Urinary tract infection.

The urinary tract is a common source for life-threatening infections. Most patients with sepsis or septic shock from a urinary source have complicated urinary tract infection. This article explains the epidemiology, risk factors, and treatment. Effective management, appropriate collection of microbiology specimens, prompt initiation of antimicrobial therapy, source control, and supportive therapy are described.

Prophylaxis Options in Children With a History of Recurrent Urinary Tract Infections: A Systematic Review.

CONTEXT: The prevention of urinary tract infection recurrence (UTI) in children has been a challenge yet to be solved. Current practice in children with recurrent UTI (RUTI) suggests that antibiotic prophylaxis may prevent further episodes of UTI and future complications. OBJECTIVE: To conduct a systematic review and meta-analysis of randomized controlled trials comparing prophylaxis options for the prevention of UTI and kidney scarring in children with a history of RUTI. DATA SOURCES: We conducted a systematic literature search through major electronic databases (PubMed/Medline, Scopus and

Reversible lung fibrosis in a 6-year-old girl after long term nitrofurantoin treatment.

BACKGROUND: Pulmonary side effects are well known, including lung fibrosis, in elderly patients treated with long-term nitrofurantoin to prevent urinary tract infections and secondary renal injury. However, pulmonary side effects have only been reported rarely in paediatric cases, despite nitrofurantoin being a first line prophylactic treatment of recurrent childhood urinary tract infection. CASE PRESENTATIONS: A 6-year-old girl was admitted to the hospital with dyspnea, general fatigue, loss of appetite and need for nasal oxygen treatment after long-term nitrofurantoin treatment. A computed

Bacterial colonization of the prepuce in boys with vesicoureteral reflux who receive antibiotic prophylaxis.

Culture swabs were taken from the periurethral area of 59 consecutive uncircumcised boys with vesicoureteral reflux (VUR) and 36 boys undergoing circumcision. A pure growth of a uropathogen was isolated in 22 (37%) boys with VUR who were receiving antibiotic prophylaxis and in 10 (28%) boys who underwent circumcision. Antibiotic prophylaxis in patients with VUR was not effective in reducing the bacterial colonization of the prepuce. We recommend circumcision in boys, particularly infants with VUR, to reduce the risk of urinary tract infections.

Genomics reveal Staphylococcus aureus persists during long-term urinary catheterization despite antimicrobial therapy and catheter exchanges.

Urinary catheters, the most frequently placed medical devices in the US, increase the risk of developing symptomatic catheter-associated urinary tract infection (CAUTI) and asymptomatic bacteriuria (ASB) - the presence of bacteria in the urine - with those requiring long-term urinary catheters (LTUCs) at highest risk. While ASB and CAUTI are caused by a broad range of uropathogens, most remain understudied. We use whole-genome sequencing to investigate the understudied uropathogen, Staphylococcus aureus. Analysis of 153 longitudinal S. aureus isolates previously collected from urinary catheter

Inhibition of adherence of multi-drug resistant E. coli by proanthocyanidin.

Proanthocyanidin is commonly used for inhibiting urinary tract infection (UTI) of sensitive strains of Escherichia coli. The aim of this study was to investigate the effect of proanthocyanidin on adherence of uropathogenic multi-drug resistant E. coli to uroepithelial cells, which has not yet been investigated so far. Extracts of the purified proanthocyanidin were prepared from dried cranberry juice. Purity and structural assignment of proanthocyanidin was assessed using high performance liquid chromatography and (13)C nuclear magnetic resonance spectroscopy, respectively. Subsequently, its af

Increased rates of trimethoprim resistance in uncomplicated urinary tract infection: cause for concern?

AIMS: To assess changes in trimethoprim resistance over 2 years in bacteria causing uncomplicated urinary tract infections (UTIs) presenting to a representative group of general practitioners (GPs) in Christchurch. METHODS: Seventy-six randomly selected GPs in Christchurch (the Christchurch Sentinel network) participated in the study. Using the same methodology as in the previously reported 2000 collection, midstream urine (MSU) samples were prospectively collected for standard microbiological analysis on all women between the ages of 16 and 50 years presenting with symptoms of dysuria and fr

A model of polymicrobial catheter-associated urinary tract infection reveals biofilm-mediated modulation of treatment efficacy.

BACKGROUND: Catheter-associated urinary tract infections (CAUTI) are among the most prevalent healthcare-associated infections and an important site for development and spread of antimicrobial resistance. Although CAUTI are frequently polymicrobial, the majority of research focuses on individual pathogens in monoculture, largely due to a lack of representative and tractable models. AIMS: The aim of this study was to develop a tractable and reproducible model of polymicrobial CAUTI. METHODS: Here we describe the use of an in vitro model of the catheterized urinary tract to generate polymicrob

In vitro activity of nitroxoline against carbapenem-resistant Acinetobacter baumannii isolated from the urinary tract.

BACKGROUND: The old antimicrobial nitroxoline is currently repurposed for oral treatment of uncomplicated urinary tract infections (UTIs). OBJECTIVES: To investigate the in vitro activity of nitroxoline against carbapenem-resistant Acinetobacter baumannii (CRAb). METHODS: From an international collection of previously well-characterized clinical A. baumannii isolates, 34 isolates from urinary tract sources with different carbapenem-resistance mechanisms were selected. Nitroxoline activity was analysed with broth microdilution (BMD), disc diffusion (DD) and within an in vitro biofilm model. M

Antibiotic prophylaxis in the management of vesicoureteric reflux: a randomized double-blind placebo-controlled trial.

BACKGROUND: The benefits of long-term low-dose antibiotics in preventing urinary tract infection (UTI) and renal damage in children with primary vesicoureteric reflux (VUR) are unclear. METHODS: Children aged between 1 and 12 years with VUR grade I-IV and a microbiologically proven UTI were randomized into two groups to receive either antibiotic prophylaxis [2 mg/kg trimethoprim + sulfamethoxazole (TMP-SMX)] daily or placebo, respectively, for 12 months. Primary outcome was microbiologically confirmed symptomatic UTI. Intention-to-treat analysis using time-to-event data was performed. RESULT
