Sleep Disorder and Heart Failure with Preserved Ejection Fraction.

It is generally considered that obstructive sleep apnea is a potential cause of heart failure (HF), and insomnia and central sleep apnea are results of HF. However, the number of reports describing the bidirectional relationship between sleep disorder and HF has increased. Sleep disorder may contribute to left ventricular diastolic dysfunction via left atrial overload, left ventricular remodeling, pulmonary hypertension, and atrial fibrillation, which lead to HF with preserved left ventricular ejection fraction. Overnight rostral fluid shift and lung congestion may lead to airflow obstruction

Clinical Risks and Underlying Mechanisms of Atrial Fibrillation after Myocardial Infarction.

Atrial Fibrillation (AF) following Myocardial Infarction (MI) is a common clinical complication that significantly impairs patient prognosis and quality of life. This review examines the clinical risks and underlying pathophysiological mechanisms of post-MI AF. The pathophysiology of AF after MI involves multiple maladaptive processes, particularly atrial structural and electrical remodeling, which collectively establish the arrhythmogenic substrate. This review summarizes the molecular mechanisms contributing to AF in the post-infarction setting. Furthermore, it was synthesized current eviden

Clinical Implications of Complete Left-Sided Reverse Remodeling With Cardiac Resynchronization Therapy: A MADIT-CRT Substudy.

BACKGROUND: Clinical implications of complete left-sided reverse remodeling due to cardiac resynchronization therapy with a defibrillator (CRT-D), defined as reduction in both left ventricular end-systolic volume (LVESV) and left atrial volume (LAV), are unknown. OBJECTIVES: This study aimed to evaluate the rate and predictive value of complete left-sided reverse remodeling on heart failure (HF) and death events in CRT-D patients with left bundle branch block (LBBB) enrolled in MADIT-CRT (Multicenter Automatic Defibrillator Implantation Trial With Cardiac Resynchronization Therapy). METHODS:

Cardiac reverse remodeling in a mouse model with many phenotypical features of heart failure with preserved ejection fraction: effects of modifying lifestyle.

Multiple factors cause heart failure with preserved ejection fraction (HFpEF) and involve various systems. HFpEF prevalence is rapidly rising, and its prognosis remains poor after the first hospitalization. Adopting a more active lifestyle has been shown to provide beneficial clinical outcomes for patients with HFpEF. Using a two-hit HfpEF murine model, we studied cardiac reverse remodeling (RR) after stopping the causing stress and introducing voluntary exercise (VE). We checked in 2-mo-old male and female C57Bl6/J mice the heart's response to angiotensin II (ANG II; 1.5 mg/kg/day for 28 days

Subclinical Primary Aldosteronism and Cardiovascular Health: A Population-Based Cohort Study.

BACKGROUND: Primary aldosteronism, characterized by overt renin-independent aldosterone production, is a common but underrecognized form of hypertension and cardiovascular disease. Growing evidence suggests that milder and subclinical forms of primary aldosteronism are highly prevalent, yet their contribution to cardiovascular disease is not well characterized. METHODS: This prospective study included 1284 participants between the ages of 40 and 69 years from the randomly sampled population-based CARTaGENE cohort (Québec, Canada). Regression models were used to analyze associations of aldoste

Vaspin Ameliorates Cardiac Remodeling by Suppressing Phosphoinositide 3-Kinase/Protein Kinase B Pathway to Improve Oxidative Stress in Heart Failure Rats.

This study aimed to explore whether vaspin could alleviate cardiac remodeling through attenuating oxidative stress in heart failure rats and to determine the associated signaling pathway. Cardiac remodeling was induced by myocardial infarction, transverse aortic constriction, or angiotensin (Ang) II infusion in vivo, and the neonatal rat cardiomyocytes (NRCMs) and neonatal rat cardiac fibroblasts (NRCFs) were treated with Ang II. Vaspin treatment alleviated fibrosis in myocardial infarction, transverse aortic constriction, and Ang II-treated rats. The Ang II-induced increases of atrial natriur

Nicotinamide Riboside Alleviates Cardiac Dysfunction and Remodeling in Pressure Overload Cardiac Hypertrophy.

BACKGROUND: Cardiac hypertrophy is a compensatory response to pressure overload, which eventually leads to heart failure. The current study explored the protective effect of nicotinamide riboside (NR), a NAD+ booster that may be administered through the diet, on the occurrence of myocardial hypertrophy and revealed details of its underlying mechanism. METHODS: Transverse aortic constriction (TAC) surgery was performed to establish a murine model of myocardial hypertrophy. Mice were randomly divided into four groups: sham, TAC, sham+NR, and TAC+NR. NR treatment was given daily by oral gavage.

Clonal hematopoiesis of indeterminate potential and atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained arrhythmia. Clonal hematopoiesis of indeterminate potential, characterized by the clon … inflammation is a central driver that promotes abnormal calcium handling, which further accelerates atrial remodeling. For specific mutations, TET2 mutations correlate strongest with AF, and other mutations in genes, such as ASXL1, JAK2, TP53, and PPM1D, and spliceosomes, may also modulate AF susceptibility, although their precise roles require further investigatio …

Histone deacetylase 2-dependent ventricular electrical remodeling in a porcine model of early heart failure.

