INTRODUCTION: Many drugs may provoke oesophageal disorders. In France, doxycyclin, potassium chloride non-steroidal anti-inflammatories are the most frequent agents incriminated.
OBSERVATION: A 27 year-old man consulted for dysphagia and odynophagia due to oesophagitis following administration of bacampicillin.
COMMENTS: Several drugs can provoke oesophageal disorders including ulcerations. Predisposing circumstances exist. Ingestion of the drugs with a sufficient quantity of water (60 to 100 ml), whilst standing or sitting should be able to limit this type of event.

At least 13% of the US adult population have chronic heartburn, and unknown percentages of others have related symptoms of gastroesophageal reflux disease. Studies show that most people with symptoms of reflux self-medicate and do not seek medical advice. Also, they fail to mention these symptoms when visiting their physician for other conditions. Therefore, the importance of thorough history taking cannot be overstated, and it may be prudent for primary care physicians to question all patients routinely about reflux symptoms.

Analysis of clinical data obtained in a double-blind randomized study, which compared liquid antacid (neutralizing capacity 120 mmol per day) with 1 g cimetidine in the treatment of 125 patients with gastric ulcer, revealed that, before starting treatment, 71% of the patients complained of epigastric pain, approximately 50% of bloating, and approximately 30% of nausea, heartburn, constipation or vomiting. It is assessed that heartburn with associated oesophagitis is experienced by 45% to 70% of pregnant women. Posture is the most constant associated factor; bile regurgitation through the pylorus is likely to be important in its aetiology but gastric acidity is not. Treatment with alkalis or dilute hydrochloric acid affords some relief in 95% of cases and patient acceptability is the most important factor in choice of preparation. Hiatus hernia is likely to be present in severe cases and rupture of the oseophagus has been reported. Early delivery may be advisable in the interest of the mother.
