After last year's anthrax attacks, EDs at Capital Health System in Trenton, NJ, had to address decontamination of large numbers of patients. Patients who didn't require decontamination were sent to the hospital's employee health facility so they could bypass the ED. Clinicians, nursing staff, educators, and paramedics were trained in the decontamination process. Direct communication with first responders was established with a portable scanner.

Bacillus anthracis, the agent responsible for inhalation anthrax, exerts its lethal effects via the production of anthrax toxin (protective antigen, lethal factor and oedema factor); anthrax kills because the toxin overwhelms the patient before innate host defence systems have a chance to eradicate the invaders. Structural studies on these three components provide a starting point for the design of novel drugs to neutralize toxin action, yielding a valuable adjunct to conventional antibiotic treatment.

Antrax infection, a major bacterial zoonosis caused by B. anthracis, affects animals, particulary the herbivores. The infection can be accidentally transmitted to man, in whom it has two forms. Cutaneous anthrax, more frequently encountered (95%), the transmission being favoured by the contact with contaminated animal or, after the sacrifice of the animal, with various contaminated products (skin, wool, hair, especially of goat, as well as bones, meat, blood); the evolution is favourable following treatment. Internal (visceral)--pulmonary, gastro-intestinal, meningo-encephalytic--anthrax cause…

BACKGROUND: Anthrax is an acute infectious disease caused by the spore-forming bacterium Bacillus anthracis. Due to the current world threat of unpredictable biological terrorism, the Department of Defense has mandated the systematic vaccination of all US military personnel against this warfare agent. Many may experience al mild flu-like illness and soreness at the injection site, but systemic reactions are rare.
CASE REPORT: We report a delayed and potentially serious life-threatening adverse reaction to anthrax vaccine. A previously healthy 34-year-old male was transported to the emergency …

PURPOSE: To provide nurse practitioners (NPs) with a basic understanding of clinical presentation, transmission, diagnosis, pharmacological treatment, and post-exposure prophylaxis of inhalational anthrax.
DATA SOURCES: Selected research and clinical articles and government guidelines.
CONCLUSIONS: Inhalational anthrax has an incubation period of 1 to 6 days and is very difficult to diagnose early. The chest radiograph consistently reveals a widened mediastinum and pleural effusion without infiltrates. Mortality for inhalational anthrax is high, despite aggressive treatment after onset of sy…

We examined the entry of anthrax edema toxin (EdTx) into polarized human T84 epithelial cells using cyclic AMP-regulated Cl- secretion as an index of toxin entry. EdTx is a binary A/B toxin which self assembles at the cell surface from anthrax edema factor and protective antigen (PA). PA binds to cell surface receptors and delivers EF, an adenylate cyclase, to the cytosol. EdTx elicited a strong Cl- secretory response when it was applied to the basolateral surface of T84 cells but no response when it was applied to the apical surface. PA alone had no effect when it was applied to either surfac…

Anthrax is an acute infectious disease caused by the spore-forming bacterium Bacillus anthracis. Anthrax is most common in agricultural regions, where it occurs in animals. It can also infect humans. Cutaneous anthrax infections occur when the bacterium enters a cut or abrasion on the skin. A case of cutaneous anthrax infection of the arm is presented. The patient needed to undergo a skin graft. He subsequently developed an ulnar nerve lesion after severe edema in his arm and hand.

Report of 5 cases of palpebral anthrax treated after the acute phase, in a cicatricial or precicatricial stade. The authors recommend to delay plastic surgery up to three months and to select total skin graft for upper eye-lid and pediculed skin flap for lower eye-lid.

In September 2024, the ninth documented case of welder's anthrax was identified in a previously healthy male welder, aged 18 years, from Louisiana, who was hospitalized with pneumonia and respiratory failure requiring intubation and mechanical ventilation. Welder's anthrax is a recently described life-threatening pneumonia caused by infection with anthrax toxin-producing Bacillus cereus group bacteria; risk factors for infection are not well-understood. Eight previous cases (six fatal) were reported among welders or metalworkers from Louisiana and Texas. A coordinated state and federal respons…

Anthrax lethal toxin (LT) and edema toxin (ET) are two of the major virulence factors of Bacillus anthracis, the causative pathogen of anthrax disease. While the roles of LT in anthrax pathogenesis have been extensively studied, the pathogenic mechanism of ET remains poorly understood. ET is a calmodulin-dependent adenylate cyclase that elevates intracellular cAMP by converting ATP to cAMP. Thus, it was postulated that the ET-induced in vivo toxicity is mediated by certain cAMP-dependent events. However, mechanisms linking cAMP elevation and ET-induced damage have not been established. Cholera…
