Spinal exposure for anterior lumbar interbody fusion (ALIF) in the lateral decubitus position: anatomical and technical considerations.

PURPOSE: Single position surgery has demonstrated to reduce hospital length of stay, operative times, blood loss, postoperative pain, ileus, and complications. ALIF and LLIF surgeries offer advantages of placing large interbody devices under direct compression and can be performed by a minimally invasive approach in the lateral position. Furthermore, simultaneous access to the anterior and posterior column is possible in the lateral position without the need for patient repositioning. The purpose of this study is to outline the anatomical and technical considerations for performing anterior lu

Fully automatic 3D reconstruction of the placenta and its peripheral vasculature in intrauterine fetal MRI.

Recent advances in fetal magnetic resonance imaging (MRI) open the door to improved detection and characterization of fetal and placental abnormalities. Since interpreting MRI data can be complex and ambiguous, there is a need for robust computational methods able to quantify placental anatomy (including its vasculature) and function. In this work, we propose a novel fully-automated method to segment the placenta and its peripheral blood vessels from fetal MRI. First, a super-resolution reconstruction of the uterus is generated by combining axial, sagittal and coronal views. The placenta is th

The Tonsillouvular Fissure Approach: Access to Dorsal and Lateral Aspects of the Fourth Ventricle.

OBJECTIVE: Although approaches to the fourth ventricle (FV) have been studied well, approaches to the lesions located in the dorsal and lateral aspects of the FV have not been shown in anatomic or clinical studies. The aim of this study is to show for the first time in the literature the tonsillouvular fissure approach (TUFA) in anatomic dissections and its use in surgical series. METHODS: For anatomic studies, 4 formalin-fixed human cadaveric heads infused with colored silicone and 10 cerebellar specimens were dissected in a stepwise manner. Records of 12 patients operated on via TUFA were a

Interspinalis-longissimus mid-line approach for thoracic and thoracolumbar minimally invasive fixation: Technical note.

The authors present a new minimally invasive posterior thoracic and upper lumbar spinal approach, using midline-staged incision with bilateral dissection via the natural muscular cleavage plane between the spinalis and longissimus muscles for pedicle fixation. The aim was to clarify anatomy and compare results between this technique and the conventional paraspinal approach. An anatomic MRI study determined the position of the superficial aponeurosis of the spinalis-longissimus intermuscular cleavage plane with respect to the midline. The surgical procedure is described. A radiological and clin

The size and shape of the foramen magnum in man.

BACKGROUND: The foramen magnum (FM) has garnered broad interest across the disciplines of anthropology, comparative anatomy, evolutionary biology, and clinical sciences. Most studies regarding the structure of the FM in humans have been intrapopulation morphometric studies rather than interpopulation morphologic studies. The few studies assessing the morphology of the foramen have utilized ambiguous and subjective descriptors to describe foraminal shape and are, consequently, difficult to reproduce. Therefore, detailed study of FM shape among craniofacially and geographically diverse populatio

The short appendix vermiformis as a risk factor for colorectal cancer.

The human appendix vermiformis is regarded as an evolutionarily vestigial organ, although it has presumptive immune system functions and appears to support beneficial bacterial gut flora, both of which could influence cancer progression. A review of the comparative anatomy of the mammalian appendix reveals a significantly longer appendix in herbivores than in carnivorous animals. The lengthier appendix vermiformis in herbivores has been associated with the presence of cellulose-digesting bacteria that colonize the structure. In light of recent studies that have reported the digestion of small

Mesenteric lymph: the bridge to future management of critical illness.

Toxic factors released from the intestine have been implicated in the pathophysiology of severe acute illness, including acute pancreatitis, trauma and hemorrhagic shock, and burns. Toxic factors in mesenteric lymph may induce an inflammatory systemic response while bypassing the portal circulation and liver. This paper reviews current knowledge of the anatomy, physiology and pathophysiology of mesenteric lymph and focuses on factors influencing its composition and flow, and potential therapeutic interventions. A search of the Ovid MEDLINE database up until the end of January 2006 yielded 1,76

Percutaneous cannulation for extracorporeal membrane oxygenation (ECMO): A method for pig experimental models.

Pigs are often used for experimental models of cardiogenic shock, cardiac arrest or acute lung injury with veno-arterial (VA) or veno-venous (VV) extracorporeal membrane oxygenation (ECMO) implementation. Percutaneous (as opposed to surgical) cannulation in experimental models has potential advantages, including, less surgical trauma or stressful stimuli and less bleeding complications when compared to open chest cannulation. However, pig anatomy can also be a challenge because of the deep location and angled anatomy of the femoral artery. The Seldinger technique and the use of a percutaneous

[Surface replacement shoulder: arthroplasty indications and limits].

Surface replacement arthroplasty of the shoulder offers the ability to restore anatomy and biomechanics of the glenohumeral joint, if sufficient humeral bone stock of at least 60% of the humeral head is preserved and no severe deformity is present. The implantation following the individual anatomy helps to avoid complications such as prosthetic malpositioning and periprosthetic fractures. While the use of a glenoid component is technically demanding, revision to a stem prosthesis is relatively easy because of bone stock preservation. Indications for humeral head resurfacing are osteoarthritis

Anatomy of the orbital apex and cavernous sinus on high-resolution magnetic resonance images.

Diseases of the orbital apex and cavernous sinus usually present with involvement of multiple cranial nerves, corresponding to the complex anatomy of the region. In nontraumatic disorders, magnetic resonance imaging is the diagnostic modality of choice. However, its capabilities can be fully used only with thorough knowledge of the complicated topographic relationships in this region. This article describes the imaging anatomy of the cranio-orbital junction and adjacent subarachnoid spaces. High-resolution magnetic resonance images of normal subjects are presented, and the results are compared

Anatomy of the Visual Word Form Area in Dyslexia.

