltrexone and acamprosate reduce the relapse in the general AUD population, though data in ALD are limited. Baclofen is the only drug tested in randomized trials in cirrhosis, with early benefit but mixed results in later studies. Gabapentin and topiramate are promising off-lab

Managing Alcohol Use Disorder in Alcohol-Related Liver Disease.

This review article focuses on the management of alcohol use disorder (AUD) in patients with comorbid alcohol-associated liver disease (ALD). We highlight the importance of different screening tools for AUD in addition to the role of FDA and off-label medications for AUD and ALD treatment. For long-term recovery, we also speak to the importance of non-pharmacological interventions, including mutual aid organizations such as alcohols anonymous, peer recovery specialists, and emerging digital tools. Through a combination of these different tools, we can aid in patients in alcohol consumption red

Understanding How Alcohol Induces Human Acute Alcoholic Pancreatitis.

Why only a minority of heavy alcohol drinkers develop acute alcoholic pancreatitis has been a puzzle. In this review, the sparse data available from published studies were collected and, based on them, a hypothesis was formed. Long-term high alcohol consumption results in lowered cholecystokinin and cholinergic stimulus of the pancreas, and causes concentration and acidification of pancreatic fluid, predisposing to protein secretion. Early during the withdrawal period when returning to a normal or high-fat nonalcoholic diet, there is a relative hyperstimulation of the pancreas, a well-establis

Latest Insights in Alcohol-Related Liver Disease and Alcoholic Hepatitis.

Alcohol-related liver disease (ALD) is still to this date one of the leading causes of chronic liver disease globally. ALD comprises a wide disease spectrum, from the benign liver steatosis, to the life-threatening inflammatory acute phenotype of alcoholic hepatitis (AH) and ultimately, advanced liver fibrosis and cirrhosis. AH represents an acute inflammatory liver condition caused by prolonged high quantities of alcohol intake. Disease outcome varies from mild to severe, with systemic implication and high mortality. Although the pathogenesis has been extensively studied over the years, littl

Diagnosis of Alcohol Use Disorder and Alcohol-Associated Liver Disease.

Harmful alcohol use and alcohol use disorder (AUD) are common worldwide, and rates of alcohol-associated liver disease (ALD) are also increasing. AUD is a disease that is treatable and can be diagnosed and managed, and recovery from AUD through abstinence or reductions in drinking is possible. Management of AUD among individuals with ALD is increasingly being addressed via integrated medical and psychosocial treatment teams that can support reductions in drinking and prevent progression of liver disease. Early diagnosis of AUD and ALD can improve lives and reduce mortality.

Revisiting the alcohol-aggression link: The impact of alcohol consumption patterns.

Laboratory studies have repeatedly reported a link between alcohol and aggression, yet many rely on single-dose administration methods and overlook variations in alcohol consumption patterns. The present study investigates the effects of alcohol on aggressive behavior using a double-blind, placebo-controlled cumulative drinking administration approach that mirrors the natural drinking behaviors often observed in pubs within a laboratory setting. This study also pioneers the examination of how alcohol consumption patterns (light or heavy) moderate the relationship between precise Breath Alcohol

Microbiome Alterations in Alcohol Use Disorder and Alcoholic Liver Disease.

Microbiome alterations are emerging as one of the most important factors that influence the course of alcohol use disorder (AUD). Recent advances in bioinformatics enable more robust and accurate characterization of changes in the composition of the microbiome. In this study, our objective was to provide the most comprehensive and up-to-date evaluation of microbiome alterations associated with AUD and alcoholic liver disease (ALD). To achieve it, we have applied consistent, state of art bioinformatic workflow to raw reads from multiple 16S rRNA sequencing datasets. The study population consist

Fatty Liver Disease-Alcoholic and Non-Alcoholic: Similar but Different.

In alcohol-induced liver disease (ALD) and in non-alcoholic fatty liver disease (NAFLD), there are abnormal accumulations of fat in the liver. This phenomenon may be related to excessive alcohol consumption, as well as the combination of alcohol consumption and medications. There is an evolution from simple steatosis to steatohepatitis, fibrosis and cirrhosis leading to hepatocellular carcinoma (HCC). Hepatic pathology is very similar regarding non-alcoholic fatty liver disease (NAFLD) and ALD. Initially, there is lipid accumulation in parenchyma and progression to lobular inflammation. The mo

Sex Differences in Alcohol Consumption and Alcohol-Associated Liver Disease.

