Among the different types of arthritis associated with meningococcal disease, isolated primary meningococcal arthritis is unusual. A case of isolated group C meningococcal septic arthritis in an HIV negative homosexual male is described and its possible implication discussed.

Sexually transmitted infections may provoke a wide variety of rheumatic lesions. Disseminated N. gonorrhoeae infection leads to septic arthritis, which may be rapidly destructive but which responds promptly to appropriate antibiotic therapy. In the case reported, the diagnosis of gonococcal arthritis unfolded over the course of 1 week and was not fully conclusive until presumptive therapy was initiated. Although the clinical, microbiologic, and immunologic characteristics of gonococcal arthritis can be differentiated from other types of bacterial arthritides, not all textbook symptoms are present at one time in one particular case. We describe a patient who underwent inadvertent gastroileostomy, and subsequently developed malabsorption, granulomatous hepatitis, arthritis, and reactivation of tuberculosis accompanied by circulating immune complexes and lowered C1q. Surgical correction resulted in marked improvement, with disappearance of arthritis, return to normal of complement levels, and absence of demonstrable circulating immune complexes. Arthritis has not been documented in the spectrum of abnormalities described after gastroileostomy.

Lyme disease is a newly recognized, multi-system disorder that may be associated with chronic arthritis. A bacteriology technologist was found to have acute polyarticular arthritis after a brief prodromal illness. Gram's stain of fluid from the right knee showed pleomorphic gram-negative organisms, while that of fluid from the right elbow and left wrist showed gram-negative cocci and diplococci. Six cases of Lancefield Group G streptococcocal arthritis are described. Two cases had pre-existing chronic arthritis (one rheumatoid) with infection of a joint prosthesis. Three cases had neoplastic disease before or at the same time as septic arthritis. Skin reactions, including cellulitis and scarlatiniform rash were prominent in five cases. One patient may have acquired a Group G streptococcus from her dog. Five cases responded well to penicillin and the sixth who was allergic to that drug was cured by erythromycin. Cervical spondylosis may be the most common cause of arm pain in the elderly. Although patients describe this pain as "severe," they seem able to ease it by massaging the area, which is not the case with other painful rheumatoid conditions. Elevated serum uric acid in an arthritis patient is not always a sign of gout. A definite diagnosis of gout can be made only when urate crystals are found in the joint fluid.

Pasteurella multocida, a small gram-negative rod, is a domestic animal saprophyte that occasionally causes disease in humans. Infectious arthritis may develop from a superficial animal bite or scratch. Nine previous cases of infectious arthritis due to this organism have been documented in the literature, and a tenth case is reported here. A middle-aged man developed multiple subcutaneous rheumatoid granulomata, high titer of rheumatoid factor, diffuse interstitial pulmonary fibrosis and digital clubbing in the absence of clinicoradiological evidence of synovial disease. This patient supports the concept of rheumatoid disease without arthritis.