The Visual Word Form Area (VWFA), located in ventral occipitotemporal cortex, plays a critical role in skilled reading. Researchers have theorized that the VWFA develops in its specific anatomical location due to the convergence of major white matter tracts and proximity to functionally similar regions. This suggests that precise anatomical positioning may be crucial for optimal VWFA function. Previous research has identified several functional differences in this region between typical and struggling readers (i.e. dyslexia): struggling readers show weaker text-selective responses and often ex

Endoscopic management of common bile duct stones using double balloon endoscopy in postoperative patients: evaluation of single-session complete stone clearance.

BACKGROUND: Endoscopic treatment of common bile duct (CBD) stones in patients with surgically altered anatomy is challenging because access to the duodenal papilla is often difficult. Although double balloon endoscopy-assisted ERCP (DB-ERCP) has become widespread, data on outcomes by reconstruction type remain limited. METHODS: A total of 218 patients treated between November 2016 and October 2024 were retrospectively reviewed. Among these, 212 patients were included: 159 with Roux-en-Y reconstruction (R group) and 53 with Billroth II reconstruction (B group). The primary endpoints were singl

Computed Tomographic Tenography of the Equine Carpal Flexor Tendon Sheath.

Exploratory tenoscopy is considered the gold standard technique to identify intrathecal pathology of the equine carpal flexor tendon sheath (CFTS). Preoperative diagnosis allows for more precise prognostication and surgical planning, potentially associated with improved surgical outcomes. This prospective, descriptive, anatomical study aimed to describe the anatomy of the equine CFTS using noncontrast and contrast CT in clinically normal cadaver limbs. Ten pairs of equine forelimbs free of gross external abnormalities were examined from mid-antebrachium to distal metacarpus. Specimens initiall

Automatic segmentation of male pelvic floor soft tissue structures for anatomical simulation and morphological assessment in lower rectal cancer surgery.

BACKGROUND: Pelvic anatomy is a complex network of organs that varies between individuals. Understanding the anatomy of individual patients is crucial for precise rectal cancer surgeries. Therefore, developing technology that can allow visualization of anatomy before surgery is necessary. This study aims to develop an auto-segmentation model of pelvic structures using AI technology and to evaluate the accuracy of the model toward preoperative anatomical understanding. METHODS: Data were collected from 63 male patients who underwent 3D MRI during a preoperative examination for colorectal and u

Impact of pulmonary vein anatomy and ostial dimensions on long-term outcome after single-shot device-guided cryoablation for paroxysmal atrial fibrillation.

BACKGROUND: Cryoballoon (CB)-guided pulmonary vein isolation (PVI) is an established treatment for atrial fibrillation (AF). This observational study aimed to assess the role of individual anatomical characteristics to predict long-term freedom from arrhythmia recurrence after CB-guided PVI for paroxysmal AF (PAF). METHODS: Three hundred fifty three consecutive patients (58 ± 11 years, 56% males), undergoing PVI between 2012 and 2018 were analysed. Individual pulmonary vein (PV) anatomy was assessed using preprocedural cardiac magnetic resonance imaging (MRI). For each PV, the cross-sectional

Cardiac Magnetic Resonance to Evaluate Percutaneous Pulmonary Valve Implantation in Children and Young Adults.

Experience with cardiac magnetic resonance to evaluate coronary arteries in children and young adult patients is limited. Because noninvasive imaging has advantages over coronary angiography, we compared the effectiveness of these techniques in patients who were being considered for percutaneous pulmonary valve implantation. We retrospectively reviewed the cases of 26 patients (mean age, 12.53 ± 4.85 yr; range, 5-25 yr), all of whom had previous right ventricular-to-pulmonary artery homografts. We studied T2-prepared whole-heart images for coronary anatomy, velocity-encoded cine images for ven

Educational utility of advanced three-dimensional virtual imaging in evaluating the anatomical configuration of the frontal recess.

BACKGROUND: Endoscopic sinus surgery represents a cornerstone in the professional development of otorhinolaryngology trainees. Mastery of these surgical skills requires an understanding of paranasal sinus and skull-base anatomy. The frontal sinus is associated with a wide range of variation and complex anatomical configuration, and thus represents an important challenge for all trainees performing endoscopic sinus surgery. METHODS: Forty-five otorhinolaryngology trainees and 20 medical school students from 5 academic institutions were enrolled and randomized into 1 of 2 groups. Each subject u

Prevalence and Anatomy of Retroesophageal Major Aortopulmonary Collateral Arteries.

BACKGROUND: Major aortopulmonary collateral arteries (MAPCAs) are the sole source of pulmonary blood flow in patients with pulmonary atresia and absent ductus arteriosus. The anatomy of MAPCAs can be highly variable, both in the number of MAPCAs supplying each lung and the anatomic origin and course of the MAPCAs. This study evaluated the prevalence and anatomy of retroesophageal MAPCAs in patients undergoing repair of pulmonary atresia/ventricular septal defect/MAPCAs. METHODS: This was a concurrent analysis of 68 consecutive patients (March 2013 through October 2015) undergoing a primary su

Does a Customized 3D Printing Plate Based on Virtual Reduction Facilitate the Restoration of Original Anatomy in Fractures?

The purpose of this study was to evaluate the restoration of original anatomy after fixation of sawbone fractures using case-specific 3D printing plates based on virtual reduction (VR). Three-dimensional models of 28 tibia sawbones with cortical marking holes were obtained. The sawbones were fractured at various locations of the shaft and 3D models were obtained. The fractured models were reduced virtually and customized non-locking metal plates that fit the reduced model were produced via 3D printing. The fractured sawbones were actually fixed to the customized plate with nonlocking screws an
