Aphakic retinal detachment.

The incidence of aphakia in our series of retinal detachment is 14.82%. The incidence of retinal detachment in aphakia is 2.74%. The use of alpha-chymotrypsin does not influence this incidence. In our own patients, we did not find a sex predominance, although there was a predominance of the male sex in the referred group. The mean age of the patients affected by aphakic retinal detachment was 65.87 years and 82.6% of the cases occurred above the age of 56 years. There was no statistical significant difference between the right and the left eye involvement. Finally, 38.37% of the detachments ap…

Myopic and nonmyopic aphakic retinal detachment: time interval and location of breaks.

Of 471 retinal detachments occurring in aphakic patients after undergoing intracapsular cataract extraction, we reviewed 318 eyes with uncomplicated aphakia (no vitreous loss). Myopic eyes had equatorial breaks 38% of the time versus 19% for nonmyopic eyes. The time interval between aphakia and detachment was shorter for myopic than for nonmyopic eyes. Our results showed a significant incidence of: (1) equatorial breaks in myopic eyes versus nonmyopic eyes (P less than 0.005), and (2) earlier detachments for myopic eyes versus nonmyopic eyes (P less than 0.005). The presence of equatorial brea…

Retrospective study of secondary implantation of retroiridial fixation lenses in patients without capsular support: functional results and complications.

INTRODUCTION: The correction of aphakia when there is no adequate capsular support remains a therapeutic challenge. The use of retroiridial fixation lenses has been extended given their lower complication rate compared to other available options. MATERIAL AND METHODS: Retrospective study including all cases operated with an Artisan® retropupillary aphakia implant. RESULTS: 33 eyes were included out of a total of 28 patients. The follow-up period has been 38.55 months (1-96). 32.3% had a lens dislocation and 67.7% had an intraocular lens dislocation. The previous mean best corrected visual ac…

Intractable diplopia after vision restoration in unilateral cataract.

Twenty-four patients lost their ability to fuse when their binocular function was disrupted for at least 2 1/2 years by a unilateral traumatic cataract or a unilateral traumatic cataract followed by uncorrected aphakia. Three patients were 6 years old, one was 8 years old, and the remaining 20 patients were aged 10 years or older at the time of the injury. All patients had intractable diplopia when the cataract was removed and the aphakia corrected. Aniseikonia was not the cause of this inability to fuse and the insertion of an intraocular lens provided no relief. The prognosis for the elimina…

Corneal toxicity after Ozurdex(®) migration into anterior chamber.

OBJECTIVE: To describe a case of corneal toxicity after migration of a dexamethasone implant into the anterior chamber. CASE REPORT: A 62-year-old man with aphakia and a history of vitrectomy received a dexamethasone implant for a refractory Irvine-Gass syndrome. Thirty days later, the implant migrated into the anterior chamber causing endothelial contact with secondary corneal oedema that justified the removal of the implant without resolution of the oedema. DISCUSSION: Clinical tolerability to dislocated implant is poor in cases with pre-existing corneal oedema, and because of this, it mus…

A New Technique with Scleral Grooves for Sutureless Scleral Fixation of the Carlevale Intraocular Lens.

INTRODUCTION: A novel technique is described employing scleral grooves to facilitate sutureless scleral fixation of the Carlevale intraocular lens (CIL). We describe its use in a series of 47 patients with aphakia. METHODS: A retrospective study of all patients with aphakia who underwent CIL implantation with the new technique by a single surgeon during 1 year. The novelty of this technique consists in the creation of two partial-thickness linear sclerotomies (grooves), 180° apart, 2 mm from and parallel to the limbus. In the middle of each groove a full-thickness sclerotomy is performed to f…

Reper intraocular lens with artificial iris: implantation techniques and outcomes.

The authors describe the surgical techniques to implant a novel intraocular lens with artificial iris (Reper) in patients affected by traumatic aphakia and aniridia. Two surgical approaches are proposed: implantation by suture-loaded cartridge and "open sky" implantation combined with penetrating keratoplasty. The method of establishing the exact position of the scleral fixation points and the pre-assembly of the suture of Reper before its implantation are the main novel proposed surgical procedures. The rationale is to minimize surgical procedures in the anterior chamber and to prevent knot l…

Scleral Fixated Intraocular Lens in Aphakic Patient with Bilateral Microcornea and Microphthalmia.

We are reporting a case of a 22-year-old lady with bilateral microphthalmia and microcornea, in which a modified technique for sutureless scleral fixated intraocular lens implantation provided a successful aphakic rehabilitation alternative with a good visual outcome and significant improvement in quality of life. Management of aphakia in microphthalmic eyes is challenging due to the anatomical abnormalities and limited literature on managing such cases. Visual rehabilitation for aphakia using contact lenses is limited by intolerance and poor lens fitting. Significant optical aberrations may l…

Management of cataracts in infancy and childhood.

In recent years there has been a considerable improvement in our understanding of the mechanism of stimulus deprivation amblyopia and a continuous development of better surgical techniques for the removal of cataracts in very young patients. In spite of these advances and the fact that modern constant wear soft contact lenses are now a reliable proposition, there has not been a parallel improvement in the visual results achieved after cataract extraction and aphakia continues to present considerable problems of management in children. This study analyses over 100 cases treated at Moorfields Ey…

Endothelial morphology related to disease activity in human corneas.

Twenty normal and four groups of pathological corneas with the diagnoses aphakia, macula after herpetic keratitis. Fuchs' endothelial dystrophy and graft rejection were stained with trypane blue and alizarin red. The morphology of the endothelium was described in terms of cell density, coefficient of variation for cell area, percentage of hexagonal cells, percentage of joint meetings of more than three cells, nuclei per cell and nuclear shape. The groups of aphakia, keratitis, Fuchs' dystrophy and graft rejection were considered to represent increasing degrees of endothelial disease activity a…

The Outcome of Artisan Intraocular Lens Implantation in Children: A Narrative Review.

Aphakia is a condition in which the eye's crystalline lens is not in its proper position because of a perforating injury, surgical removal, dislocation of the lens, or congenital anomaly. The management of aphakia can be either conservative or surgical. Various surgical techniques could be used, including retro pupillary-fixated iris-claw intraocular lenses (IOLs) and anterior-fixated iris-claw IOLs. One of the challenges faced by ophthalmologists is the optical rehabilitation of pediatric aphakic patients because a child's eye is still growing, resulting in fundamental variations in their ref…

[Influence of material on biocompatibility of intraocular lenses].

There are many types of intraocular lenses (IOL), different in optic features and materials. The first indication for IOL implantation was aphakia after cataract surgery. The problem of aphakia exists as long as cataract is treated, that is at least 45 hundreds of years. Correction of aphakia using glasses is uncomfortable for patients and even impossible in some cases. The invention of an artificial intraocular lens was a crucial event in the history of ophthalmology. Nowadays there are more indications for cataract surgery and implantation of intraocular lens, including correction of refract…
