Therapeutic advances in traditional Chinese and Tibetan medicine for high-altitude cerebral edema.

INTRODUCTION: High-altitude cerebral edema (HACE) is an exacerbation of acute mountain sickness, but also a distinct disease characterized by vasogenic and cytotoxic brain edema. Exploring alternative therapies, such as traditional Chinese medicine (TCM) and Tibetan medicine (TM), is important. We aimed to provide a comprehensive overview of the existing research on the therapeutic effects and underlying molecular mechanisms of TCM and TM, encompassing herbal formulations, single botanical drugs, and their bioactive metabolites. METHODS: We employed keywords such as "TCM," "TM," "Botanical dr

Effect of acetazolamide on pulmonary and muscle gas exchange during normoxic and hypoxic exercise.

Acetazolamide (ACZ) is used to prevent acute mountain sickness at altitude. Because it could affect O2 transport in several different and potentially conflicting ways, we examined its effects on pulmonary and muscle gas exchange and acid-base status during cycle exercise at approximately 30, 50 and 90% VO2max in normoxia (F(IO2) = 0.2093) and acute hypoxia (F(IO2) = 0.125). In a double-blind, order-balanced, crossover design, six healthy, trained men (normoxic VO2max= 59 ml kg(-1) min(-1)) exercised at both F(IO2) values after ACZ (3 doses of 250 mg, 8 h apart) and placebo. One week later this

Store-operated channels in the pulmonary circulation of high- and low-altitude neonatal lambs.

We determined whether store-operated channels (SOC) are involved in neonatal pulmonary artery function under conditions of acute and chronic hypoxia, using newborn sheep gestated and born either at high altitude (HA, 3,600 m) or low altitude (LA, 520 m). Cardiopulmonary variables were recorded in vivo, with and without SOC blockade by 2-aminoethyldiphenylborinate (2-APB), during basal or acute hypoxic conditions. 2-APB did not have effects on basal mean pulmonary arterial pressure (mPAP), cardiac output, systemic arterial blood pressure, or systemic vascular resistance in both groups of neonat

Polymorphisms of renin--angiotensin system genes as a risk factor for high-altitude pulmonary oedema.

The genes of the renin--angiotensin system (RAS) play an important role in the regulation of pulmonary vascular tone. Although studies on individual genes polymorphisms have reported association with high-altitude pulmonary oedema (HAPE), studies on multiple genes or epistasis are lacking. We therefore investigated the association of the RAS polymorphisms with HAPE. In a case-control design, we screened 163 HAPE-resistant/controls (HAPE-r) and 160 HAPEpatients (HAPE-p) of Indian origin for eight polymorphisms of four RAS genes, ACE, AGT, AGTR1 and AGTR2. Significant difference in genotype and

Development and validation of a machine learning based early warning scoring system for high altitude polycythemia.

BACKGROUND: High-altitude polycythemia (HAPC) lacks a lifestyle-focused risk-stratification tool among lifelong high-altitude residents. Here we aimed to develop and validate a novel machine-learning predictive scoring system for HAPC using readily modifiable lifestyle variables in this population. METHODS: In a high altitude cohort (≥4,500 m, n = 1,089), 82 candidate variables were reduced to seven lifestyle predictors via LASSO, Logistic regression, XGBoost and random forest models were trained and compared (10 fold cross validation). RESULTS: Logistic regression achieved the best balance

Risk factors and outcomes of HPH in high-altitude residents via echocardiographic evaluation.

BACKGROUND: Over 80 million people live above 2,500 m, where hypobaric hypoxia increases hypoxic pulmonary hypertension (HPH) risk. In Qinghai, China (> 3,000 m), over 5 million face a high HPH burden with limited medical resources. OBJECTIVE: To evaluate risk factors and outcomes of HPH in high-altitude residents using echocardiography. METHODS: This retrospective study analyzed 627 HPH patients (PASP > 50 mmHg) in Qinghai (2018-2022), categorized as moderate (50 < PASP < 70 mmHg) or severe (PASP ≥ 70 mmHg). Clinical, biochemical, and echocardiographic data were assessed, with mortality tra

DNA methylation in adaptation to high-altitude environments and pathogenesis of related diseases.

High-altitude environments, characterized by hypoxia, low temperatures, and intense ultraviolet radiation, pose significant challenges to human physiology and health. DNA methylation, as a key epigenetic regulatory mechanism, plays a central role in human adaptation to high-altitude environments and in disease pathogenesis. Current research indicates that high-altitude native populations (such as Tibetans and Andeans) modulate the methylation of hypoxia-responsive genes like EPAS1 and EGLN1 to enhance oxygen transport efficiency and energy metabolism patterns, while simultaneously suppressing

Trimetazidine attenuates high-altitude fatigue and cardiorespiratory fitness impairment: A randomized double-blinded placebo-controlled clinical trial.

Trimetazidine (TMZ) has been shown to optimize myocardial energy metabolism and is a common anti-ischemic agent. Our trial (ChiCTR-TRC-13003298) aimed to explore whether TMZ has any preventive effect on high-altitude fatigue (HAF), cardiac function and cardiorespiratory fitness upon acute high-altitude exposure and how it works on HAF. Thirty-nine healthy young subjects were enrolled in a randomized double-blinded placebo-controlled trial and were randomized to take oral TMZ (n = 20) or placebo (n = 19), 20 mg tid, 14 days prior to departure until the end of study. Sleep architecture changes during a trek from 1400 to 5000 m in the Nepal Himalaya.

The aim of this study was to examine sleep architecture at high altitude and its relationship to periodic breathing during incremental increases in altitude. Nineteen normal, sea level-dwelling volunteers were studied at sea level and five altitudes in the Nepal Himalaya. Morning arterial blood gases and overnight polysomnography were performed in 14 subjects at altitudes: 0, 1400, 3500, 3900, 4200 and 5000 m above sea level. Subjects became progressively more hypoxic, hypocapnic and alkalinic with increasing altitude. As expected, sleep architecture was affected by increasing altitude. While

Susceptibility to mild and severe acute mountain sickness is associated with distinct urine metabolite profiles.

Rapid ascent to high altitude (HA) in unacclimatized lowlanders elicits a series of hematologic, ventilatory, metabolic, and cardiovascular adaptations to counteract the lower partial pressure of oxygen. When ascent occurs faster than the body can acclimatize, high-altitude illnesses (HAIs) can occur. The most prevalent of the HAIs is acute mountain sickness (AMS), which can range in severity from mild (minor inconvenience) to severe (total incapacitation). Symptoms of AMS include headache, fatigue, gastrointestinal distress, dizziness, and in some cases sleep disturbances. Identifying individ