Alcohol-associated liver disease is becoming increasingly prevalent throughout the United States. Previously alcohol-associated liver disease was known to affect men more often than women; however, this gap between the sexes is narrowing. Studies show that women develop liver disease with lesser alcohol exposure and suffer worse disease as compared with men. This review article explores the increasing prevalence of alcohol-associated liver disease in women, reasons for changing patterns in alcohol consumption and liver disease development including obesity and bariatric surgery, proposed mecha

Patterns of Prenatal Alcohol Exposure and Alcohol-Related Dysmorphic Features.

BACKGROUND: In animal models, it is possible to induce different alcohol-related dysmorphic abnormalities based on the timing of prenatal alcohol exposure (PAE). Our objective was to assess whether patterns of PAE differentially predict alcohol-related dysmorphic features in 415 infants. METHODS: We analyzed a prospective pregnancy cohort in western Ukraine enrolled between 2008 and 2014. Five distinct trajectories were previously identified to summarize PAE: (i) minimal/no PAE (n = 253), (ii) low/moderate PAE with reduction early in gestation (n = 78), (iii) low/moderate sustained PAE (n = 2

Alcohol policies and alcohol-attributable cancer mortality in U.S. States.

BACKGROUND: Although more restrictive alcohol control policies (e.g., higher alcohol taxes) are related to lower levels of alcohol consumption, little is known about the relationship between alcohol policies and rates of alcohol-attributable cancer. METHODS: State alcohol policy restrictiveness, as measured by a validated policy scale, were related to state rates of six alcohol attributable cancers in the U.S. from 2006 to 2010 in a lagged, cross-sectional linear regression that controlled for a variety of state-level factors. Cancer mortality rates were from the Center for Disease Control an

Alcoholic and non-alcoholic steatohepatitis: global perspective and emerging science.

Alcohol and high-fat diet are two major risk factors responsible for metabolic diseases, which are manifested as steatohepatitis and liver cancer in the liver, and chronic pancreatitis and pancreatic adenocarcinoma (PDAC) in the pancreas. These metabolic diseases are becoming increasingly prevalent around the globe, and more importantly, their two major etiologies commonly coexist to precipitate the disease processes. To highlight the importance of these metabolic diseases, Japanese Society of Gastroenterology (JSGE) and National Institute on Alcoholism and Alcohol Abuse of National Institute

Prescription Drug and Alcohol Use Disorders: Alcohol Use Disorder.

Alcohol use is a common contributor to medical disease and a leading cause of preventable death. Because brief interventions for unhealthy alcohol use are effective, and because risk factors for unhealthy alcohol use are not always immediately identifiable, universal screening is recommended for adults. For patients who screen positive, physicians should assess further to diagnose alcohol use disorder, determine risk of withdrawal symptoms, and recommend an appropriate level of treatment. Outcomes of alcohol use disorder management are improved with evidence-based psychosocial therapy, but the

Use of alcohol biomarkers to identify alcohol misuse in organ donors.

Phosphatidylethanol is a direct alcohol biomarker for identifying alcohol misuse. It carries several advantages over other alcohol biomarkers, including a detection half-life of several weeks and little confounding by patient characteristics or organ dysfunction. The aim of this study is to derive an optimal phosphatidylethanol cut point to identify organ donors with alcohol misuse, and to assess the impact of alcohol misuse on organ allocation. Discrimination of phosphatidylethanol was evaluated using the area under the ROC curve from a mixed effects logistic regression model. Phosphatidyleth

[Fetal alcohol syndrome is not the only consequence of prenatal alcohol exposure].

Fetal alcohol syndrome is not the only consequence of prenatal alcohol exposure  The prevalence of fetal alcohol syndrome and fetal alcohol spectrum disorders in larger communities in USA is now updated to 0.4 % and 4.8 % respectively. Affected individuals bear witness to disease symptoms from many organ systems in addition to the brain and behavioural dysfunctions. In the light of modern epigenetic research, early alcohol exposure appears to play a hidden role in fetal reprogramming. The underlying mechanisms explain the »developmental origin of health and disease«, which has an impact on com

Alcohol Vapor Inhalation as a Model of Alcohol-Induced Organ Disease.

BACKGROUND: Chronic intermittent ethanol vapor (CIEV) exposure has been used extensively to produce rodent models of alcohol dependence, but unlike other models of alcohol abuse, CIEV has not been assessed as a model of end-organ damage. The purpose of this study was to characterize the effects of CIEV on peripheral organ systems affected by alcohol abuse, including the liver, lungs, and cardiovascular system. METHODS: Adult male Sprague-Dawley rats were exposed to daily CIEV for a period of 8 weeks (14HR ON/10HR OFF), producing blood alcohol levels of ~200 mg/dl. Controls were exposed to roo