AIMS: Heart failure (HF) is linked to electrical remodeling that promotes ventricular arrhythmias. Underlying molecular signaling is insufficiently understood, in particular concerning patients with early disease stages. Previous observations suggest a key role for epigenetic mechanisms in cardiac remodeling processes. We hypothesized that histone deacetylases (HDACs) 1 and 2 contribute to cellular electrophysiological dysregulation in ventricular cardiomyocytes during HF development. MATERIALS AND METHODS: HDAC and ion channel expression was quantified in a porcine model of early HF induced

Giant Left Atrial Thrombus Despite Anticoagulation with Apixaban in a Patient with Mitral Stenosis and Atrial Fibrillation.

BACKGROUND In patients with atrial fibrillation (AF), the presence of a left atrial thrombus correlates with the highest risk of stroke. Mitral stenosis (MS) is an acquired disease that leads to atrial pressure overload and subsequent significant anatomical and electrical remodeling of the left atrium. This promotes the occurrence of AF and atrial thrombus formation. Proper anticoagulation decreases the stroke risk in AF patients. Unfortunately, there is insufficient data on the effectiveness of non-vitamin K antagonist oral anticoagulants (NOAC) in patients with AF and MS. CASE REPORT We pres

New insights into the molecular mechanisms of SGLT2 inhibitors on ventricular remodeling.

Ventricular remodeling is a pathological process of ventricular response to continuous stimuli such as pressure overload, ischemia or ischemia-reperfusion, which can lead to the change of cardiac structure and function structure, which is central to the pathophysiology of heart failure (HF) and is an established prognostic factor in patients with HF. Sodium glucose cotransporter 2 inhibitors (SGLT2i) get a new hypoglycemic drug that inhibit sodium glucose coconspirator on renal tubular epithelial cells. Recently, clinical trials increasingly and animal experiments increasingly have shown that

Percutaneous Management of Recurrent Postsurgical Right Atrial Cicatricial Stenosis Causing Supratricuspid Obstruction.

BACKGROUND: Postsurgical right atrial cicatricial stenosis is a rare iatrogenic complication following extensive atrial reconstruction. When located above the tricuspid valve, this fibrotic tissue may cause supratricuspid obstruction, producing symptoms and hemodynamic findings similar to tricuspid stenosis. CASE SUMMARY: A 41-year-old man with a history of complex atrial reconstruction using bovine pericardium and a Dacron graft presented with progressive dyspnea 1 year postoperatively. Echocardiography revealed a cicatricial supratricuspid membrane with a peak gradient of 18 mm Hg. Percutan

Pharmacotherapy for atrial arrhythmias: present and future.

Atrial fibrillation (AF) is the most commonly encountered arrhythmia in clinical practice. It is associated with significant morbidity, including palpitations, exercise intolerance, congestive heart failure, and increased risk of embolic stroke. Mortality is increased twofold in patients with AF. Management of AF with antiarrhythmic drugs traditionally has been hindered by lack of efficacy, poor tolerance of side effects, drug-associated toxicity, and proarrhythmic potential. Improved understanding of atrial electrical and structural remodeling as well as advances in rational drug design have

Aerobic exercise reduces lipid accumulation via ACLY-mediated metabolic remodeling to alleviate cardiac remodeling.

BACKGROUND: Pathological cardiac remodeling, a key contributor to heart failure, is characterized by significant reprogramming of cardiac energy metabolism. ATP-citrate lyase (ACLY), which connects glucose metabolism with lipid synthesis, plays an important regulatory role in this process. However, the role of ACLY in exercise-induced metabolic remodeling and cardiac remodeling attenuation remains unclear. METHODS: Cardiac remodeling was induced in H9c2 cells with isoproterenol or urea, or in rats by left anterior descending artery (LAD) ligation or mice by isoproterenol. Interventions includ

Clinical and imaging phenotypes.

Functional/Secondary tricuspid regurgitation (STR) accounts for over 85% of clinically significant tricuspid regurgitation (TR) and is associated with adverse prognosis and impaired quality of life. Advances in percutaneous tricuspid valve (TV) interventions underscore the need to differentiate TR aetiologies, mechanisms, and phenotypes. STR is subdivided into atrial (A-STR), caused by right atrial dilation and tricuspid annular enlargement without significant leaflet tethering, and ventricular (V-STR), resulting from right ventricular dilation/dysfunction with leaflet tethering. A-STR, increa

New aspects of endocrine control of atrial fibrillation and possibilities for clinical translation.

Hormones are potent endo-, para-, and autocrine endogenous regulators of the function of multiple organs, including the heart. Endocrine dysfunction promotes a number of cardiovascular diseases, including atrial fibrillation (AF). While the heart is a target for endocrine regulation, it is also an active endocrine organ itself, secreting a number of important bioactive hormones that convey significant endocrine effects, but also through para-/autocrine actions, actively participate in cardiac self-regulation. The hormones regulating heart-function work in concert to support myocardial performa
