The sodium channel in membranes of electroplax. Binding of batrachotoxinin-a [(3)H]benzoate to particulate preparations from electric eel (electrophorus).

Batrachotoxinin-A [(3)H]benzoate ([(3)H]BTX-B) binds specifically and with high affinity (K(D) 48 nM) to sites (B(max) 2.1 pmol/mg protein) associated with voltage-dependent sodium channels in rodent brain vesicular preparations. High affinity binding requires the presence of scorpion (Leiurus) venom and a membrane potential. Local anesthetics antagonize the binding. Nonspecific binding is defined in the presence of veratridine. In particulate preparations from electroplax of the eel Electrophorus electricus, [(3)H]BTX-B binds with a K(D) of about 140 nM and a B(max) of 2.5 pmol/mg protein in

Allergy to dexchlorpheniramine. Study of a case.

INTRODUCTION: Dexchlorpheniramine (DH) is a classical or first generation antihistamine belonging to the ethanolamine group. Adverse effects related to these antihistamines are frequent, but the hypersensitivity reactions described in the literature since 1940 are exceptional. We report the case of a 32-year-old woman who experienced two episodes of akathisia secondary to intravenous (i.v.) dexchlorpheniramine administration for a possible hypersensitivity reaction to local anesthetics. MATERIAL AND METHODS: Allergological study consisted of the following tests: skin prick tests with routine

Management of postoperative pain by continuous epidural infusion of analgesics.

The anatomy and physiology of the epidural space and the mechanism of action, sites of action, and pharmacokinetics of analgesics administered by continuous epidural infusion are reviewed, and the efficacy, adverse effects, and postoperative indications for use of analgesics administered by this route are discussed. Narcotics selectively block pain conduction by occupying specific opiate receptors in the spinal cord. Local anesthetics provide analgesia by axonal membrane blockade; they also can produce nonselective sympathetic and somatic (sensory and motor) blockade in addition to analgesia.

Expression of Nav1.9 channels in human dental pulp and trigeminal ganglion.

There is a higher incidence of local anesthetic failure in endodontic patients experiencing pulpal hyperalgesia. Up-regulation of Nav1.9, a voltage-gated sodium channel isoform, might play a key role in local anesthetic failure because Nav1.9 channels increase neuronal excitability and have low sensitivity to blockade by local anesthetics. Immunocytochemistry was used to examine Nav1.9 channel expression in axons of symptomatic (painful) versus asymptomatic human dental pulp and to determine Nav1.9 expression levels in neuronal somata of the human trigeminal ganglion. Nav1.9 channel immunoreac

Minimizing pain in deer antler removal: Local anaesthetics in ZnO nanoparticle based collagen dressings as a promising solution.

Antler removal in deer is a common practice for various purposes, including meat production and traditional medicine. However, the current industry practice using lidocaine as a local anesthetic has limitations, such as short duration of action and the potential for postoperative infections. In this study, we investigated the performance of a ZnO collagen nanocomposites loaded with local anesthetics to improve wound management and alleviate pain associated with antler removal in red deer. The research involved the preparation of collagen nanocomposites with local anesthetics and testing the dr

Dexmedetomidine as an adjunct to peripheral nerve blocks in pediatric patients.

BACKGROUND: Combining local anesthetics with dexmedetomidine (DEX) for nerve blocks has been tested and confirmed as an alternative technique to strengthen analgesia in adults, but the evidence supported this proposal in pediatrics was not convincing enough. Thus, we intend to conduct a meta-analysis to make an appraisal of the effects of DEX as a local anesthetic adjuvant on peripheral nerve blocks in children. METHODS: The analysis has been registered on https://www.crd.york.ac.uk/prospero/ with the registration number CRD42021254029 on June 10th, 2021. PubMed, Ovid MEDLINE, Embase and Coch

Unlabeled topical anesthetic-induced severe methemoglobinemia in a pediatric burn patient.

This case report presents a patient with acquired methemoglobinemia due to unlabeled local anesthetic use. Some local anesthetics disrupt oxidative phosphorylation in erythrocytes, forming methemoglobin, which cannot transport oxygen. A 10-year-old boy with 60% total body surface area flame burns experienced sudden deterioration on Day 6 in the pediatric intensive care unit, presenting with cyanosis, tachycardia, hypotension, and SpO2 of 76% on room air, improving to 90% on 100% FiO2. A telemedicine consultation was requested, and evaluation revealed an elevated methemoglobin level of 22%. The

The Utilization of Ultrasound-Guided Regional Nerve Blocks in Anesthetic Management for Fracture Surgery.

Fracture surgeries are frequently accompanied by severe pain, necessitating efficacious pain management strategies to enhance postoperative recovery. Nerve block techniques, which are critical in mitigating pain, involve the targeted administration of local anesthetics to disrupt nerve signal transmission, thereby achieving significant analgesia. Traditionally, these techniques rely on anatomical landmarks and the clinician's expertise, which can introduce variability and potential risks. The adoption of ultrasound-guided nerve blocks has increased, facilitated by advancements in imaging techn

Neurological impairment during long-term intrathecal infusion of bupivacaine in cancer patients: a sign of spinal cord compression.

Adequate pain relief in patients with far advanced cancer sometimes requires intrathecal (IT) administration of a combination of opioids and local anesthetics. Tumor progression as well as the IT administration of local anesthetics can lead to neurologic dysfunction during treatment. Five patients showed symptoms of compression of the cauda equina or spinal cord shortly after the start of combined IT administration of morphine and bupivacaine in a dosage usually not associated with neurologic symptoms. Unexpectedly, neurologic evaluation suggested compression of the cauda equina and spinal cor

Delayed-type hypersensitivity (type IV) reactions in dental anesthesia.

The recommended methodology of evaluating patients who have presented with reactions to local anesthetics consists of epicutaneous skin testing and serial subcutaneous challenge. However, the role of type IV reactions in this group has been poorly documented. Epicutaneous routine testing and subcutaneous challenge to local anesthetic was performed, as well as patch testing and subcutaneous rechallenge of both at 24 and 48 hours with evaluation up to 72 hours was performed. Three patients presented with a history of localized edema after dental anesthesia. All had negative lidocaine and mepivac

Efficacy and safety of caudal dexmedetomidine in pediatric infra-umbilical surgery: a meta-analysis and trial-sequential analysis of randomized controlled trials.

BACKGROUND: Dexmedetomidine is used as a local-anesthetics adjuvant in caudal block to prolong analgesia in pediatric infra-umbilical surgery. OBJECTIVE: We evaluated the analgesic efficacy and safety of the addition of caudal dexmedetomidine to local anesthetics (vs local anesthetics alone) in pediatric infra-umbilical surgery. EVIDENCE REVIEW: We searched 10 databases for randomized controlled trials (RCTs) of pediatric patients undergoing infra-umbilical surgery, comparing caudal block with and without dexmedetomidine as local anesthetic adjuvant. We performed a frequentist random-effects

Influence of mexiletine on action potential discharge and conduction in nodose Adelta afferent neurons innervating guinea pig isolated trachea.

Local anesthetics are among the most effective peripherally acting antitussives. A complete understanding of their pharmacological properties in airway afferent neurons associated with the cough reflex has been hampered by an incomplete understanding of the contribution of various classes of afferent neuron to cough. The aim of this study was to evaluate the influence of the antitussive local anesthetic mexiletine on nodose ganglion-derived vagal afferent Adelta-fibers innervating guinea pig trachea. This distinct subtype of airway sensory neuron was recently shown to be involved in evoking co
